De ce fait, l'action du praticien vise à éradiquer les bactéries pathogènes responsables ; dans cette optique, l'utilisation d'une antibiothérapie en complément du traitement mécanique conventionnel est parfois nécessaire. Notre intérêt est ici de définir un cadre d'utilisation, et de déterminer la place de cette stratégie antimicrobienne dans le plan de traitement parodontal. Afin d'en préciser l'étiologie, la première partie analyse l'aspect bactériologique des maladies parodontales. Puis, sont précisées les indications de l'utilisation d'agents antibiotiques, en association avec un diagnostic étiologique précis. Enfin, une étude sur l'apport réel fourni par une antibiothérapie locale sur les résultats cliniques est menée, à travers différentes publications. Les différents produits, ainsi que les différents supports disponibles sont ensuite exposés. Leur efficacité thérapeutique est évaluée, en examinant les résultats bactériologiques et cliniques publiés. Par souci d'objectivité, une analyse critique de quelques articles de la thèse a été réalisée selon les recommandations de 1'ANAES

Book excerpt: La @compétition entre les facteurs de défences immunologique de l'hôte et la flore parodontopathique sont à l'origine du développement des maladies parodontales. Leur traitement a été principalement centré autour du détartrage et surfaçage mécanique afin de désorganiser le biofilm microbien. Ce traitement présente toutefois certaines limites, notament la recontamination bactérienne posttraitement. Parce que la maladie parodontale est principalement d'étiologie bactérienne, dse traitements complémentaires ont été proposés, en particuliers l'antibiothérapie administrée par voie systémique ou locale. Plusieurs molécules ont été testées en application locale : parmi elles, la minocycline. Il s'agit d'une tétracycline de seconde génération qui possède une activité antibiotique contre de nimbreuses bactéries parodontopathogènes et une activité anti-inflammatoire. La Parocline® est un système de microencapsulation qui permet une libération lente, régulière et à forte concentration de minocycline 2% au cantact du fluide gingival. Les nombreux travaux étudiés ne permettent pas de conclure de façon catégorique à une efficacité supérieure de l'association détartrage-surfaçage radiculaire et application locale de minocycline mais elle permet toutefois une amélioration des paramètres cliniques dans des maladie au cours du traitement de maintenance parodontale, les poches parodontales réfractaires localisées après la thérapeutique mécanique conventionnelle et l'association à la thérapeutique chirurgicale.

Dans la 1ère partie, nous avons procédé à une revue de la littérature concernant les bases fondamentales de l'antibiothérapie curative et préventive. La notion de variabilité de la sensibilité bactérienne a été introduite. Dans la 2è partie, nous avons étudié, à partir de 60 prélèvements de flore sous gingivale effectués chez 20 patients atteints de parodontites agressives, la variabilité de la sensibilité de 82 souches bactériennes. Dix antibiotiques ont été testés (2 méthodes d'antibiogramme). De fortes variabilités inter individuelles et intra individuelles ont été observées spécialement avec les antibiotiques dits de 1ère génération (pénicilline G, tétracycline et érythromycine) et la ciprofloxacine. P. intermedia montra le coefficient de variation le plus élevé. La réalisation de plusieurs antibiogrammes à partir de CFU distinctes de P. intermedia est recommandée dans les parodontites agressives.
