Book excerpt: Excerpt from Contributions to the Study of Diseases of the Heart and Lungs These monographs which from time to time have appeared in the medical journals are here collected, and the dates of their publication given, at the request of very many medical friends in widely different parts of the country. This publication by no means aspires to the dignity of a systematic treatise, but embodies some new views of a revolutionary character, which may be of some practical use to the profession. All that is new in them of physical signs in diagnosis of pathological processes connected with the organs of respiration, had its origin in the discovery of true respiratory murmur, its cause and site in 1859-60. It is true that bronchial respiration and pulmonary respiration are terms which were used by Laennec. They more correctly describe the site of their origin than the term substituted by Andral, vesicular murmur. At that time the existence and active quality of residual air was but little known. Laennec taught, as did his followers, that the cause of respiratory murmurs was friction of air in motion, meeting resistance in its course in the air passages and in the air cells. Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

Comprehensive and accurate disease surveillance systems are needed to implement successful efforts which will reduce the burden of chronic diseases on the U.S. population. A number of sources of surveillance data-including population surveys, cohort studies, disease registries, administrative health data, and vital statistics-contribute critical information about chronic disease. But no central surveillance system provides the information needed to analyze how chronic disease impacts the U.S. population, to identify public health priorities, or to track the progress of preventive efforts. A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases outlines a conceptual framework for building a national chronic disease surveillance system focused primarily on cardiovascular and chronic lung diseases. This system should be capable of providing data on disparities in incidence and prevalence of the diseases by race, ethnicity, socioeconomic status, and geographic region, along with data on disease risk factors, clinical care delivery, and functional health outcomes. This coordinated surveillance system is needed to integrate and expand existing information across the multiple levels of decision making in order to generate actionable, timely knowledge for a range of stakeholders at the local, state or regional, and national levels. The recommendations presented in A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases focus on data collection, resource allocation, monitoring activities, and implementation. The report also recommends that systems evolve along with new knowledge about emerging risk factors, advancing technologies, and new understanding of the basis for disease. This report will inform decision-making among federal health agencies, especially the Department of Health and Human Services; public health and clinical practitioners; non-governmental organizations; and policy makers, among others.

Over the years a variety of techniques have been applied to the study of the pulmonary circulation to acquire a greater understanding of the alterations occurring in various diseases. Congenital heart disease has been of particular interest since it may impose unusual hemodynamic stresses upon the vessels of the lungs. For over two decades we have had the opportunity, first at Yale-New Haven Hospital, New Haven, Connecticut and then at University Hospital, San Diego, California, to study the pulmonary and bronchial circulations in patients with congenital heart disease. Much has been learned from the application of new anatomic tech niques to such patients. The clinical correlations, it is to be hoped, will help to bridge the gulf between anatomic and physiologic data. These observa tions have been recorded with the thought that pathology often illuminates normal anatomy, and both contribute to the understanding of physiology. Incursions into the several realms should perhaps not be considered trespass but rather an attempt to glimpse more of one country. The sources of help for a book of this scope are bound to be many and varied. My colleague Dr. Averill A.
