Book excerpt: Objective: To develop an assessment tool of Thai menopause-specific quality of life in Surat Thani Province, Thailand. Methodology: This study was divided research design into three phases as follows; phase 1 Concept clarification: a thorough study of related literature was conducted to search for the concept of menopause-specific quality of life and its existing instruments to guide developing instrument, phase II Item development: in-depth interviews with 30 Thai menopausal women in Surat Thani was conducted and an initial pool of items relating to menopause-specific quality of life was generated to develop the instrument, then, tested with 399 menopausal women, and phase III psychometric test: the developed questionnaire was tested for validity and reliability with 402 menopausal women. Result: The Thai Menopause-Specific Quality of Life instrument was well developed with 63 items, internal consistency reliability coefficient obtained 0.952, comprised of 3 domains including physic-psychological well-being (51 items), sexual-socio-economic well-being (9 items) and vasomotor well-being (3 items). Conclusion: The present study was directed to develop an effective instrument with integrity of psychometric property. This instrument was more likely meaningful to detect quality of life specific to Thai menopausal women.

Book excerpt: It was over five years ago that the Centre for Medicines Research organized a workshop entitled "Quality of Life: Assessment and Application". This workshop brought together a unique group of participants, some of whom had been involved in studies on quality of life for well over a decade, whilst others were meeting the subject for the first time. This blend of experienced researchers and enthusiastic newcomers was a great stimulus to the discus sions which followed individual presentations as well as that resulting from the study groups. In the ensuing publication, a balance was sought between a consideration of the complex principles underlying the assessment of quality of life and the application of such assessments to specific clinical conditions which necessitated this approach. The organization in 1991 of a second workshop entitled "Quality of Life Assessment: Key Issues in the 1990s" resulted in a further consideration of the quality of life philosophy, concepts and key instruments together with an update on assessing quality of life in a number of major disease areas. Of particular importance was an examination of various viewpoints concerned with ethical questions and their implications, and quality oflife from industry, regulatory and health care purchasers' perspectives. As a result of this second workshop, the editors of the original book referred to above decided to produce a second edition with a number of updates and additional chapters.

Book excerpt: This article reports on the development and psychometric properties of a short-form version of the 50-item Patient Satisfaction Questionnaire III (PSQ-III). The short-form instrument, the PSQ-18, contains 18 items tapping each of the seven dimensions of satisfaction with medical care measured by the PSQ-III: general satisfaction, technical quality, interpersonal manner, communication, financial aspects, time spent with doctor, and accessibility and convenience. PSQ-18 subscale scores are substantially correlated with their full-scale counterparts and possess generally adequate internal consistency reliability. Moreover, both the magnitude of the correlation coefficients and the overall pattern of correlations among PSQ-18 subscales are highly similar to those observed for the PSQ-III. These preliminary analyses support the use of the PSQ-18 in situations where the need for brevity precludes administration of the full-length PSQ-III.

This applies especially to its basic mechanisms: the types of airway sensors, the phar- cological receptors on their membranes, the brainstem organization of the ‘cough centre’, and the involvement of the cerebral cortex in the sensations and the vol- tary control of cough. With the exception of the last of these, nearly all the studies have been on experimental animals rather than humans, for obvious reasons. One group of experimental studies has particular relevance to human patients, and that is the demonstration of the sensitization of cough pathways both in the periphery and in the brainstem. Similar sensitizations have been shown for patients with chronic cough or who have been exposed to pollutants, and it is reasonable to suppose that this is the basis of their cough and that the underlying mechanisms are generally similar in humans and other species. Important advances are also being made in clinical cough research. For the three main causes of clinical cough, asthma, post-nasal drip syndrome, and gast- oesophageal re?ux disease, we are beginning to understand the pathological processes involved. There remains a diagnostically obdurate group of idiopathic chronic coughers, but even for them approaches are being devised to clarify und- lying mechanisms and to establish diagnoses. Perhaps surprisingly, the ?eld in which there has been the least spectacular - vance is the therapy of cough.

All the more surprising then that a group of researchers from several different disciplines, should have come together in the late 1980s with nothing more substantial on the agenda than that they share their thoughts on the topic of measuring the value of health, or more specifically, on the way that the value of health might vary across different countries. Few scientific enterprises can have begun as cautiously or uncertainly. Few can have developed a cohesion and dynamism that lasted decades and continues to drive ahead after long years of scientific endeavour. Such is the good fortune that befell those of us who came together to form what was later to be known as the Euro- Qol Group. The Group's creation is principally due to the shared professional asso- ation of its members with one man, an economist by training and a visionary academic by inclination and temperament - Alan Williams.
