Book excerpt: It is long been acknowledged that psychosocial and mental health concerns must be addressed if schools are to function satisfactorily and students are to learn and perform effectively. This reality is reflected in the aims of the No Child Left Behind Act and the Individuals with Disabilities Education Act. And, it is consonant with the goals and recommendations of the President's New Freedom Commission on Mental Health. Indeed, these initiatives reflect a shared agenda and must coalesce in school improvement policies and initiatives in ways that more wisely invest and use sparse resources. In pursuit of a shared agenda, existing resources can be deployed and redeployed in ways that enhance equity with respect to availability, access, and effectiveness. There are about 90,000 schools in the U.S.A. In a real sense, schools are primary care and public health settings, and thus, school staff are primary care providers and agents for public health, although most don't identify as such. Moreover, our society calls on schools to serve all students without regard to disorder, disability, ethnicity, economic status, gender identity, and so forth. As a result, efforts to transform how mental health is delivered in this country need to include a specific emphasis on enhancing the focus on mental health in schools. To this end, this brief highlights ways in which the President's New Freedom Commission on Mental Health's recommendations apply to mental health in schools. As conceived here, mental health in schools is (a) part of essential student support systems that enable students to learn so that schools can achieve their mission and (b) a fundamental facet of the initiative to transform the mental health system. (Four appendices include: (1) Highlights of the Goals and Recommendations of the President's New Freedom Commission on Mental Health; (2) Federal Mental Health in Schools Program; (3) Note About Building on Current In-School Practices; and (4) Guidelines, Standards and Accountability for MH in Schools. For many professionals, it is the primary way they access information and resources. Thus, analyses of what is and isn't online are essential in advancing the internet's value for enterprises such as the New Freedom Initiative. This report presents analyses of a synthesis of online, noncommercial resources that have relevance for advancing efforts to integrate mental health in schools into the New Freedom Initiative. Not surprisingly, the analyses indicate that the online resources in many arenas of concern are limited, and there are major gaps. The importance of this report is not in this statement of the obvious, but in the specifics that provide a detailed needs assessment. The delineation of needs is intended to guide development of a more basic set of online resources with a view to accelerating the envisioned transformation of the mental health system. We conclude by urging those responsible for the New Freedom Initiative to bring together relevant agencies, centers, and organizations to encourage them to establish a coordinated and cohesive approach for filling online resource gaps. Book excerpt: This compendium of 17 articles addresses the goals set forth by the President's New Freedom Commission on Mental Health in its 2003 report, Achieving the Promise: Transforming Mental Health Care in America. The report represents the first time since the Carter Administration that such a high-level group evaluated U.S. mental health care. The report painted a dismal picture of the nation's mental health system, saying the system was so broken that it was "beyond simple repair." The Commission said that current services focused on "managing disabilities" rather than helping patients achieve a meaningful life in their communities. It also stated that mental health service providers ignored the preferences of consumers and their families. The articles in Transforming Mental Health Services: Implementing the Federal Agenda for Change, originally published between 2006 and 2009 in Psychiatric Services (journal of the American Psychiatric Association), offer recommendations to assist adults with serious mental illness and children with serious emotional disturbances. They include a series of reforms in which the emphasis is on recovery as an achievable goal, and the need for a person-centered orientation in service delivery. There is also discussion of the reasons many service providers resist using a recovery orientation and how this can be remedied. Transforming Mental Health Services: Implementing the Federal Agenda for Change consists of updates of papers written by the Commission's subcommittees addressing issues fundamental to those living with mental illness. It is organized into four sections: The first focuses on the interface between mental health and general health, and on employment, housing, and Medicaid financing. The second continues addressing financing and Medicaid as well as issues related to school mental health, recovery, transformation of data systems, and acceleration of research. The third includes reports from four states with transformation initiatives designed to ensure that consumers have a strong voice in the development of recovery-oriented services. The final section describes progress five years after the President's Commission Report and concludes with a proposal by the current director of the Center for Mental Health Services for a public health model of mental health care for the 21st century. This compilation of well-researched and well-written articles offers an excellent resource for frontline care providers, facility administrators and advocates. It serves as an equally valuable resource for state policy makers who wish to present a convincing case that change is happening and that the recommendations can be translated into effective policies. Although consumers and their families will receive support for their perception that service providers ignore their needs, they will also be encouraged that change for the better is coming to the U.S. mental health care system.

Book excerpt: Today's school and community stakeholders are attempting to address complex, multifaceted, and overlapping psychosocial and mental health concerns in diverse, fragmented and at-times marginalized ways. This has led to competition for sparse resources and inadequate results. Enhancing mental health in schools is not an easy task. The bottom line is that limited efficacy seems inevitable as long as the full continuum of necessary programs is unavailable and staff development remains deficient; limited cost effectiveness seems inevitable as long as related interventions are carried out in isolation of each other; and limited systemic change is likely as long as the entire enterprise is marginalized in policy and practice. This state of affairs calls for realigning policy and practice around a unifying and cohesive framework based on well-conceived models and the best available scholarship. Initiatives for mental health in schools must be connected in major ways with the mission of schools and integrated into a restructured system of education support programs and services. This means braiding resources and interventions with a view to ensuring there is a system of learning supports that meets overlapping needs and does so by fully integrating mental health agenda into school improvement planning at school and district levels. Policy and practice must end the marginalization of mental health in schools. Six appendices include: (1) A. From a public health perspective, we now appreciate the enormous significance of mental distress and illness as causes of disability and impairment. Stress and anxiety, and other mental illnesses are linked to risks in the environment. This book questions how and why the social and physical environment matters for mental health and psychological wellbeing in human populations. While putting forward a number of different points of view, there is a particular emphasis on ideas and research from health geography, which conceptualises space and place in ways that provide a distinctive focus on the interactions between people and their social and physical environment. The book begins with an overview of a rich body of theory and research from sociology, psychology, social epidemiology, social psychiatry and neuroscience, considering arguments concerning 'mind-body dualism', and presenting a conceptual framework for studying how attributes of 'space' and 'place' are associated with human mental wellbeing.
