Intra-tumor hypoxia profoundly affects the biological behavior of cancer cells, which become resistant to conventional therapies and acquire a more invasive and metastatic phenotype. Hypoxia is a hallmark of the malignant phenotype and a key feature of the tumor microenvironment. Hypoxia Inducible Factor 1 (HIF-1) is a master regulator of the transcriptional response to oxygen deprivation. HIF triggers the expression of genes whose products induce angiogenesis, decrease oxygen consumption, switch metabolism to glycolysis, maintain a stem cell phenotype and select for more invasive and metastatic cells. Therapeutic approaches targeting HIF, directly or downstream mediators of its transcriptional activity, are being developed. Intra-tumor hypoxia is a topic has been gaining scientific interest over the last few years for its wide involvement in many physiological and pathological processes. This volume will cover the latest research and translational aspects associated with intra-tumor hypoxia, along with opportunities for drug development offered by this unique feature of the tumor microenvironment. This new volume discusses the implications of these new findings on the pathophysiology, development, and treatment of hypoxic metabolic acidosis. The volume is part of the Clinical Physiology series sponsored by the American Physiological Society, and is based on a FASEB symposium held in May 1988. Hypoxia was once thought to affect organs in a similar manner, but it is now known that each is affected differently. The author shows how hypoxia and metabolic acidosis affect the heart, lungs, blood vessels and other organs at the cellular level, the tissue level, and finally, at the level of the entire organ. The book then proceeds to a description of the situations in which hypoxic metabolic acidosis develops, such as during high altitude exposure, cardiac arrest, and lactic acidosis. The last few chapters give an overview of treatment. Traditional therapy has consisted largely of the intravenous administration of sodium bicarbonate. This method has come under increasing scrutiny however, and the range of problems associated with the use of sodium bicarbonate is examined fully. Newer alternative agents for managing hypoxic acidosis are reviewed as well. This up-to-date review of hypoxia and metabolic acidosis should be of interest to physiologists, internists, cardiologists, chest physicians, anesthesiologists, and intensive care specialists.

Book excerpt: Adaptation to altitude hypoxia is characterized by a variety offunctional changes which collectively facilitate oxygen trans port from the ambient medium to the cells of the body. All of these changes can be seen at one time or another in the course of hypoxic exposure. Yet, as already stressed (Hannon and Vogel, 1977), an examination of the literature gives only a sketchy and often conflicting picture of the exact nature of these changes and how they interact as a function of exposure duration. This is partly because of the limited number of variables explored in a given study, but it is also attributable to differences in experimental design, differences among species in susceptibility to hypoxia, nonstandardized experimental conditions, lack of proper control of physical (e. g. , temperature) and physiological variables (e. g. , body mass), failure to take measurements at key periods of exposure, and gaps in knowledge about some fundamental mechanisms. Furthermore the available data on animals native to high altitude are meager and/or inconclusive. Extensive further work under well-controlled experimental conditions is required before a detailed picture can be made. Nevertheless, it has been a guiding principle in the prepara tion of this monograph rather to summarize the vastly dis persed material that constitutes the comparative physiology of adaptation to high altitude into a coherent picture, than to provide a comprehensive survey of the field.

The hypoxic zone, an area of low dissolved oxygen that cannot s- port marine life, generally manifests itself in the spring. Since marine species either die or ee the hypoxic zone, the spread of hypoxia reduces the available habitat for marine species, which are important for the ecosystem as well as commercial and recreational shing in the Gulf. Since 2001, the hypoxic zone has averaged 2 1 16,500 km during its peak summer months , an area slightly larger than the state 2 2 of Connecticut, and ranged from a low of 8,500 km to a high of 22,000 km . To address the hypoxia problem, the Mississippi River/Gulf of Mexico Watershed Nutrient Task Force (or Task Force) was formed to bring together represen- tives from federal agencies, states, and tribes to consider options for responding to hypoxia. The Task Force asked the White House Of ce of Science and Technology Policy to conduct a scienti c assessment of the causes and consequences of Gulf hypoxia through its Committee on Environment and Natural Resources (CENR).
