Book excerpt: Review of literature shows that contrast-induced nephropathy (CIN) continues to be a risk for patients undergoing angiography with contrast media (CM). With increased age in the population, more elderly patients are undergoing procedures that require administration of CM, and most of those patients are already dealing with advanced health conditions. An average of 10% of patients in the outpatient setting are experiencing CIN (Mitchell, Jones, Tumlin, and Kline, 2010) and CIN is the third leading cause of hospital-acquired renal failure (Bansal, 2012). These health conditions along with high doses of contrast media are putting patients at a higher risk for developing CIN. CIN is described as a significant decrease in renal function after contrast media that is measured by serum creatinine (SCr) and estimated glomular filtration rate (eGFR). CM affects the tubular cells, causes a decrease in real blood flow, cellular damage and renal vasoconstriction. With more procedures being done in outpatient centers it has become vital that health care providers, physicians and nurses are aware of risks and prevention associated with CIN. Prevention methods such as hydration, avoiding nephrotoxic medications and limiting the amount of contrast use are the best measures to take with every patient undergoing angiography with CM. It has been proven through research that volume expansion is the most effective way to prevent renal failure that is caused by CM, and sodium bicarbonate (NaHCO3) along with sodium chloride (NaCl) is far more superior to NaCl alone. Proposal presented to Pedes Orange County to adapt a policy to use NaHCO3 along with hydration of NaCl in patients that present for angiography with an eGFR below 60 mL/min/1.73 m2. Education and information is given how to evaluate patients before and after procedures and what patients are at risk for CIN. By incorporating prevented measures to reduce the risk of CIN, fewer patients will have to endure the life altering effects of renal damage associated with CM.
