Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Book excerpt: Understanding whether healthcare policies can narrow and eliminate racial/ethnic disparities in health care access and utilization is an important public health issue. This dissertation examines whether two reforms, the Massachusetts (MA) health care reform law (Chapter 58) and the Patient Protection and Affordable Care Act (ACA), reduced racial/ethnic-related disparities in health care access and utilization between Hispanic and NH-white children. The first aim of this study examined the long-term effects of the MA health reform on disparities in insurance access, utilization of care, and health status among Hispanic children compared to non-Hispanic white children. The second and third aims examined the short-term effects of the ACA on disparities among Hispanic children compared to non-Hispanic white children. The second aim evaluated the impact of the ACA on insurance access and utilization of health care, and the third aim examined ACA impacts on financial burden due to medical costs experienced by the child’s family. Data from the National Survey of Children’s Health and the National Health Interview Survey were evaluated before and after implementation of the MA health reform (aim 1) and the ACA insurance expansion in 2014 (aims 2 & 3). The study subjects were children ages 0 to 17 years old. The impact of insurance expansion through health reform on disparities was evaluated using a triple-difference (difference-in-difference-in-difference, or DDD) analysis, which controlled for events not captured by the covariates that can affect the outcomes. Each aim utilized a nonequivalent pretest/posttest comparison group study design. For the first aim, children living in Massachusetts were the intervention group, and children living in surrounding states (Rhode Island, New Hampshire, and Connecticut) were in the comparison group. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

The ACA has multiple provisions specific to race, ethnicity, and language and other provisions with significant implications for racially and ethnically diverse populations. In April 2013, the roundtable held a workshop to address many issues surrounding the ACA, including expansion of coverage, delivery systems, and access points, service delivery and payment reform, public-private partnerships, and challenges to the safety net. This report summarizes the presentations and discussions from the workshop.
