The role of the Army Medical Department is to conserve the fighting strength through preventive medicine, evacuation, and treatment of the sick and wounded. Minimizing death and morbidity requires appropriate and timely evacuation and treatment. Joint medical doctrine has been changed to emphasize evacuation of less-stable patients out of theater and decrease the theater hospital footprint. Review of recent military operations indicates that hospital care has played a large role and that prolonged evacuation resulted in increased mortality. However, deployed hospitals are large and slow to deploy. Army transformation will result in units that are rapidly deployable and can enter combat upon arrival in a theater of operations. Combat, especially forced entry operations, results in casualties that must be cared for. Current Army medical force structure does not support the operations considered likely with Army transformation. The Army Medical Department must transform to provide high quality combat casualty care to the Army of 2015.

Book excerpt: The Army is in the process of transforming itself and is developing new operational concepts and technologies to enable this transformation. The Army Medical Department (AMEDD) has been deeply involved in the overall Army transformation effort since its inception, and it has identified operational medicine issues whose resolution will be critical to its ability to support the Army of the future. The purpose of this research was to develop a method to assess these issues and further investigate a subset of them. Rand Arroyo Center designed and conducted the Army Medical Department Transformation Workshops (ATW) held in April, August, and November 2002. The purpose of the workshops was to initiate an assessment of the medical risks associated with emerging Army operational concepts and the capacity of AMEDD to mitigate these risks. Medical risk is defined generally as the number, severity, and fate of casualties incurred. The workshops examined combat operations of a notional future force, each supported by a different health service support (HSS) structure. Eight hours of simulated combat provided the context for the workshops, generating casualty data to support the analysis of the HSS structure. AMEDD experts determined likely outcomes for individual casualties resulting from the Army simulation of its 2015 future force. It was concluded that, in this particular simulation, the structure and concepts postulated for the evacuation and treatment of future force combat casualties were overwhelmed by 8 hours of relatively low-intensity combat. This report describes the development of issues that provided a basis for the workshops, workshop organization, the composition of the various teams and cells, objectives and issues, the scenario used, and the analysis methodology employed. Finally, the report provides results and observations. (3 tables, 2 figures).

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Furthermore, the report analyzes how these weaknesses affect the PLA's performance of missions tasked by Beijing. China's military isn't ready to win wars despite spending heavily to modernize. The People's Liberation Army suffers from 'potentially serious weaknesses' that could limit its ability to conduct the operations required to fight and win future conflicts. "Although the PLA's capabilities have increased dramatically, its remaining weaknesses increase the risk of failure to successfully perform the missions the Chinese Communist Party leaders may task it to perform," the report says. It cited Taiwan contingencies, maritime claim missions, protecting sea lines of communications and some non-war military operations. The report looks at two critical shortcomings: institutional and combat capabilities. On institutional issues, the PLA faces shortcomings regarding outdated command structures, quality of personnel, professionalism and corruption. Combat weaknesses include logistical, insufficient strategic airlift capabilities, limited numbers of special-mission aircraft, and deficiencies in fleet air defense and anti-submarine warfare.

They also agreed to strengthen health systems address emerging multi-drug and insecticide resistance and intensify national cross-border and regional efforts to scale up malaria responses to protect everyone at risk.
