Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. This study is the result of close collaboration between the European Observatory on Health Systems and Policies and the WHO Regional Office for Europe Division of Health Systems and Public Health. In the early twentieth century, many urban reformers claimed these structures would make city government more responsive to the popular will. But on the whole, the effects of these reforms have been to make citizens less likely to vote in local elections and local governments less representative of their constituents. How and why did this happen? Ariane Liazos examines the urban reform movement that swept through the country in the early twentieth century and its unintended consequences. Reformers hoped to make cities simultaneously more efficient and more democratic, broadening the scope of what local government should do for residents while also reconsidering how citizens should participate in their governance. However, they increasingly focused on efficiency, appealing to business groups and compromising to avoid controversial and divisive topics, including the voting rights of African Americans and women. Liazos weaves together wide-ranging nationwide analysis with in-depth case studies. She offers nuanced accounts of reform in five cities; details the activities of the National Municipal League, made up of prominent national reformers and political scientists; and analyzes quantitative data on changes in the structures of government in over three hundred cities. Reforming the City is an important study for American history and political development, with powerful insights into the relationships between scholarship and reform and between the structures of city government and urban democracy.

Book excerpt: Governments fail to provide the public goods needed for development when its leaders knowingly and deliberately ignore sound technical advice or are unable to follow it, despite the best of intentions, because of political constraints. This report focuses on two forces—citizen engagement and transparency—that hold the key to solving government failures by shaping how political markets function. Citizens are not only queueing at voting booths, but are also taking to the streets and using diverse media to pressure, sanction and select the leaders who wield power within government, including by entering as contenders for leadership. This political engagement can function in highly nuanced ways within the same formal institutional context and across the political spectrum, from autocracies to democracies. Unhealthy political engagement, when leaders are selected and sanctioned on the basis of their provision of private benefits rather than public goods, gives rise to government failures. The solutions to these failures lie in fostering healthy political engagement within any institutional context, and not in circumventing or suppressing it. Transparency, which is citizen access to publicly available information about the actions of those in government, and the consequences of these actions, can play a crucial role by nourishing political engagement.
