Computers are being integrated into almost every medical imaging system. Medical Image Analysis and Informatics demonstrates how quantitative analysis becomes possible by the application of computational procedures to medical images. Furthermore, it shows how quantitative and objective analysis facilitated by medical image informatics, CBIR, and CAD could lead to improved diagnosis by physicians. Whereas CAD has become a part of the clinical workflow in the detection of breast cancer with mammograms, it is not yet established in other applications. CBIR is an alternative and complementary approach for image retrieval based on measures derived from images, which could also facilitate CAD. This book shows how digital image processing techniques can assist in quantitative analysis of medical images, how pattern recognition and classification techniques can facilitate CAD, and how CAD systems can assist in achieving efficient diagnosis, in designing optimal treatment protocols, in analyzing the effects of or response to treatment, and in clinical management of various conditions. To this end, a group of 85 psychologists and engineers coming from industry, government, and academia convened to discuss, and to generate a "state-of-the-art" consensus of the problems and solutions associated with the human IS ability to cope with the increasing scale of consequences of failures within complex technical systems. The Introduction of this volume reviews their findings. The Symposium was organized to include brief formal presentations of papers sent to participants about two months in advance of the meeting, and considerable discussion both during plenary sessions and within more specialized workshops. Summaries of the discussions and workshop reports appear in this volume.

Book excerpt: The germs of the ideas in this book became implanted in me during my experience as a resident in clinical pathology at Boston University Medical Center. At the time, I had inklings that the test results churned out by our laboratories were more than scientific facts. As a philosophically unsophisticated young physician, however, I had no language or framework to analyze what I saw as a deep philosophical problem, a problem largely unrecognized by most physicians. The test results provided by our laboratories were accurate and of great practical importance for patient care. However, most of the physicians who relied on our test results to diagnose and treat their patients either did not have the time or interest to consider the philosophical issues inherent in diagnosis, or, like me, had inadequate means to further analyze them. It was more than ten years later that I began doctoral studies in philosophy, and I was fortunate to find a faculty that was supportive of my efforts to address the problem. This book began as my doctoral dissertation in the Department of Philosophy at Georgetown University. I would like to acknowledge the assistance of my mentor, Robert Veatch, Ph. D. In Digitizing Diagnosis, Andrew Lea examines these efforts—and the larger questions, debates, and transformations that emerged in their wake. While surveying the continuities spanning the analog and digital worlds of medicine, Lea uncovers how the introduction of the computer to medical diagnosis reconfigured the identities of patients, diseases, and physicians. Debates about how and whether to apply computers to the problem of diagnosis, he demonstrates, were animated by larger concerns about the nature of medical reasoning, the definitions of disease, and the authority and identity of physicians and patients. In their attempts to digitize diagnosis, these interdisciplinary groups of researchers repeatedly came up against fundamental moral and philosophical questions. How should doctors classify diseases? Could humans understand, and come to trust, the opaque decision-making processes of machines? And how might computerized systems circumvent—or calcify—bias? GENERAL The term "diagnostics" refers to the general theory of diagnosis, not to the study of specific diagnoses but to their general framework. It borrows from different sciences and from different philosophies. Traditionally, the general framework of diagnostics was not distinguished from the framework of medicine. It was not taught in special courses in any systematic way; it was not accorded special attention: students absorbed it intuitively. There is almost no comprehensive study of diagnostics. The instruction in diagnosis provided in medical schools is exclusively specific. Clinical instruction includes (in addition to vital background information, such as anatomy and physiology) specific instruction in nosology, the theory and classification of diseases, and this includes information on diagnoses and prognoses of diverse diseases. What is the cause of the neglect of diagnostics, and of its integrated teaching? The main cause may be the prevalence of the view of diagnostics as part-and parcel of nosology. In this book nosology is taken as a given, autonomous field of study, which invites almost no comments; we shall freely borrow from it a few important general theses and a few examples. We attempt to integrate here three studies: ll of the way nosology is used in the diagnostic process; of the diagnostic process as a branch of applied ethics; ~ of the diagnostic process as a branch of social science and social technology.
