Book excerpt: Lawrence Livermore National Laboratory (LLNL) and the NNSA Livermore Site Office teamed up to prepare a site specific work smart standard setting requirements for preparation of safety basis documents for LLNL non-nuclear operations and facilities. This standard documents how all hazards (biological, chemical, explosive, industrial, and radiological) shall be evaluated, classified, analyzed, and controls developed. This standard was developed to evaluate hazards at the facility level to mesh with LLNL's ISM system for reviewing hazards at the activity level. This standard presents an approach to establishing safety basis for non-nuclear operations and activities, taking a graded approach based on the potential for impacts to the health of collocated workers and the public. Direct worker safety is covered by LLNL's work activity level reviews and requirements. This standard includes streamlined mechanisms for classifying hazards based upon the unmitigated potential for human health impacts. A review or practices at several private industries, government laboratories, and DOE complex sites provided a benchmark and comparison of safety analysis processes. These approaches were compared with LLNL's existing systems, leading to a determination that facility specific safety basis documents added value to a rapid authorization for new work activities in LLNL facilities. A process for hazard classification that would be viewed as more credible than the previous facility classification system was developed, including a method allowing correlation of chemical inventories with TEEL* concentrations. A graded approach for classification of explosive hazards, consistent with the DOE Explosive Safety Manual, was included. The standard was designed to be complementary with LLNL's existing work smart standards covering the hazards identified in a facility. A standard for LLNL's Work Smart Standard set was prepared that will assure all hazards are covered with appropriate levels of analysis to provide a safety basis for non-nuclear facilities and operations. Other sites may find the overall approach a credible mechanism with a graded approach to classification and hazard and accident analysis to provide a safety basis for non-nuclear facilities and operations.

Book excerpt: The objective of this paper is to introduce the process and philosophies used to implement the new Work Smart Standard (WSS), ''Safety Basis Requirements for Nonnuclear Facilities at Lawrence Livermore National Laboratory Site Specific Standard'' (UCRL-ID-150214), approved in 2003 and revised January, 2004. This work relates directly to the following workshop theme: ''Improvements in Chemical, Biological, and Non-nuclear Safety Analysis.'' This paper will describe the approach used to implement the new nonnuclear standard at LLNL and corresponding guidance manual: ES & H Manual, Document 3.1. The varied activities can be broken down into three main parts: (1) Implementation Plan Schedule. The Implementation Plan includes the due dates for revising nonnuclear facility safety analysis documentation to meet the new standard. Implementation of the new methodology is being phased over a 4-year period. Each directorate was tasked to schedule the revision date for each of their nonnuclear facilities, using agreed upon priority-ranking criteria. (2) Program Infrastructure. This includes the development of training courses, procedures, a website and tools required to perform the work (i.e. Q List, de minimus list) or tools helpful to perform the work; such as a program to automate the classification of chemical inventories and establish maximum facility inventory limits (MFILs). (3) Delegation request packages. NNSA agreed to grant delegation to LLNL for local approval of nonnuclear safety basis documents (SBDs) in a phased manner. The first package submitted was for the Tier 1 (or Screening Report SBDs), the next is for the Tier 2 SBDs, and the last package will include the Tier 3 SBDs. The packages generally include 2-4 example SBDs at the level for which NNSA delegation is being sought, relevant training course material, and applicable procedures. The Implementation Plan was approved by LLNL's Deputy Director of Operations (DDO) in August 2004 and will be reviewed, updated, and re-approved, as necessary, on a 6-month basis. All listed program infrastructure items are in place, except the Chemical Classification tool (which is 90% complete). Full delegation approval for the Tier 1 SBDs is anticipated from NNSA in early May, 2005, and the Tier 2 SBD package is 90% complete and will be submitted to NNSA in the near future. This paper illustrates the myriad of tasks necessary to implement a new Nonnuclear program, and provides a strategy for implementation that includes input from stakeholders and also provides the necessary guidance, training and tools needed to achieve a smooth transition.

