Book excerpt: La TH conduit à différentes étapes de la chirurgie à des variations hémodynamiques majeures et à des complications cardiovasculaires, ces évènements peuvent avoir des implications majeures sur le devenir post-opératoire. La mesure du Strain est une nouvelle technique échographique pour l'évaluation de la contractilité myocardique. L'objectif de cette étude était d'évaluer si la dysfonction systolique au cours de la TH détectée par la mesure du Strain pouvait être associée au devenir du greffon. Cette étude prospective monocentrique a été réalisée au CHU de LYON et des patients adultes devant bénéficier d'une TH ont été inclus de novembre 2020 à juillet 2021. Des mesures via ETO ont été réalisées après l'incision chirurgicale (t1), pendant la période d'anhépathie (t2), après la reperfusion (t3) et à la fin de la chirurgie (t4). Les paramètres analysés étaient des indices de fonction VG habituels et la mesure du Strain longitudinal global (SLG), des indices de pression de remplissage du VG, des indices de fonction VD, y compris la mesure du Strain VD (SPLVD) et l'estimation des pressions artérielles pulmonaires (PAP). Le critère de jugement principal d'évaluer toute association entre les mesures ETO et la dysfonction greffon via le score EAD à J7. A J7, 11/31 patients de la cohorte avaient un score EAD positif, alors que 20/31 avaient un score EAD négatif. Nous avons observé une augmentation du SLG (altération de la contractilité) entre t3 et t2 dans le groupe EAD (1,4 ; IQR 0,2 - 1,9) et une diminution (amélioration de la contractilité) dans le groupe sans EAD (-4 ,7 ; IQR -6,7 - -3,5), p=0,003. Le débit pulmonaire était plus faible à t3 (reperfusion) dans le groupe EAD (2,9L/min (2,8 - 3,5) vs. 4,1L/min (3,2 - 5,4), p=0, 01), et également à t4 (3,4L/min (2,4 - 3,8) vs. 4,5 (3,7 - 5,3), p=0,01). Il n'y avait pas de différence entre les deux groupes concernant les indices de pression de remplissage VG, les indices de fonction VD ou l'estimation des PAP. Une altération persistante de la contractilité myocardique après la reperfusion hépatique détectée comme une augmentation du SLG à t2 sans diminution à t3 était associée à l'EAD à J7. Cette variation était concomitante d'une diminution du DC après la reperfusion qui était également associée à l'EAD à J7.
