Book excerpt: The United States Air Force (USAF) aeromedical evacuation system, under the command and control of Air Mobility Command (AMC), has a large inventory of medical equipment items that are procured for use on military aircraft. The items must have the capability to be adequately secured within the aircraft, in most cases in the immediate vicinity of the patient. Aeromedical research technicians from the Armstrong Laboratory, Aeromedical Research Function have designed several devices that may be effectively used for securing many of the various medical equipment items likely to be brought aboard the aircraft. It was concluded that there is a slight potential for hazard presented by the protruding thumbscrews on the original Horton Bracket, and it is relatively heavy. The Waters Bracket will probably require relatively high fabrication cost due to design complexity, and also cannot be used on C-130 aircraft. However, provided the devices are fabricated and used as specified in this report, all 5 devices are safe and an effective means for securing selected aeromedical evacuation medical equipment items.

The acceptability/nonacceptability designations apply only to the routine use of a particular piece of equipment in the unique aeromedical evacuation environment of the Department of Defense and are not intended as representations to be relied upon by persons or entities outside of the Department of Defense. This Status Report is intended as a quick reference guide for users. Requisition of equipment/supply items should be initiated through normal procurement channels. The Status Report is divided into the following sections: SECTION 1)- Medical equipment items acceptable for use onboard aeromedical evacuation aircraft and available from the manufacturer; SECTION 2)- Medical equipment items that are acceptable for use onboard aeromedical evacuation aircraft but are no longer available from the manufacturer; SECTION 3)- Medical equipment items that are acceptable for use onboard aeromedical evacuation aircraft; procured via local fabrication or contract; SECTION 4)- Medical equipment items not acceptable for use onboard aeromedical evaluation aircraft; SECTION 5)- Medical supply items acceptable for use onboard aeromedical evacuation aircraft; and SECTION 6)- Medical supply items no longer available from the manufacturer, and medical supply items not acceptable for use onboard aerodynamical evacuation aircraft.

Book excerpt: The US Army Institute of Surgical Research (USAISR) has developed a Special Medical Emergency Evacuation Device (SMEED) platform (US patent pending), designed to secure commercial, off-the-shelf monitors, infusion pumps, ventilators, and similar equipment to the standard NATO litter. This system fits the most commonly used equipment in the US military inventory, and is easily customized to fit other devices. The need for such a system was evident during a mass-casualty training exercise for burns conducted by the USAISR Special Medical Augmentation Response Team (SMART Team) aboard a C 17 USAF aircraft in March 2000: there is no quick and efficient way to secure vital equipment to litters. The following requirements were established for such a product: rugged, lightweight, inexpensive, compatible with current and future commercial devices; customizable to meet individual customer and patient needs and aproved for in-flight use aboard military aircraft. ISR designed the first prototype platform and directed its fabrication at the Air Force Research Laboratory machine shop at Brooks AFB, TX. The SMART Team rigorously tested it aboard a broad range of vehicles during field training exercises at the Soldier-Medic Training Site, Camp Bullis, TX; Ft. Hood, TX; and the Florida Ranger Camp. The platform under went further revision at the USAISR and has been transitioned to advanced product development by the US Army Medical Materiel Development Agency (USAMMDA). The new version (Model IV SMEED) has successfully undergone USAF AFMED testing. The SMEED passed all of the tests successfully and was approved for use during all phases of flight on all USAF Aircraft.

Book excerpt: This research examines the Air Force Aeromedical Evacuation, Patient Movement Items (PMI) Program. This thesis analyzes the primary question of cost savings or equipment deferment based on projected casualty rates. It uses a simple linear program, focused on minimizing beginning inventory, and maps an optimal order plan based on manufacturer capacity and lead time. This thesis suggest updating demand requirements for variability from projected demand using an exponentially weighted moving average calculation. This thesis illustrates that initial deferment can generate substantial savings. This thesis recommends increasing readiness capabilities and cost avoidance by implementing the deferment plan. This thesis recommends additional areas of further research to include consolidating patient movement and inventory tracking systems and utilizing advanced simulation software to determine medical requirements in theaters of operation. These initiatives, if analyzed more thoroughly, could provide DoD policy makers clearer insight for potential system-wide savings.