Book excerpt: The safety policy of LLNL is to take every reasonable precaution in the performance of work to protect the environment and the health and safety of employees and the public, and to prevent property damage. With respect to hazardous agents, this protection is provided by limiting human exposures, releases to the environment, and contamination of property to levels that are as low as reasonably achievable (ALARA). It is the intent of this Plan to supply the broad outline for completing environmental investigations within ALARA guidelines. It may not be possible to determine actual working conditions in advance of the work; therefore, planning must allow the opportunity to provide a range of protection based upon actual working conditions. Requirements will be the least restrictive possible for a given set of circumstances, such that work can be completed in an efficient and timely fashion. Due to the relatively large size of the LLNL Site and the different types of activities underway, site-specific Operational Safety Procedures (OSPs) will be prepared to supplement activities not covered by this Plan. These site-specific OSPs provide the detailed information for each specific activity and act as an addendum to this Plan, which provides the general plan for LLNL Main Site operation.

Book excerpt: This revision of the LLNL Fire Protection Baseline Needs Assessment (BNA) was prepared by John A. Sharry, LLNL Fire Marshal and LLNL Division Leader for Fire Protection and reviewed by Martin Gresho, Sandia/CA Fire Marshal. The document follows and expands upon the format and contents of the DOE Model Fire Protection Baseline Capabilities Assessment document contained on the DOE Fire Protection Web Site, but only address emergency response. The original LLNL BNA was created on April 23, 1997 as a means of collecting all requirements concerning emergency response capabilities at LLNL (including response to emergencies at Sandia/CA) into one BNA document. The original BNA documented the basis for emergency response, emergency personnel staffing, and emergency response equipment over the years. The BNA has been updated and reissued five times since in 1998, 1999, 2000, 2002, and 2004. A significant format change was performed in the 2004 update of the BNA in that it was 'zero based.' Starting with the requirement documents, the 2004 BNA evaluated the requirements, and determined minimum needs without regard to previous evaluations. This 2010 update maintains the same basic format and requirements as the 2004 BNA. In this 2010 BNA, as in the previous BNA, the document has been intentionally divided into two separate documents - the needs assessment (1) and the compliance assessment (2). The needs assessment will be referred to as the BNA and the compliance assessment will be referred to as the BNA Compliance Assessment. The primary driver for separation is that the needs assessment identifies the detailed applicable regulations (primarily NFPA Standards) for emergency response capabilities based on the hazards present at LLNL and Sandia/CA and the geographical location of the facilities. The needs assessment also identifies areas where the modification of the requirements in the applicable NFPA standards is appropriate, due to the improved fire protection provided, the remote location and low population density of some the facilities. As such, the needs assessment contains equivalencies to the applicable requirements. The compliance assessment contains no such equivalencies and simply assesses the existing emergency response resources to the requirements of the BNA and can be updated as compliance changes independent of the BNA update schedule. There are numerous NFPA codes and standards and other requirements and guidance documents that address the subject of emergency response. These requirements documents are not always well coordinated and may contain duplicative or conflicting requirements or even coverage gaps. Left unaddressed, this regulatory situation results in frequent interpretation of requirements documents. Different interpretations can then lead to inconsistent implementation. This BNA addresses this situation by compiling applicable requirements from all identified sources (see Section 5) and analyzing them collectively to address conflict and overlap as applicable to the hazards presented by the LLNL and Sandia/CA sites (see Section 7). The BNA also generates requirements when needed to fill any identified gaps in regulatory coverage. Finally, the BNA produces a customized simple set of requirements, appropriate for the DOE protection goals, such as those defined in DOE O 420.1B, the hazard level, the population density, the topography, and the site layout at LLNL and Sandia/CA that will be used as the baseline requirements set - the 'baseline needs' - for emergency response at LLNL and Sandia/CA. A template approach is utilized to accomplish this evaluation for each of the nine topical areas that comprise the baseline needs for emergency response. The basis for conclusions reached in determining the baseline needs for each of the topical areas is presented in Sections 7.1 through 7.9. This BNA identifies only mandatory requirements and establishes the minimum performance criteria. The minimum performance criteria may not be the level of performance desired Lawrence Livermore National Laboratory or Sandia/CA. Performance at levels greater than those established by this document will provide a higher level of fire safety, fire protection, or loss control and is encouraged. In Section 7, Determination of Baseline Needs, a standard template was used to describe the process used that involves separating basic emergency response needs into nine separate services. Each service being evaluated contains a determination of minimum requirements, an analysis of the requirements, a statement of minimum performance, and finally a summary of the minimum performance. The requirement documents, listed in Section 5, are those laws, regulations, DOE Directives, contractual obligations, or LLNL policies that establish service levels. The determination of minimum requirements section explains the rationale or method used to determine the minimum requirements.

