Book excerpt: This study seeks to determine whether Internet-Based Expressive Writing (IB-EW), a brief, low-cost, easily disseminated, and resource-efficient intervention, can reduce psychological symptoms and improve functioning among veterans returning from hazardous deployments. Although Expressive Writing's evidence-base is strong in civilian populations, its efficacy in combat veterans has not been tested. Nevertheless, Expressive Writing, as a highly private, readily accessible, and non-stigmatizing intervention, holds exceptional promise in overcoming barriers to mentally distressed veterans? help-seeking. We expect to further increase the accessibility of the intervention by delivering it over the internet (Internet- Based Expressive Writing). Long term objectives of this line of research are to develop and implement efficient, accessible, and effective interventions that facilitate combat deployment-to-civilian life transitions, thereby reducing risk of long-term, military related psychopathology and disability. Toward that end, the study will also attempt to identify individual difference characteristics related to the efficacy of the treatment, to indicate who is most likely to benefit from the treatment in order to inform treatment implementation strategies.

The worry and uncertainty associated with combat deployments provokes anxiety in family members left at home. Lengthy separations may challenge the personal, social, and economic coping resources of families at home. In this war, thanks to medical advances, many service members who previously would have died of their injuries are returning home to live long, although altered lives. As a result, families are facing the additional challenge of assisting service members who have experienced amputation, traumatic brain injury, and psychological wounds. These challenges are faced not only by service members in the active component of the armed forces, but also by service members in the National Guard and Reserves. In response, the Department of Defense has launched unprecedented efforts to support service members and families before, during and after deployment in all locations of the country as well as in remote locations. These support efforts are focused not only on medical care, but also mental health care and logistical support. Research about families and war tends to move forward in fits and starts associated with major conflicts, and there is currently an increasing flow of family research moving into the scientific domain. Military Families and the Aftermath of Deployment focuses heavily on the aftermath of deployment for families. It is the first compilation of such chapters released in relation to the wars in Iraq and Afghanistan, and includes contributions from leading researchers from diverse disciplines and arenas, including universities, the Veterans Administration, and the Department of Defense, as well as international researchers from Canada, and Croatia, among others. This work will be of use to graduate students and researchers in family studies, social work, counseling, military science, psychology and sociology.

Book excerpt: Battlefield-related isolation from family and community, post-traumatic stress disorder, and the complications military personnel encounter while transitioning to civilian life following deployment are poorly understood. Situational-Specific Transition Theory (SSTT) is suitable for use in explaining, exploring, and understanding periods of transition for military personnel undergoing major life transitions after deployment. Using a qualitative descriptive design and drawing on content analysis, this study explores how deployed military personnel self-report changes in family and community dynamics during the process of reintegration following deployment. Seventeen major themes and subthemes were collapsed into 5 overreaching themes: Deploying, Emotionally and physically drained, getting my life together, needing civilian and military family support and reflection of military service. Additionally, the five overarching themes were reduced into one 1 grand theme that captures the meanings associated with the narrative data: Challenging experience. This study adds to the literature on deployment by showing that service members reintegration to family and society has not been fully understood. Findings in this study illustrate the complex challenges facing service members returning home from deployment. Each individual service member highlights unique challenges and unique health care needs. This study shows that etiologies for post-deployment challenges are multidimensional and are further complicated by unique individual needs.

Family members provide support to service members while they serve or when they have difficulties; family problems can interfere with the ability of service members to deploy or remain in theater; and family members are central influences on whether members continue to serve. In addition, rising family diversity and complexity will likely increase the difficulty of creating military policies, programs and practices that adequately support families in the performance of military duties. Strengthening the Military Family Readiness System for a Changing American Society examines the challenges and opportunities facing military families and what is known about effective strategies for supporting and protecting military children and families, as well as lessons to be learned from these experiences. This report offers recommendations regarding what is needed to strengthen the support system for military families.

