Book excerpt: Background: The research proposed herein investigates factors associated with substance use trajectories leading to injection drug use (injection) and the circumstances surrounding injection within two different populations of injection drug users (IDUs). This study aims to identify factors associated with: (1) initiation of substance use through injection, (2) lifetime and current agua celeste use (an inhalant unique to Northern Mexico), and (3) time to initiation of sex work, and initiation of injection. Methods: Three manuscripts form the basis of this dissertation using existing data from two datasets. Manuscript 1 uses data from IDU from the Proyecto El Cuete phase III dataset, and Manuscript 2 and 3 use data from female sex workers who inject drugs (FSW-IDU) in the Mujer Mas Segura dataset. Results: Manuscript 1 identified a sub-group of IDUs who initiated illicit drug use by injecting the drug ('injection first'). 'Injection first' IDUs were more likely to first inject heroin and inject alone, but were not significantly riskier in their current drug use or sexual behaviors compared to IDUs who used illicit drugs prior to injection. Manuscript 2 found that only FSW-IDUs in Ciudad Juarez reported agua celeste use and prevalence overall (53%) and within the past 6 months (47%) was high. Manuscript 3 identified that early abuse and illicit drug use was associated an accelerated time to initiation of both sex work and injection. Low education and a perceived need to enter sex work to support family were associated with a shorter time to sex work initiation, and geographical migration was found to decelerate time to injection initiation. Having an intimate partner suggest sex work and injecting to deal with feelings of depression accelerated time to both sex work and injection initiation when they occurred at the same age. Conclusions: The unique populations of IDUs studied and novel research questions make the findings within this dissertation valuable to the larger field of research on substance use careers. These findings build on previous literature showing that inhalant use is an important risk factor for subsequent injection and may help identify possible avenues for intervention that can delay or prevent onset of initiation of injection drug use and/or sex work.

Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Book excerpt: US-Mexico border region area has unique social, demographic and policy forces at work that shape the health of its residents as well as serves as a microcosm of migration health challenges facing an increasingly mobile and globalized world. This region reflects the largest migratory flow between any two nations in the world. Data from the Pew Research Center shows over the last 25 years there has never been lower than 140,000 annual immigrants from Mexico to the United States (with peaks over 700,000). This migratory route is extremely hazardous due to natural (e.g., arid and hot desert regions) and human made barriers as well as border enforcement practices tied to socio-political and geopolitical pressures. Also, reflecting the national interdependency of public health and human services needs, during the most recent five year period surveyed the migratory flow between the US and Mexico has equaled that of the flow of Mexico to the US--both around 1.4 million persons. Of particular public health concern, within the US-Mexico region of both nations there is among the highest disparities in income, education, infrastructure and access to health care--factors within the World Health Organization’s conceptualization of the Social Determinants of Health, and among the highest rates of chronic disease. For instance obesity and diabetes rates in this region are among the highest of those monitored in the world, with adult population estimates of the former over 40% and estimates in some population sub-groups for the latter over 20%. The publications reflected in this Research Topic, all reviewed from experts in the field, addressed many of the public health issues in the US Mexico Border Health Commission’s Healthy Border 2020 objectives. Those objectives-- broad public health goals used to guide a diverse range of government, research and community-based stakeholders--include Non Communicable Diseases (including adult and childhood obesity-related ones; cancer), Infectious Diseases (e.g., tuberculosis; HIV; emerging diseases--particularly mosquito borne illnesses), Maternal and Child Health, Mental Health Disorders, and Motor Vehicle Accidents. Other relevant public health issues affecting this region, for example environmental health, binational health services coordination (e.g., immunization), the impact of migration throughout the Americas and globally in this region, health issues related to the physical climate, access to quality health care, discrimination/mistreatment and well-being, acculturative/immigration stress, violence, substance use/abuse, oral health, respiratory disease, and well-being from a social determinants of health framework, are critical areas addressed in these publications or for future research. Each of these Research Topic publications presented applied solutions (e.g., new programs, technology or infrastructure) and/or public health policy recommendations relevant to each public health challenge addressed.

Their illicit trade also adversely impacts governance, security, stability and development in Afghanistan, in its neighbors, in the broader region and beyond. This report, the second such report of the United Nations Office on Drugs and Crime research project on the topic, covers worldwide flows of Afghan opiates, as well as trafficking in precursor chemicals used to turn opium into heroin. By providing a better understanding of the global impact of Afghan opiates, this report can help the international community identify vulnerabilities and possible countermeasures. This report presents data on the distribution of trafficking flows for Afghan opiates and their health impact throughout the world. A worrying development that requires international attention is the increasing use of Africa as a way station for Afghan heroin shipments to Europe, North America and Oceania. This is fuelling heroin consumption in Africa, a region generally ill-equipped to provide treatment to drug users and to fight off the corrupting effects of drug money. Another new trend is the growing use of sea and air transport to move Afghan heroin around the world, as well as to smuggle chemicals used in heroin production into Afghanistan. Traffickers in Afghan heroin have traditionally relied on overland routes, and law enforcement services will need to respond to this new threat. The findings of this report identify areas that need more attention. Strengthening border controls at the most vulnerable points, such as along Afghanistan's border with Pakistan's Baluchistan province, could help stem the largest flows of heroin, opium and precursor chemicals. Increasing the capacity to monitor and search shipping containers in airports, seaports and dry ports at key transit points and in destination countries could improve interdiction rates. Building capacity and fostering intelligence sharing between ports and law enforcement authorities in key countries and regions would help step up interdiction of both opiates and precursor chemicals. Addressing Afghan opium and insecurity will help the entire region, with ripple effects that spread much farther. Enhancing security, the rule of law and rural development are all necessary to achieve sustainable results in reducing poppy cultivation and poverty in Afghanistan. This will benefit the Afghan people, the wider region and the international community as a whole. But addressing the supply side and trafficking is not enough. We need a balanced approach that gives equal weight to counteracting demand for opiates.
