Book excerpt: Hypertension (HTN) is "the most common condition seen in primary care and can lead to myocardial infarction, stroke, renal failure, and death if not detected early and treated appropriately". High BP is consistently of the most fatal health conditions in the US. Despite often having no warning signs, untreated high blood pressure increases the odds of suffering from poor outcomes from cardiovascular disease and stroke. This study focuses on managing HTN in an ambulatory care setting in San Jose, Ca. serving ethnically diverse and vulnerable populations. The Iowa Model for Evidence-Based Practice to Promote Quality Care, developed by Titler et al., "is well-known and highlights the importance of considering the entire healthcare system from the provider to the patient, to the infrastructure, using research within these contexts to guide practice decisions". The theoretical framework of the Middle-Range Theory of Self-Care of Chronic Illness is applied to the study as the patient population is identified as having the chronic condition of HTN and the need for self-care strategies. The central phenomenon of self-care is defined as a "process of maintaining health through health-promoting practices and managing illness". A second theoretical framework is Wagner's chronic care model, a process improvement model to achieve improved coordination of long-term medical conditions within healthcare organizations. This project's methodology is the utilization of a multimodel team approach in educating patients on hypertension management. Patients are selected as identified as having essential hypertension according to the EMR. A joint educational presentation utilizing PowerPoint to review HTN and its consequences with a demonstration on appropriate blood pressure measurement is provided. Community partners discuss food preparation/selection. After the presentation, there is a 6-7 - minute relaxation video. Patients are given a blood pressure machine and log to take home. There is also a brief survey for the participants to complete. There is a time set aside to meet with the provider and pharmacist to review medications. Patient confidentiality is respected throughout the process. In conclusion, effective group classes with collaborative efforts to address the critical risk factor of HTN can help patients lower their blood pressures, better maintain a steady medication routine and diet, provide strategies in helping to manage blood pressure, ultimately leading to an improved quality of life and reduced risk of death. This finding compares favorably to that of the literature review, which supports multimodal strategies in treating this chronic illness. Ultimately, the goal of this evidence-based outcomes project utilizing the Iowa model with self-care and chronic care theoretical frameworks as a guide will improve blood pressure results and provide strategies for providers and patients to help manage their chronic illness.

Elevated blood pressure will disrupt the function of the cardiovascular system and impact the function of major organs, resulting in devastating complications for the patient while increasing cost for the healthcare system. This evidence-based quality improvement project is aimed at increasing control for veterans with high blood pressure and lowering blood pressure to a goal under 140/90 through a nurse driven education program. A quasi-experimental project design was utilized to improve nursing practice in managing hypertension care by addressing veterans understanding of self-care management and nurses’ knowledge of hypertension management. Descriptive statistics were used to describe the data collected over a two-month period. Findings suggested that when using a combination of the structured hypertension education program, optimized nursing support, and frequent monitoring, blood pressure and self-care management for veterans improved. Implications for practice is to implement a nurse-driven protocol to be adopted by the Primary Care service line for enhanced hypertension care.

Book excerpt: Purpose: Primary care providers can improve hypertension (HTN) management by using the Hear the Beat (HTB) pamphlet to guide the comprehensive management of HTN, especially for older adults. The 2020 International Society of Hypertension Global Hypertension Practice Guidelines provide updated blood pressure (BP) parameters and encourage greater patient participation. The HTB pamphlet needed revision to comply with the latest guidelines. The Doctor of Nursing Practice (DNP) team modified the pamphlet and created training on updates for clinical staff at the Ogden Clinic - Farmington family practice and urgent care. Methodology: Clinical staff completed a pre-education survey and were educated on the updated HTB pamphlet. Next, the staff completed a post-education survey and utilized the pamphlet for two months. Finally, a post-implementation survey was given. A questionnaire was given to patients two weeks after receiving HTB so that Ogden Clinic's quality department staff could determine the usefulness of the updates. Qualitative and quantitative data were gathered, with most on a Likert scale or Yes/No/Unsure. Results: Data analysis revealed an increase in HTB usage by clinical staff. Staff members also improved in defining national BP parameters and explaining home BP tracking. Patient responses showed a universal belief that the HTB pamphlet is helpful. In addition, 93.75% of patients agreed that their knowledge and frequency of home BP monitoring increased. Over 80% saw a BP improvement. Implications for practice: These results indicate that HTB is a clinical tool capable of managing HTN and is preferred by staff and patients.