Book excerpt: "The impact of service-related trauma on those returning from deployment remains a considerable issue among United States service members. While some veterans experience few to no setbacks upon homecoming, many service members struggle to reintegrate into their stateside lives, families, and communities following deployment. Little is yet understood about how veterans utilize social support to process trauma, the impact of trauma on service members' abilities to access the support available to them, and whether social support may help soldiers with trauma to reintegrate into civilian communities with greater success. The purpose of this study is to add to the body of literature concerning service members' experiences of trauma, social support, and postdeployment reintegration. Over the past five years, the Veterans Administration has observed a 35% increase in the number of service members receiving mental health care. Moreover, completed suicides among service members are at an all-time high, with more service member lives lost to suicide than to combat in recent years (Blumenthal, Maliha, & Mathews, 2012; Donnelly, 2011; Pilkington, 2013). Much is now known about the risk and resiliency factors that contribute to a service member's ability to metabolize service-related trauma and reintegrate stateside. Yet, despite the resources devoted to studying these influences and the increased utilization of mental health services, outcomes for many returning veterans continue to be bleak. As the drawdown of OIF/OEF/OND troops continues and the potential deployment of ground troops to combat ISIS/ISIL looms, the demand for a comprehensive understanding of the unique needs of those service members experiencing reintegration difficulties can be expected to increase. Since stigma surrounding mental health treatment remains high among military service members (Johnson & Faller, 2011; Zoroya, 2010), many veterans will remain untreated for months, or even years, before life becomes uncomfortable or unmanageable enough to press them into treatment (Hoge et al., 2004). Civilian and military counselors, social workers, clergymen, public health educators, psychologists, psychiatrists, and service members themselves need to be better informed about the manifestations of combat-related trauma, the influence of social support on trauma symptomology, and the potential role of social support in ameliorating trauma response and facilitating service member reintegration. Mental health workers with a deeper knowledge of how trauma and social support interact to inform postdeployment experiences will be better suited to facilitate stateside transitions in service members. Accordingly, a Web-based assessment was deployed to examine the relationships among the variables of service-related trauma, postdeployment social support, and reintegration in a sample of veterans of the conflicts in Iraq and Afghanistan. While some of these changes may be negative and lead to adverse outcomes for the members, they also may lead to potential benefits, such as increased self-confidence and self-tolerance, expanded political understanding, and improved military qualifications. To date, studies of the post-deployment reintegration experience have been limited to the Army. However, there is strong interest to examine the post-deployment reintegration process within the Air Force community. This study is an initial attempt to determine the aspects of post-deployment reintegration within this environment. The study used the Army reintegration experience as a starting point. Several similarities and differences between the two environments were identified and the basis of an Air Force post-deployment reintegration measurement tool was established.

More like civilian workers who deploy overseas to assist in disaster and post-conflict zones, Australian Army reservists are coalesced into temporary formations. They then leave the context of their families and civilian employment for the deployment and then return back to their civilian lives, often abruptly. Participants were Army reservists deployed to Timor L'Este for seven months (N=92) in 2002/03, and three consecutive groups (N=350) deployed to the Solomon Islands in 2006/07. Scars and Stripes Coffee (SSC) is a company on a unique mission to empower veterans to start their own e-commerce business. The author is driven by the belief that this company’s purpose is much bigger than selling a bag of coffee. The companies purpose stems from my belief that veterans have substantial reintegration needs which our society is not meeting. Our young veterans are returning with images and voices that never leave their heads. This is caused by a unique combat zone experiences where the enemy is at times unknown. Like veterans from other wars, they return from war, but the war never leaves them. They find themselves to be strangers in their own country even in a room full of familiar faces. Based on the authors literature review, the common theme surrounding the veteran reintegration dilemma is emotional or physical trauma. In addition, a lack of formal education, sense of purpose and understanding their new mission in life leads to anger and confusion. The veteran reports of having difficulty applying their military skills to a civilian job or business culture. Reported levels of Post-Traumatic Stress Disorder (PTSD) are increasing. I feel that is being caused by the lack of understanding from the veteran on how to navigate in an individualistic society versus a military community-based environment. As a veteran only business model, SSC is employing a novel approach to develop a successful company. The mission is to continue to build a for-profit company that will be sustainable through the sale of coffee. The military structure is what makes the company scalable because each member can continue to increase in rank. SSC will be built through a known military rank structure starting with the basic Team member to Squad Leader to Platoon Sergeant then on to 1ST Sergeant. With this structure, the company will be able to build communities of veterans with a common mission, purpose, team and accountability. Eventually, there will be an SSC team to join in every town in America. SSC will reignite the camaraderie that makes the U.S. Military so successful and adapt that type of culture into a civilian based company. In turn, veteran “transition coaches” will be developed throughout the members joining the company to training and advance other veterans through the rank structure. This will create a team environment so that the joining veteran has a group of other veterans to help them navigate the reintegration process. In addition, an empirically designed study will be discussed to test the efficacy of this model."--Abstract.

