Leur diffusion a été définie dans la procédure de « prise de connaissance d’un document qualité ». Cette thèse décrit comment nous avons instauré ce système de management de la qualité grâce à l’engagement des titulaires et l’implication de l’équipe, et quelles ont été les difficultés rencontrées tant sur le plan psychologique que sur le plan rédactionnel avec l’élaboration d’un modèle de procédure, et celles enfin concernant le fond et la forme des procédures.

Book excerpt: Notre état des lieux de la qualité en officine, a mis en avant le retard de l'officine face aux autres métiers de la santé. Bien que divers dispositifs existent déjà pour accompagner le pharmacien, ils ne paraissaient pas suffisants. Face à ce constat l'ordre des pharmaciens a donc choisi de réaliser un nouveau site : https://www.demarchequaliteofficine.fr, ayant pour objectif l'engagement dans la qualité de 100% des officines. Après avoir revu les étapes théoriques de la mise en place d'un système qualité et de ses outils, nous avons étudié la composition du site, comprenant notamment : une autoévaluation, un référentiel qualité officine, ou encore divers outils. La comparaison de ce référentiel aux normes ISO 9001 et l'ISO 15 189, grandes références qualité en santé, montre que le référentiel paraît complet. Lors de l'étude de sa mise en place dans une officine qui a duré six mois, nous avons pu tester ce qui est proposé. Les avantages principaux de ce qui est proposé, sont sa gratuité, sa spécificité à l'officine, sa facilité d'utilisation et de compréhension. Il réunirait toutes les clés pour débuter un système qualité. Nous avons des pistes d'amélioration comme l'organisation d'un système documentaire, l'accompagnement avec des indicateurs qualité, la proposition d'une procédure mère vierge, ou encore l'introduction à certains outils de base de la qualité.

Elle constitue une réelle remise en question du fonctionnement général de la pharmacie mais aussi des pratiques individuelles. Comme tout changement, cela entraîne un certain nombre de réactions de la part des personnes qui le vivent : refus de comprendre, résistance, résignation... Pour réussir, il est nécessaire de conduire le changement en utilisant une démarche de gestion de projet et en prenant en compte la dimension humaine. Cette thèse décrit comment nous avons instauré une démarche qualité grâce à la mise en place d’une démarche structurée basée sur la participation de l’ensemble de l’équipe et sur la communication. Malgré les difficultés rencontrées, notre travail a abouti à la création d’un système documentaire qualité, à une sécurité renforcée de la délivrance, à une meilleure communication grâce à un circuit de l’information accessible à tous les collaborateurs, et à une meilleure cohésion de l’équipe. Ceci n’aurait pas été possible si nous n’avions pas réussi à vaincre les principales résistances et obtenu l’implication de la majorité de l’équipe.

Book excerpt: Les pharmaciens ont pris conscience de la nécessité de mettre en place une démarche qualité dans les officines. Ils ne sont pas tous d'accord sur la façon de le faire. Face aux détracteurs de la norme ISO 9001 spécifiant qu'elle n'est pas adaptée à l'exercice officinal, les groupements de pharmaciens ont lancé une dynamique de certification QMS Pharma / ISO 9001. En parallèle, l'Ordre National des Pharmaciens n'est pas prêt à imposer un référentiel même s'il incite à mettre en place l'assurance qualité à l'officine, au travers du pharmacien responsable assurance qualité. Les référentiels QMS Pharma et ISO 9001 sont complémentaires. Le premier est orienté « métier », le second « gestion ». Le QMS Pharma, référentiel de l'organisation ordinale PharmaSuisse s'attache à valoriser le rôle du pharmacien et à évaluer la sécurisation des actes. La norme NF EN ISO 9001 s'attache à évaluer un système de management de la qualité à tous les niveaux hiérarchiques et dans tous les domaines de l'entreprise sans entrer dans les détails du coeur de métier et tout en permettant d'améliorer les pratiques quotidiennes au travers de la qualité globale. Sa généralité lui confère cette force de s'appliquer à n'importe quel domaine d'activité tout en le faisant progresser. Enfin face à une situation économique qui se dégrade pour les officines, elle doit permettre d'apporter un certain nombre de réponses pour éviter la non-qualité. C'est pourquoi elle s'attache à d'autres pans que le coeur de métier, à savoir par exemple la gestion, les achats.

The small French town of Ainay-le-Ch�teau and its environs are the site of a "family colony" for men, established in 1900.
