Malnutrition, however, remains prevalent. This book details strategies and practical approaches designed to alleviate hunger and malnutrition in a new era where technological change, markets, patterns of governance, and social programs have an increasingly global dimension. More specifically, this book addresses a range of considerations including the role of small farmers in a world where the global reach of multinational corporations have enormous control from the farm to local markets and the grocery store; misgivings and misperceptions about genetically modified foods; the increasing competition of food and energy sectors for agricultural output; the importance of micronutrient deficiencies and chronic disease related to obesity, which often coexists in the same communities as hunger; and issues of sustainability of the food and agricultural system in an period when there is increasing concerns over global warming and environmental degradation. Currently there is also more emphasis on evidence-based policymaking, which has raised the standard of proof for evaluating the impact of micro-level interventions that have traditionally been so widely embraced and are now under increased scrutiny. It is in this context that this book provides practical advice on programs that can effectively target those at greatest risk of malnutrition and guidance on policies to promote a healthy and sustainable food and agricultural system. Overlaying all of these challenges is the book's emphasis on both identifying data and information needs for decision-making, and practical considerations for better understanding the domestic and international political and social constraints that need to be addressed when trying to translate scientific knowledge and information into practice.

These guiding principles take a holistic approach to diets; they consider international nutrition recommendations; the environmental cost of food production and consumption; and the adaptability to local social, cultural and economic contexts. This publication aims to support the efforts of countries as they work to transform food systems to deliver on sustainable healthy diets, contributing to the achievement of the SDGs at country level, especially Goals 1 (No Poverty), 2 (Zero Hunger), 3 (Good Health and Well-Being), 4 (Quality Education), 5 (Gender Equality) and 12 (Responsible Consumption and Production) and 13 (Climate Action).

Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

Book excerpt: This paper analyses the potential signalling effect from a tax on sugar-sweetened beverages (SSBs) that Catalonia introduced in May 2017, with a view to understand the ways in which, and the extent to which taxes discourage SSBs consumption. A questionnaire was distributed in November 2019 in two neighbourhoods from Barcelona with different mean income levels. We explored associations between variables constructed from the questionnaire and declared reduced consumption. Our goal was to disentangle the different effects from the price increase itself and from the signalling effect of the price increase on the reduction of SSBs consumption, analyzing results that account for socio-demographic characteristics, in order to provide policy recommendations. Respondents mainly declared that a higher awareness of SSBs’ health risks made them curb their consumption of SSBs (98.5 percent). Only 10.6 percent of them declared that the new price was considered high enough to have discouraged their SSBs consumption. Nevertheless, the tax in Catalonia seems to have had a signalling effect through the increase in prices (100 percent pass-through of the law), and much less through the awareness of the existence of the tax. The fact that people had noticed the increase in prices is associated to a recent knowledge of SSBs’ health risks, which is itself related to reduced consumption in the sample. Implementing higher and more salient SSBs taxes on the shelves, and having public campaigns making sure taxes are known among the entire population, aiming at behavioural changes among young populations, would enhance their signalling effect.

Many of them suffer lifelong consequences in terms of higher risk of mortality and future illness, impaired cognitive ability (including lower IQ) and educational attainment, and overall productivity loss. This report provides the findings from an in-depth data-based analysis of malnutrition in Ethiopia and its causes. It assesses various aspects of current nutrition programming in the country, noting the importance of Ethiopia's first National Nutrition Strategy and National Nutrition Program established in 2008. The report also examines key details including costs and benefits of a range of interventions against malnutrition in the country, including some that are not yet being implemented but could potentially be introduced. The analysis draws on data from household surveys, detailed program data and data from various sources on costs, impacts and potential coverage, among others. The report finds that contrary to what is commonly believed and traditionally used to guide policymaking, a substantial amount of the malnutrition in Ethiopia is due to factors other than food insecurity, pointing to the need for a multi-sectoral approach. Nutrition interventions in general are found to have high benefit-to-cost ratios - sometimes in the double or triple digits - with these ratios being especially high for micronutrient interventions, insecticide-treated bednets, deworming and community-based interventions. Among others, the report recommends the introduction of deworming for pregnant women in Ethiopia - which has the highest benefit-to-cost ratio among the interventions analyzed - and scaling up community-based interventions. These include the Community-Based Nutrition program which shows impressive results in Ethiopia after just over two years of implementation. The report also recommends various ways of improving the effectiveness of nutrition programming including by improving program targeting, enhancing coordination and linkages among programs, and establishing an effective nutrition information and surveillance system.

Book excerpt: On an Empty Stomach examines the practical techniques humanitarians have used to manage and measure starvation, from Victorian "scientific" soup kitchens to space-age, high-protein foods. Tracing the evolution of these techniques since the start of the nineteenth century, Tom Scott-Smith argues that humanitarianism is not a simple story of progress and improvement, but rather is profoundly shaped by sociopolitical conditions. Aid is often presented as an apolitical and technical project, but the way humanitarians conceive and tackle human needs has always been deeply influenced by culture, politics, and society. Txhese influences extend down to the most detailed mechanisms for measuring malnutrition and providing sustenance. As Scott-Smith shows, over the past century, the humanitarian approach to hunger has redefined food as nutrients and hunger as a medical condition. Aid has become more individualized, medicalized, and rationalized, shaped by modernism in bureaucracy, commerce, and food technology. On an Empty Stomach focuses on the gains and losses that result, examining the complex compromises that arise between efficiency of distribution and quality of care. Scott-Smith concludes that humanitarian groups have developed an approach to the empty stomach that is dependent on compact, commercially produced devices and is often paternalistic and culturally insensitive.