To prevent complications, the goal of a Primary Care Provider's patient panel maintaining a blood pressure less than 140/90 has been established on the organization's national office. In the Primary Care Clinic, an interdisciplinary team consisting of a Registered Nurse, Clinical Psychologist, and Cardiologist implemented a group education class aimed at a Hypertensive Veteran cohort. The purpose of this project is to improve self-management behaviors in the Veteran population, and to help increase the hypertension performance metric by 2% one month after class implementation. Blood pressure readings were measured before and after the class, as well as the percentage of well-controlled hypertensive patients on the Primary Care Dashboard. Data collection revealed a 2.2% performance increase one month after the class implementation; this reflects an improvement in the Primary Care Provider's ability to manage his hypertensive cohort, while also indicating an overall increase in the number of hypertensive patients whose systolic blood pressure was lower than 140 mmHg, and diastolic blood pressure lower than 90 mmHg.

Hypertension is the first and third primary cause of death in the United States. Forty six percent of Americans have their blood pressure controlled. High blood pressure costs the United States 76 billion for health care services, medications, and missed work days (CDC, 2011). Patients in the Primary Care Clinic at the Baltimore Veterans Administration Medical Center are returning with uncontrolled blood pressure in spite of the programs in place for hypertension management. These programs include a group class, one-to-one and tele-health weight control sessions, and a smoking cessation group class. Other programs are medication management and a walk-in hypertension clinic with medication dose adjustments. Objective: The objective is to improve blood pressure control in a population of veteran male patients between the ages of 45-64 with uncontrolled hypertension who are receiving antihypertensive medications through patient education and telephone follow-up calls. Methods: A letter of invitation was sent to 100 to 150 veterans to participate in a hypertension group education session with telephone follow-up call. Of those participants who responded, 30 participated in the project. The initial hypertension group education was followed by a two week telephone follow-up call and a monthly blood pressure check. Findings: The veterans demonstrated improved blood pressure control and adherence to lifestyle changes and medications secondary to patient education and telephone follow-up call." -- Abstract.

Book excerpt: Qualitative researchers have traditionally been cautious about claiming that their work was scientific. The "right-on" schools have exaggerated this caution into an outright rejection of science as a model for their work. Science is, for them, outmoded; "an archaic form of consciousness surviving for a while yet in a degraded form" (Tyler 1986:200). Scientists' assertions that they are in pursuit of truth simply camouflage their own lust for power. There is no essential difference between truth and propaganda. The authors acknowledge that the boundary between science and propaganda has often been breached and some distrust of scientific claims may be healthy. They also question the claim that science creates disinterested and objective knowledge of an observer-independent world without concluding that science is impossible. The skeptics' reservations about qualitative research are based on the deep-rooted assumption among natural scientists, and some social scientists, that there is a world "out there," prior to, and independent of, their observations. This world can be known objectively in the sense that all observers will, if identically placed, see it in exactly the same way. If a suitable language were available, they would also all produce identical descriptions. From these observations they can work out the laws governing the world's operations. The authors try to resolve these contrary claims by asserting that science is a procedural commitment. It consists of openness to refutation, a conscientious and systematic search for contradictory evidence, and a readiness to subject one's preconceptions to critical examination. The devotion to truth as a regulative ideal is an essential difference between science and propaganda. This work is a unique and innovative defense of scientific method.
