Book excerpt: INTRODUCTION : Devant le nombre croissant d'admissions dans les structures d'Urgences, le personnel soignant peut rencontrer des difficultés à gérer ce flux. La problématique f(engorgement résulte de difficultés rencontrées en amont de l'hôpital et en aval des urgences. Les conséquences pour les patients, pour les professionnels de santé et pour l'économie de santé publique sont non négligeables. Afin de tenter de réduire ce flux, l'objectif de cette thèse est d'analyser les admissions inappropriées aux Urgences du CHR Felix Guyon eet d'étudier l'intérêt et la possibilité d'optimiser la filière d'hospitalisation directe. MATERIEL ET METHODES : Nous avons entrepris une étude rétrospective observationnelle descriptive sur 2 mois, incluant 88 patients adressés par leur médecin traitant aux Urgences puis hospitalisés dans les services de Médecine à des horaires compatibles avec une entrée directe. Les dossiers des patients ont d'abord été analysés d'après les critères de sévérité clinique de la grille AEPf (Apprpriateness Evaluation Protocol français) puis ont été expertisés par 3 médecins afin de juger de la justification ou non de l'admission aux Urgences avant 'hospitalisation. RESULTATS : Après analyse des dossiers par la grille AEPf, 61 % des passages aux Urgences sont considérés comme non pertinents car ne répondant pas aux critères de sévérité clinique. L'expertise des dossiers a révélé que 39 % des 88 patients auraient pu être admis directement dans un service de Médecine sans préjudice pour leur prise en charge. CONCLUSION : Les admissions inappropriées aux Urgences sont nombreuses mais il existe des possibilités afin de réduire les passages non pertinents dont l'optimisation de la filière d'hospitalisation directe au CHR F.G. Pour tenter de résoudre cette problématique, nous proposons de mettre en place une assistance téléphonique dans les services de Médecine ; les médecins généralistes pourront avoir directement un sénior au téléphone pour avoir un avis spécialisé ou programmer une hospitalisation.

Book excerpt: The most recent Ebola epidemic that began in late 2013 alerted the entire world to the gaps in infectious disease emergency preparedness and response. The regional outbreak that progressed to a significant public health emergency of international concern (PHEIC) in a matter of months killed 11,310 and infected more than 28,616. While this outbreak bears some unique distinctions to past outbreaks, many characteristics remain the same and contributed to tragic loss of human life and unnecessary expenditure of capital: insufficient knowledge of the disease, its reservoirs, and its transmission; delayed prevention efforts and treatment; poor control of the disease in hospital settings; and inadequate community and international responses. Recognizing the opportunity to learn from the countless lessons of this epidemic, the National Academies of Sciences, Engineering, and Medicine convened a workshop in March 2015 to discuss the challenges to successful outbreak responses at the scientific, clinical, and global health levels. Workshop participants explored the epidemic from multiple perspectives, identified important questions about Ebola that remained unanswered, and sought to apply this understanding to the broad challenges posed by Ebola and other emerging pathogens, to prevent the international community from being taken by surprise once again in the face of these threats. This publication summarizes the presentations and discussions from the workshop.

The author had the privilege to immerse himself, around the clock, in the Yakaphones’ activities and thoughts in southwest DR Congo from 1972 to 1974, and intermittently in Kinshasa’s shanty towns, from 1986 to 2003. The author first examines what sparked his choice to come to Congo, and then to pursue research among the Yakaphones in the borderland with Angola. He then invites us to follow the trajectory of his plural anthropological view on today’s multicentric world. It leads us to his praise for honorary doctor Jean-Marc Ela’s work. He then examines the proletarian outbursts of violence that rocked Congo’s major cities in 1991 and 1993. These can be read as a settling of scores with the disillusioning colonial and missionary modernisation, along with president Mobutu’s millenarian Popular Movement of the Revolution. Furthermore, after considering the morose reduction of a major Yaka dancing mask into a mere museum-bound curio in Antwerp, the book unravels the Yakaphones’ perspectives on spirits and sorcery’s threat. It also analyses their commitment to classical Bantu-African healing cults, along with their parallel consulting physicians and healers. By sharing the Yakaphones’ life-world, the analysis highlights their body-group-world weave, interlaced by the principle of co-resonance. A phenomenological and perspectivist look unfolds the local actors’ views, thereby disclosing the Bantu-African genius and setting for a major reversal of perspectives. Indeed, seeing 'here' from 'there' allows the author to uncover some alienating dynamics at work in his native Belgian Flemish-speaking culture. To better grasp the realm of life beyond the speakable and factual reasoning, the approach occasionally turns to the later Lacan’s focus on the unconscious desire, the body and its affects. The book addresses students and researchers in the humanities and, more broadly, all those immersed in the heat of the encounter with the culturally different.

It addresses the evolution of humanitarian goals, the resistance to these goals and the political arrangements that overcame this resistance (or that failed to do so). The contributors seek to analyse the political transactions and balances of power and interests that allow aid activities to move forward, but that are usually masked by the lofty rhetoric of "humanitarian principles". They focus on one key question: what is an acceptable compromise for MSF? This book seeks to puncture a number of the myths that have grown up over the forty years since MSF was founded and describes in detail how the ideals of humanitarian principles and "humanitarian space" operating in conflict zones are in reality illusory. How, in fact, it is the grubby negotiations with varying parties, each of whom have their own vested interests, that may allow organisations such as MSF to operate in a given crisis situation - or not.
