Book excerpt: ABSTRACT: Research suggests that more than ten percent of older adults experience behavioral health problems (including mental health problems and/or substance abuse). However, very few actually receive care from a behavioral health care provider or even a primary care provider. One major barrier to accessing and receiving care is the feeling of perceived stigma commonly associated with behavioral health problems. The present study examined the relationships among attitudinal variables, feelings of stigma, and behavioral health outcomes over time in an elderly population with the secondary analysis of data collected for a previously implemented research study, the Primary Care Research in Substance Abuse and Mental Health for Elderly (PRISM-E) study. The PRISM-E research project was a multisite, randomized, comparative trial examining two models of care for persons aged 65 and older with symptoms of depression, anxiety, and/or at-risk drinking. A total of 2,022 participants over the age of 65 were included in the database. Over half of the participants indicated that they had some feelings of stigma associated with mental health and substance abuse issues. An examination of the measure used in the PRISM-E study to measure stigma revealed the presence of two factors, or components, of stigma that we labeled Perceived Stigma and Comfort Level. Statistical analyses of the data demonstrated that feelings of stigma are not constant and can indeed change over time. However, in this sample, perceived stigma was not related to behavioral health outcomes, such as a reduction in symptoms of depression, anxiety, and/or at-risk drinking. Limitations of this study include the possibility that the sample may be biased due to the fact that all participants were under the care of a primary care provider and all agreed to enter treatment for their behavioral health problem. Implications of the findings are that it is possible to influence feelings of stigma and previous research has demonstrated that lower levels of feelings of stigma in older adults as well as other age groups may lead to improvements in accessing and engaging in behavioral health treatment.

Book excerpt: The main purpose of the present study was to ascertain the nature of mental health services preferred by the community members in order to deal with the psychological problems of the aged. Specifically, the problem studied was whether people have a custodial orientation or community orientation for prevention, treatment and management of psycho logical problems in the elderly population. The relationship between attitude toward the elderly and preference for the services was also examined. In addition, the study investigated the relationship demo graphic variables have with preference for services for the psychologic ally impaired elderly, and with attitudes toward old persons. The fol lowing three instruments were used to obtain the required information: (1) Psychogeriatric Services Scales (PSS), (2) Old People Scale (OPS), and (3) Demographic Information Questionnaire (DIQ). The PSS was a specially constructed scale which measured preference for 27 different types of services representing 4 main categories: (a) Preventive Care, (b) Improved Institutional Care, (c) Community Care, and (d) After Care. The questionnaires were mailed to a random sample of 300 households in the city of Edmonton. Of these, 69 returns were complete enough for data analysis. The results show that most respondents tend to prefer community oriented services for the psychological problems of the elderly. No one has definite custodial orientation. However, a significantly high percentage (63%) of the subjects have low community orientation. whereas only 27% have high community orientation. With respect to the four groups of services, the highest preference is for Preventive Care Services followed by Improved Institutional Care, Community Care and After Care. As for the respondents' attitudes toward older persons, no subject has a definite negative attitude. However, a considerable per centage (41%) of the subjects have a low positive attitude toward the elderly as compared to those (23%) in the high attitude category. The correlation coefficient between attitude toward old persons and pre ference for services is small, suggesting that the nature of preference may not be predicted from the knowledge of one's attitude toward old persons. It is also found that, in general, most demographic variables are not related to either preference for services or to attitudes toward older persons.

Book excerpt: Mental health problems exist in ethnically and sexually diverse middle-aged and older adults, however, formal mental health services remain underutilized. Multiple factors, including acculturation and stigma prevent an individual from seeking professional help and in some cases encourage support seeking from informal sources such as family and friends. Despite the common concerns, research is limited among individuals with multiple cultural identities. The purpose of this study is to examine whether cultural factors including ethnicity, sexual orientation, and the intersection between these two cultural identities, affect mental help-seeking behaviors in the middle-aged and older adult population. As part of a larger internet and mail-in survey study, two hundred and thirty five ethnically and sexually diverse middle-aged and older adults from the United States participated in this dissertation. Questionnaires assessing level of acculturation, attitudes towards professional mental health, and perceived level of friend and family support were completed. Results suggest a significant positive relationship between level of acculturation and attitudes towards professional mental health help. Ethnically and sexually diverse participants did not demonstrate differences in attitudes towards support from professionals or from family and friends. Those with intersecting identities also did not result in significant findings for informal and formal types of help. Lack of significant results in this study may be attributed to several limitations, including having a non-clinical sample and restriction of geographical range in participants. Additional research should be performed to examine the growing number of older adults with intersecting identities. Integrating knowledge of mental health services with informal support systems may generate increased use of mental health services.

Book excerpt: Stigma associated with mental illness continues to be a significant barrier to help seeking. This book examines the impact of stigma on racial differences in treatment seeking attitudes and behaviors among older adults with depression. Findings indicate that older adults with depression have high levels of public stigma and do not intend to seek, nor are they currently engaged in mental health treatment. Findings also suggest that African American older adults endorse higher internalized stigma and less positive attitudes toward seeking mental health treatment than their white counterparts. Older African Americans identified a number of experiences living in the black community that impacted their treatment seeking attitudes and behaviors, which led to their identification and utilization of more culturally endorsed coping strategies to deal with their depression. This book provides an in depth understanding of the stigma associated with having a mental illness and its' influence on attitudes and intentions toward seeking mental health services among older adults, particularly African American elders.
