The three goals of optimizing such systems are ensuring safety, efficiency and economy. They confronted a deceptively simple question: When is a reactor “safe enough” to adequately protect the public from catastrophe? Some experts sought a deceptively simple answer: an estimate that the odds of a major accident were, literally, a million to one. Far from simple, this search to quantify accident risk proved to be a tremendously complex and controversial endeavor, one that altered the very notion of safety in nuclear power and beyond. Safe Enough? is the first history to trace these contentious efforts, following the Atomic Energy Commission and the Nuclear Regulatory Commission as their experts experimented with tools to quantify accident risk for use in regulation and to persuade the public of nuclear power’s safety. The intense conflict over the value of risk assessment offers a window on the history of the nuclear safety debate and the beliefs of its advocates and opponents. The HRP is sponsored by a group of national organizations, representing nuclear power plant regulators, utilities, and research institutions. The HRP is hosted by the Institute for Energy Technology (IFE) in Halden, Norway. HAMMLAB comprises three full-scale nuclear power plant control room research simulators. The simulator studies performed in HAMMLAB have traditionally been experimental in nature. In a simulator it is possible to study events as they unfold in real time, in a highly realistic operational environment under partially controlled conditions. This means that a wide range of human factors issues, which would be impossible or highly impracticable to study in real-life settings, can thus be addressed in HAMMLAB. Simulator-based Human Factors Studies Across 25 Years celebrates the twenty-fifth anniversary of HAMMLAB by reviewing the human factors studies performed in HAMMLAB across this time-span. A range of human factors issues have been addressed, including: • human-system interfaces; • alarm systems; • computerized procedures; • human-automation interaction; • staffing, teamwork and human reliability. The aim of HAMMLAB studies has always been the same: to generate knowledge for solving current and future challenges in nuclear power plant operation to contribute to safety. The outcomes of HAMMLAB studies have been used to support design and assessment of nuclear power plant control rooms.

In this exciting book, Gene Rochlin takes a closer look at how these familiar and pervasive productions of computerization have become embedded in all our lives, forcing us to narrow the scope of our choices, our modes of control, and our experiences with the real world. Drawing on fascinating narratives from fields that range from military command, air traffic control, and international fund transfers to library cataloging and supermarket checkouts, Rochlin shows that we are rapidly making irreversible and at times harmful changes in our business, social, and personal lives to comply with the formalities and restrictions of information systems. The threat is not the direct one once framed by the idea of insane robots or runaway mainframes usurping human functions for their own purposes, but the gradual loss of control over hardware, software, and function through networks of interconnection and dependence. What Rochlin calls the computer trap has four parts: the lure, the snare, the costs, and the long-term consequences. The lure is obvious: the promise of ever more powerful and adaptable tools with simpler and more human-centered interfaces. The snare is what usually ensues. Once heavily invested in the use of computers to perform central tasks, organizations and individuals alike are committed to new capacities and potentials, whether they eventually find them rewarding or not. The varied costs include a dependency on the manufacturers of hardware and software--and a seemingly pathological scramble to keep up with an incredible rate of sometimes unnecessary technological change. Finally, a lack of redundancy and an incredible speed of response make human intervention or control difficult at best when (and not if) something goes wrong. As Rochlin points out, this is particularly true for those systems whose interconnections and mechanisms are so deeply concealed in the computers that no human being fully understands them.