Book excerpt: It is the Lawrence Livermore National Laboratory's (LLNL) policy to perform work in a manner that protects the health and safety of employees and the public, preserves the quality of the environment, and prevents property damage using the Integrated Safety Management System. The environment, safety, and health are to take priority in the planning and execution of work activities at the Laboratory. Furthermore, it is the policy of LLNL to comply with applicable ES & H laws, regulations, and requirements (LLNL Environment, Safety and Health Manual, Document 1.2, ES & H Policies of LLNL). The program and policies that improve LLNL's ability to prevent or mitigate accidental releases are described in the LLNL Environment, Health, and Safety Manual that is available to the public. The laboratory uses an emergency management system known as the Incident Command System, in accordance with the California Standardized Emergency Management System (SEMS) to respond to Operational Emergencies and to mitigate consequences resulting from them. Operational Emergencies are defined as unplanned, significant events or conditions that require time-urgent response from outside the immediate area of the incident that could seriously impact the safety or security of the public, LLNL's employees, its facilities, or the environment. The Emergency Plan contains LLNL's Operational Emergency response policies, commitments, and institutional responsibilities for managing and recovering from emergencies. It is not possible to list in the Emergency Plan all events that could occur during any given emergency situation. However, a combination of hazard assessments, an effective Emergency Plan, and Emergency Plan Implementing Procedures (EPIPs) can provide the framework for responses to postulated emergency situations. Revision 7, 2004 of the above mentioned LLNL Emergency Plan is available to the public. The most recent revision of the LLNL Emergency Plan LLNL-AM-402556, Revision 11, March 2008, has been included as an appendix to the RMP Supplemental Information document. LLNL Site 200 is a research and development laboratory with infrastructure necessary to support its operations and personnel. Research and development activities at LLNL are focused on stockpile stewardship; achieving robust and vital scientific, engineering, and manufacturing capability; inertial confinement fusion; laser technology; materials and process science; computational and information sciences; basic sciences; engineering sciences; and biological sciences. Based upon CalARP Program regulations, guidance found in California Accidental Release Prevention Program (CalARP) Administering Guidance, Chapter 1, a review of facility specific documents, accident analyses summarized in this document & detailed in the LLNL Site 200 RMP Supporting Information document, LLNL has determined that each process meeting the CalARP threshold criteria meets the requirements for CalARP Program Level 1. In accordance with CalARP regulations, LLNL considers the natural segmentation of processes at Site 200 to be on a building basis and therefore consideration of inventory should be on a building basis rather than a 'site-wide' basis. Only those materials identified as equal to as or greater than the threshold quantities for the CalARP program on a building (process) level are reflected in this document. As such, materials and quantities reported in this document on a building (process) level will vary from materials and quantities reported in the LLNL Hazardous Materials Business Plan, Acutely Hazardous Material Registration Form on a site-wide level. For each process involving regulated quantities of lithium hydride, worst case accident analysis shows that the toxic endpoint lies within the site boundaries. These analyses document that the nearest public receptor is beyond the distance to a toxic or flammable endpoint. Refer to the LLNL Site 200 RMP Supporting Information document for a more detailed explanation of the worst case accident analyses for these processes. For the process involving regulated quantities of nitric acid, worst case accident analysis predicts a hazard zone well within areas under the jurisdiction of the Department of Energy. This analysis documents that the nearest public receptor is beyond the distance to a toxic or flammable endpoint. Refer to the LLNL Site 200 RMP Supporting Information document for a more detailed explanation of the worst case accident analysis for this process. LLNL maintains an active program to protect workers, the public, and the environment from harm resulting from its activities. Its policies and technical directions for controlling all hazards that are present as a result of its operations are described in the LLNL Environment, Health, and Safety Manual (referenced above).

