L'objectif principal de cette étude était de décrire les besoins en soins de support dans l'après-cancer chez les femmes traitées pour un cancer du sein, afin d'améliorer l'offre de soin. Méthodes : Cette étude est observationnelle, monocentrique, réalisée à l'Institut Jean Godinot à Reims, chez des patientes majeurs, survivantes après un traitement pour un cancer du sein en 2017 (cohorte A) et en 2015 (cohorte B). Elle est basée sur les réponses à un questionnaire d'évaluation des besoins après cancer, la sous-échelle de sévérité du questionnaire de l'inventaire de la peur, et le questionnaire QLQ-C30. Résultats : Cette étude a inclus 139 patientes, 73 traitées en 2017, et 66 traitées en 2015. La douleur (51,9%), la fatigue (51,9%), la prise de poids pendant le traitement (35,1%), les difficultés psycho-sociales (20,5%), et dans la vie de couple et sexuelle (13,1%) étaient les principales plaintes. La sous-échelle de sévérité de l'inventaire de la peur a montré que 28,3% des patientes ont un score entre 13 et 22, et 7,5% supérieur à 22. La peur de la récidive était une source de difficultés sociales, dans la vie de couple et sexuelle, et de troubles psychologiques. Le score global QLQ-C30 était 82,6 sur la population globale. Discussion : La fatigue et la douleur restent les difficultés principales dans l'après cancer. L'activité physique, mais aussi la prise charge de la peur de la récidive sont à intégrer de manière systématique dans le Plan Personnalisé Après Cancer.

Des leviers d'efficience ont été identifiés : favoriser le dépistage du cancer du sein, s'inspirer de l'organisation des centres de lutte contre le cancer, mieux cibler les patientes tirant bénéfice d'une chimiothérapie adjuvante par l'accès à des outils innovants tels que les tests génomiques. Dans une 3ème étude prospective, la décision de chimiothérapie adjuvante était modifiée dans 18% des cas. La génomique entre en routine en sénologie. Les cliniciens doivent comprendre cette discipline. La dernière partie de cette thèse rapporte les travaux menés en génomique dans le cancer du sein, au stade de la recherche sur un gène codant pour une protéine à activité méthyltransférase, PRDM15, prometteuse dans les cancers du sein triple négatif exprimant les récepteurs aux androgènes.

Book excerpt: Les inhibiteurs des kinases dépendantes des cyclines (CdK4/6i) ont modifié la prise en charge et l'évolution du cancer du sein métastatique avec récepteurs hormonaux positifs (RH+) et HER2 négatif (HER2-). Le ratio entre le taux de neutrophiles et de lymphocytes (NLR), indicateur de la réponse inflammatoire systémique de l'hôte, est reconnu comme facteur pronostique indépendant de la survie dans de nombreux cancers solides. Objectif : Évaluer l'impact du NLR sur la survie des patientes atteintes d'un cancer du sein traitées par hormonothérapie (HT) et CdK4/6i en première ligne métastatique ou localement avancé (LA). Matériels et Méthodes : Étude observationnelle rétrospective incluant les patientes suivies pour un cancer du sein métastatique ou LA, traitées par HT et CdK4/6i en première ligne, entre novembre 2015 et décembre 2019, au Centre François Baclesse. Analyse de la survie sans progression (SSP) et survie globale (SG) selon le NLR. Résultats : Au total, 126 patientes ont été incluses dont 71% qui présentaient une rechute métastatique et 29% qui avaient une localisation osseuse isolée. Avec un suivi médian de 33 mois [2,9-57], la SSP médiane et la SG médiane étaient respectivement de 27 et 51 mois. Un NLR pré-thérapeutique élevé (≥ 2,53) était significativement associé à une plus mauvaise SSP (Hazard Ratio (HR)=0,50, [IC95% : 0,32-0,79]) et à une mauvaise SG (HR=0,45, [IC95%: 0,23-0,87]). Conclusion : Chez les patientes suivies pour un cancer du sein RH+/HER2- en première ligne métastatique ou LA, un NLR ≥ 2,53 était associé à une moins bonne survie Le NLR est un marqueur pronostique fiable, peu coûteux et facilement accessible en pratique clinique courante.

Book excerpt: A major work by one of the more innovative thinkers of our time, Politics of Nature does nothing less than establish the conceptual context for political ecology—transplanting the terms of ecology into more fertile philosophical soil than its proponents have thus far envisioned. Bruno Latour announces his project dramatically: “Political ecology has nothing whatsoever to do with nature, this jumble of Greek philosophy, French Cartesianism and American parks.” Nature, he asserts, far from being an obvious domain of reality, is a way of assembling political order without due process. Thus, his book proposes an end to the old dichotomy between nature and society—and the constitution, in its place, of a collective, a community incorporating humans and nonhumans and building on the experiences of the sciences as they are actually practiced. In a critique of the distinction between fact and value, Latour suggests a redescription of the type of political philosophy implicated in such a “commonsense” division—which here reveals itself as distinctly uncommonsensical and in fact fatal to democracy and to a healthy development of the sciences. Moving beyond the modernist institutions of “mononaturalism” and “multiculturalism,” Latour develops the idea of “multinaturalism,” a complex collectivity determined not by outside experts claiming absolute reason but by “diplomats” who are flexible and open to experimentation.

Weber sought to reconstitute the political and social vocabulary of his day.Part I of Caesarism, Charisma and Fate examines a great writer's political passions and the linguistic creativity they generated. Specially, it is an analysis of the manner in which Weber reshaped the nineteenth century idea of ""Caesarism,"" a term traditionally associated with the authoritarian populism of Napoleon III and Bismarck, and transmuted it into a concept that was either neutral or positive. The coup de grace of this alchemy was to make Caesarism reappear as charisma. In that transformation, a highly contentious political concept, suffused with disapproval and anxiety, was naturalized into an ideal type of universal value-free sociology.Part II augments Weber's ideas for the modem age. A recurrent preoccupation of Weber's writings was human ""fate,"" a condition that evokes the pathos of choice, the political meaning of death, and the formation of national solidarity. Peter Baehr, marrying Weber and Durkheim, fashions a new concept, ""community of fate,"" for sociological theory. Communities of fate--such as the Warsaw Ghetto or Hong Kong dealing with the Severe Acute Respiratory Syndrome (SARS) crisis--are embattled social sites in which people face the prospect of collective death. They cohere because of an intense and broadly shared focus of attention on a common plight. Weber's work helps us grasp the nature of such communities, the mechanisms that produce them, and, not least, their dramatic consequences.

Few studies have examined how immigrant entrepreneurs contribute to total U.S. business ownership, formation, and income. This book examines the contributions of immigrant business owners and their businesses to the U.S. economy. The role of immigrants in high-tech entrepreneurship in the U.S. is also quantified. Among other findings, it was determined that nearly 1.5 million immigrant business owners in the U.S. represent 12.5 percent of all business owners. Moreover, the total business income generated by immigrant business owners is $67 billion, 11.6 percent of all business income in the U.S. In addition, although business owners from Mexico constitute the largest share of immigrant business owners, immigrants from around the world are sources of U.S. business formation, ownership and income. These findings indicate that immigrants make large and important contributions to business ownership, formation, and income in the U.S., particularly in some states and economic sectors.
