Drawing on concepts from medical ethics, feminist theory, and Roman Catholic social teaching, Maura A. Ryan analyzes the economic, ethical, theological, and political dimensions of assisted reproduction. Taking seriously the experience of infertility as a crisis of the self, the spirit, and the body, Ryan argues for the place of reproductive technologies within a temperate, affordable, sustainable, and just health care system. She contends that only by ceasing to treat assisted reproduction as a consumer product can meaningful questions about medical appropriateness and social responsibility be raised. She places infertility treatments within broader commitments to the common good, thereby understanding reproductive rights as an inherently social, rather than individual, issue. Arguing for some limits on access to reproductive technology, Ryan considers ways to assess the importance of assisted reproduction against other social and medical prerogatives and where to draw the line in promoting fertility. Finally, Ryan articulates the need for a compassionate spirituality within faith communities that will nurture those who are infertile.

Book excerpt: Each year, roughly a million new cases of cancer appear in the US, and more than 500,000 Americans die annually of premature death. Although medical progress has slowed cancer mortality, its incidence is increasing roughly six times faster than cancer mortality is decreasing. Breast cancer, in particular, has been increasing about one percent each year since 1973. At least two of the factors responsible for this surge in breast cancer are women's use of medically-prescribed synthetic hormones and the exposure of the entire population to chemicals such as dioxin. Both exposures increase the likelihood of breast cancer. Although many ethicists worry about involuntary societal imposition of chemicals such as dioxin, through industrial and agricultural processes, allegedly voluntary exposures also constitute both, a public-health problem and a biomedical-ethics difficulty. Physicians recommend synthetic hormones, for example, to women who apparently take them voluntarily. In the case of in vitro fertilization, doctors prescribe hormones to induce egg production and to increase the chances of reproduction for couples who are unable to have children. Despite the benefits of medical technologies such as hormone stimulation and in vitro fertilization, they also carry great risks. The price that childless women pay, for their opportunity to have children through in vitro fertilization, may be their own increased risk of diseases - such as breast cancer - that are hormone dependent.

Drawing on the expertise of contributors from science, many medical specialities, philosophy, theology and economics, we have sought to address the issues raised by these new developments, as well as a number of long-standing issues that remain as contentious as ever, but of undiminished significance. On the scientific front, the long-predicted technique of embryo biopsy and diagnosis is now a reality. This has prompted the inclusion of some of the topics addressed by this second conference.

This also means that people may become part of an experimental process without their consent. The project brings together clinicians, embryologists, lawyers and ethicists to ascertain whether there might be a risk of infertility patients being exposed to insufficiently tested procedures and to see if there is a need for fundamental research in this area. It also looks at the legislative safeguards that exist and whether a European standard for treatment should be set.

Yet, as the essays in this volume make clear, that is not an accurate assessment of the situ ation. Medical technology has not overwhelmed our moral ity. It would be more accurate to say that our society has not yet achieved consensus about the complex ethical iss ues that arise when medicine tries to assist those who seek its services in order to reproduce. Nevertheless, there is no shortage of ethical opinion about what we ought to do with respect to the use of surrogate mothers, in vitro fertil ization, embryo transfer, artificial insemination, or fertil ity drugs. Nor is it entirely accurate to describe assisted repro duction as technology. The term "technology" carries with it connotations of machines buzzing and technicians scurrying about trying to control a vast array of equip ment. Yet, most of the methods used to assist reproduc tion that are discussed in this volume do not involve exotic technologies or complicated hardware. It is technique, more than technology, that dominates the field of assisted reproduction. Efforts to help the infertile by means of the manipu lation of human reproductive materials and organs date 1 2 Caplan back at least to Biblical times. Human beings have en gaged in all manner of sexual practices and manipulations in attempts to achieve reproduction when nature has balked at allowing life to begin.
