Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. The existence of a minority in a society implies the existence of a corresponding dominant group enjoying higher social status and greater privileges.

These disparities affect the lives of an estimated 5.3-7.4 million American men, and are an important concern for public health. Until very recently, the relative invisibility of this group and a paucity of empirical data have hampered attempts to identify health disparities experienced by gay and bisexual men. This book proposes to review and synthesize evidence of health disparities among gay and bisexual men, identify individual and community factors that contribute to these disparities, and articulate strategies for public health efforts to eliminate disparities. To date, these disparities have been largely discussed in isolation in the research literature in a manner that does not permit a comprehensive examination of these problems, their underlying causes, and potential solutions.