Book excerpt: Many combat veterans underestimate the on-going traumatic effects of war, effects that eventually surface in civilian life, causing health, relationship and career problems. During a deployment, emotions such as vigilance, anger, and fear are beneficial for the soldier and aide in coping with multiple combat-related adversities. Suppression of emotions that do not assist the soldier or mission during deployment is necessary and becomes habitual as it helps the soldier stay motivated and focused for the duration of the deployment. Post-deployment, the coping mechanisms previously necessary for survival, contribute to the difficulties of reintegration. The problems encountered by veterans can include, but are not limited to: social withdrawal, economic decline, self-medication, and most problematic, suicidal tendencies. As a veteran myself, I began to ask, is there another way to prepare veterans for re-entry to civilian life, to prevent unnecessary hardships and tragedies, educate them in unfamiliar ways, and perhaps contribute to an effective healing process? As a designer I approached these questions, searching for a way to communicate the adversities veterans face from an unexpected angle. Presented here are prototypes, diagrams, and warning systems designed to help veterans 1) be more self-aware and alert to the symptoms of posttraumatic stress and depression, 2) engage the armed forces and the VA in a discussion about innovative and more effective ways to talk about and treat the psychologically damaged soldier, and 3) foster communities to support veterans in their re-entry to civilian life. The objects I designed for my thesis exhibition are not intended to correct a complex problem such as Posttraumatic Stress Disorder (PTSD) or moral injury. Instead, they are created as a collection of tools to facilitate difficult conversations, provoke thought, and as an alternative approach to reach combat veterans who are in their own process of reintegration. My work is one method to process the effects of war through a non-destructive practice for those veterans who may not pay attention to the wall of pamphlets or other forms of disseminating information.

Book excerpt: This thesis uses the narrative paradigm to make sense of United States service members' reintegration experiences after a deployment to Iraq and/or Afghanistan after September 11, 2001. The narrative paradigm assumes that all humans are storytelling beings and that telling stories allows us to make sense of the world around us. The current study asked service members, who had deployed to Iraq and/or Afghanistan post 9/11, to share their stories of coming home and transitioning back to civilian life. An interpretive research design was used to answer the research questions, using qualitative method, specifically narrative analysis to cull out overarching themes from individual service member narratives. There were seven emerging narratives where the individual service member narratives intersected. The first two narratives answered the first research question to explain what coming home from war is really like, in the service members' own words. The next four narratives all shared common challenges that service members faced upon transitioning back to a civilian world. The last narrative explained how service members often use and rely on camaraderie to make their transition easier. The study presented a new and unique take on the service member experience that resulted in a new understanding of how service members make sense of combat and reintegration.

Book excerpt: Since the terrorist attacks of September 11, 2001, military personnel participating in combat operations in Afghanistan and Iraq have been plagued by traditional barriers to successful labor market attachment such as health and mental health concerns, employer stigma, and difficulty translating military training and experience to the civilian market, but also by a lagging economy. Veteran status since Vietnam has historically been linked to negative employment outcomes over the life course. Currently, the U.S. Bureau of Labor Statistics reports an unemployment rate of 9.5% for male Iraq and Afghanistan Veterans, and a 12.1% rate for their female counterparts. Veterans aged 20-24 have a 20.1% unemployment rate, nearly five points higher than that of their civilian peers. To compound the problem, an overly passive labor market policy prevents access to education and training that civilian employers value most. As Veterans continue to separate from the armed forces the United States, employers and policymakers can choose to capitalize on their skills, experience, and willingness to serve, or risk alienating another generation of young service members. This paper addresses five key categories that serve as barriers to successful labor market attachment and summarizes both governmental and private-sector programs designed to assist military personnel in their transition to civilian work. Finally, it provides policy options for remedying the post-9/11 Veterans labor market transition problem through improving service coordination and delivery, deliberately developing human capital through military service, and increasing employer responsibility for skill development and labor market attachment.