Book excerpt: A safety assessment was performed to determine if accidents occurring at the 834 Complex at Site 300 could present undue hazards to the general public, personnel at Site 300, or have an adverse effect on the environment. The credible accidents that might have an effect on these facilities or have off-site consequences were considered. These were earthquake, extreme wind (including missiles), lightning, flood, criticality, high explosive (HE) detonation that disperses uranium and beryllium, spontaneous oxidation of plutonium, explosions due to finely divided particles, and a fire. Seismic and extreme wind (including missiles) analyses indicate that the buildings are basically sound. (However, there are a few recommendations to further enhance the structural integrity of these facilities). Additional lightning protection for these facilities is being installed. These buildings are located high above the dry creek bed so that a flood is improbable. A criticality or a high explosive detonation involving plutonium is very remote since the radioactive materials are encased and plutonium and HE are not permitted concurrently in the same area at Site 300. (The exceptions to this policy are that explosive actuating devices are sometimes located in assemblies containing fissile materials. However, a planned or accidental actuation will not effect the safe containment of the fissile material within the assembly). Even though the possibility of an HE explosion involving uranium and beryllium is remote, the off-site lung doses were calculated and found to be below the accepted standards. It was determined that a fire was unlikely due to the low fire loading and the absence of ignition sources. It was also determined that the consequences of any accidents were reduced by the remote location of these facilities, their design, and by administrative controls.

The PABD characterizes the level of intrinsic potential hazard associated with a facility and provides the basis for its hazard classification. The hazard classification determines the level of safety documentation required and the level of review and approval for the safety analysis. The hazards of primary concern associated with the BSL-3 facility are biological. The hazard classification is determined by comparing facility inventories of biological materials and activities with the BSL-3 threshold established by the Centers for Disease Control and Prevention (CDC) and the National Institutes of Health (NIH) for BSL-3 facilities.

Book excerpt: While the United States and its allies put their military focus on the post-9/11 challenges of counter-terrorism and counter-insurgency, Russia and China put their military focus onto the United States and the risks of regional wars that they came to believe they might have to fight against the United States. Their first priority was to put their intellectual houses in order-that is, to adapt military thought and strategic planning to the new problem. The result is a set of ideas about how to bring the United States and its allies to a "culminating point" where they choose to no longer run the costs and risks of continued war. This is the "red theory of victory." Beginning in the second presidential term of Obama administration, the U.S. military focus began to shift, driven by rising Russian and Chinese military assertiveness and outspoken opposition to the regional security orders on their peripheries. But U.S. military thought has been slow to catch up. As a recent bipartisan congressional commission concluded, the U.S. intellectual house is dangerously out of order for this new strategic problem. There is no Blue theory of victory. Such a theory should explain how the United States and its allies can strip away the confidence of leaders in Moscow and Beijing (and Pyongyang) in their "escalation calculus"-that is, that they will judge the costs too high, the benefits to low, and the risks incalculable. To develop, improve, and implement the needed new concepts requires a broad campaign of activities by the United States and full partnership with its allies.
