Book excerpt: L'objectif de cette thèse est d'analyser la littérature sur l'apport biologique de la rhBMP-2 en vue d'une reconstruction osseuse afin de permettre la pose d'un implant dentaire. Ainsi, des nouveaux concepts avec des résultats prévisibles et des protocoles peu invasifs ont été mis en oeuvre. En particulier, les "Bone morphogenetic proteins" dites BMPs, ont été isolés à partir de la matrice osseuse déminéralisée et se caractérisent par des puissantes propriétés ostéoinductrices. Leur faible poids moléculaire et leur présentation sous forme d'une solution liquide la nécessité d'un vecteur ; la rhBMP-2 est commercialisée avec des éponges de collagène résorbables et constitue un adjuvant bioactif. L'utilisation de rh-BMP-2 offre une véritable alernative aux techniques chirurgicales conventionnelles de reconstructions osseuses car elle permet une néoformation osseuse, cet os induit est identique à l'os de l'hôte permettant la pose d'un implant ainsi que l'ostéointégration mais il existe des désavantages notamment, leur coût très élevé et la gestion des suites opératoires. Actuellement il est difficile d'établir un protocole définitif de l'utilisation de ces rhBMP-2 d'autant plus que le succès à long terme de la mise en charge et la quantité d'os néoformé ne sont pas définis. S'il s'agit là, d'un premier pas vers une cohérence scientifique au sein des études menées sur les rhBMP-2, des études sur la dose et la concentration optimale, le vecteur idéal ainsi que les conditions de stimulation de la croissance osseuse doivent être effectués afin d'optimiser la prévisibilité et la prédictibilité dans la réalité clinique.
