Il existe des systèmes d'attaches traditionnelles type crochets répondant fonctionnellement aux principes de la triade de Housset ; mais, desservis par une esthétique discutable. C'est en cela que les systèmes d'attaches de précision (ou attachements) trouvent leur indication. En effet, les attachements permettent d'assurer la bonne rétention de la PPAC tout en s'insérant de façon discrète et esthétique au niveau des dents prothétiques. Enfin, ce travail permet de montrer qu'implantologie et PPAC ne sont pas contradictoires et qu'ils peuvent être associés pour améliorer considérablement les propriétés d'une prothèse partielle amovible avec un nombre limité d'implants. Il n'est donc pas ou plus concevable de ne proposer que du "tout fixe".

Une première expérimentation a permis de définir un "coefficient de croissance de la rétention" ou "C.C.R." et un rôle clinique de l'angle de rétention. Une deuxième expérimentation qui consiste en une étude au profilomètre des coupes des dents supports, au niveau de l'emplacement de la pointe de l'extrémité du crochet et suivant l'axe d'insertion. Elle a permis de montrer l'existence d'un modèle mathématique représenté par la courbure d'un arc de cercle dont l'approximation a été jugée satisfaisante.

Devant la multiplicité des systèmes décrits et l'effervescence des auteurs et industriels à théoriser la prothèse amovible partielle, nombres d'échecs apparurent. Ils furent à tort attribués aux systèmes d'attaches et non à la conception globale de la prothèse. Afin de pallier aux déficiences de conception des prothèses, de nombreuses classifications et indications selon l'édentement à traiter virent le jour, comme les attachements mono ou bilatéraux et les attachements rupteurs. Cette thérapeutique fut alors décriée, jugée trop complexe et partiellement abandonnée en pratique quotidienne. Ces dernières années, les attachements ont connu un regain d'intérêt dans le traitement des édentements partiels. La simplification des systèmes d'attaches, de leurs indications ainsi que l'avènement de l'implantologie en PAP ont transformé leur usage en pratique quotidienne. Une classification remaniée fait écho à ce changement de paradigme. Un guide pratique avec notamment des tableaux récapitulatifs conclura ce travail de fin d'étude.
