Book Prevention and Management of Pressure Ulcers in Adult

Download or read book Prevention and Management of Pressure Ulcers in Adult written by Helen Ette and published by . This book was released on 2013 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Although some authoritative organizations have developed and promote evidenced-based guidelines on prevention and management of pressure ulcer, research has shown that non adherence to these guidelines has resulted to significant proportion of pressure ulcer among adults ages 18 and older in health care environment and in the community. The questions comes, Why the nonadherance? When health care professionals lack knowledge and skill in identifying high risk patients that makes it a problem. Recognition of high risk patient may be based on competency in assessment skill. Because prevention of pressure ulcer is an integral part of nursing, nurses have a role to play to achieve this goal therefore it is important for nurses to assess themselves in order to have a good perception of their knowledge about what they know, and understand hence nurses should embark on improving this skill. The uses of appropriate tools, initiating treatment based on establish protocols can prevent and reduce the prevalence of pressure ulcers and its complications. This will call for a change on how nurses perform initial assessment, document on daily basis especially on high risk patients and application of appropriate devices. Because patients and families can play a part in pressure ulcer prevention, nurses will have that responsibility in educating them on the preventive measures. The article discussed in this paper supports the fact that nurses have a greater role in prevention and management of pressure ulcer and improve the life of patients at risk. During the implementation stage, it will be necessary for nurses and other health care workers to apply evidence-based research for example, turning, cushioning the prominences, nutritional support to solve this problem. Nurses knowledge on proper documentation and application of devices will be evaluated from patients chart by using an approved audit tool and based, education intervention will be used as this will improve the quality of life of the patients as well as guide nurses in delivering of quality care. These interventions can also reduce length of stay, reduce health care cost as well as reduce burden to the patient's families.

Book Fragility Fracture Nursing

Download or read book Fragility Fracture Nursing written by Karen Hertz and published by Springer. This book was released on 2018-06-15 with total page 169 pages. Available in PDF, EPUB and Kindle. Book excerpt: This open access book aims to provide a comprehensive but practical overview of the knowledge required for the assessment and management of the older adult with or at risk of fragility fracture. It considers this from the perspectives of all of the settings in which this group of patients receive nursing care. Globally, a fragility fracture is estimated to occur every 3 seconds. This amounts to 25 000 fractures per day or 9 million per year. The financial costs are reported to be: 32 billion EUR per year in Europe and 20 billon USD in the United States. As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.

Book Evidence based Pressure Ulcer Education and Prevention Guidelines

Download or read book Evidence based Pressure Ulcer Education and Prevention Guidelines written by Jordana Kearns and published by . This book was released on 2013 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Evidence-based pressure ulcer prevention guidelines for the at-risk adult patient have been previously developed and encouraged by organizations of authority. Never less, non-adherence to prescribed guidelines are often reported, and data obtained from National Nursing-Sensitive Indicators shows that at-risk patients continue to develop hospital acquired pressure ulcers (HAPU) at an unacceptable rate. The Aims of this Capstone project are to initiate a pressure ulcer education and prevention program in a medium-sized, community health care organization, and to change the attitudes and knowledge base of nurses by providing nursing staff with evidence-based knowledge, education, and skills regarding pressure ulcer development and prevention. This project consists of educating nurses on pressure ulcer pathophysiology and the importance of using early evidence-based pressure ulcer prevention education and techniques. Implementation of this project requires approval from key stakeholders. Applying Kurt Lewin's theory of change first requires the recognition that change needs to occur (Cresia and Friburg, 2011). Implementation consists of scheduled staff in-services, monthly meetings, and use of multi-technique pressure ulcer prevention products, risk-assessment, and screening forms. Data collection of pressure ulcer prevalence, for three months prior to implementation and for 3 months post-implementation, will be collected to evaluate the effectiveness of the program. Evaluation of the program will also consist of an evaluation of nursing staff's pre and post-implementation pressure ulcer prevention knowledge and skill. Dissemination of evidence to key stakeholders, health care organizations and to the greater nursing community will occur with the aim to increase knowledge of evidence-based pressure ulcer prevention techniques that will contribute to improved patient outcomes. This project will demonstrate how pressure ulcer prevalence rates, in the hospital-setting can be decreased through the consistent and knowledgeable use of evidence-based pressure ulcer prevention guidelines. PICOT Question: For the at-risk adult hospitalized patient (P), will the implementation of early, evidence-based pressure ulcer prevention interventions by nurses, who are educated in skills and knowledge regarding pressure ulcer development and prevention (I), as compared to those not educated, nor using early evidence-based pressure ulcer prevention interventions (C), decrease the probability of hospital acquired pressure ulcer development (O), in a three-month period (T).

Book Prevention of Pressure Ulcers in a Health Care Setting

Download or read book Prevention of Pressure Ulcers in a Health Care Setting written by Ramatu Swarray and published by . This book was released on 2014 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Pressure Ulcer is a growing problem in the non ambulatory health care settings affecting the young, adult, and the elderly. The growing problems caused by pressure ulcers are devastating to the patient and can result to pain, anguish, severe infection which can sometimes lead to death. Nurses experience the severity when there is an outbreak or formation of pressure ulcer which results to a situation where extra time is spent on the patients for assessment, treatment, documentation and continuous evaluation of various interventions. The facility or organization where pressure ulcer prevails suffers not only financial loss, but it also puts its image on the radar compromising the facility's credibility in the eyes of the community, or nation as whole. When all these important consequences are put in a nut shell, it is but imperative that health care industry step up, be proactive, vigilant to map out through efforts of multidisciplinary team, strategies that can minimize the ill effects pressure ulcers have on patients in particular. A well coordinated and integrated plan of care should be implemented to provide safe nursing care that would not only bring comfort and hope to the patients, but would also improve nurses' morale and the image of the facility. The aim of this proposal is to reduce the rate of pressure ulcer in our health care setting, by implementing two key nursing interventions: the use of air-relieve pressure mattress and every two hours peri care to residents who are considered prone to pressure ulcers based on certain risk assessment factors determined by nursing staff. Other various measures of preventing pressure ulcers which includes repositioning, nutrition, skin care management, and effective documentation are included as part of staff education and training. A multidisciplinary approach is used to plan, disseminate and evaluate the proposal through meaningful contributions made by stakeholders.
