Wenzel Geissler and the contributors examine how medicine and public health in Africa have been transformed as a result of economic and political liberalization and globalization, intertwined with epidemiological and technological changes. The resulting fragmented medical science landscape is shaped and sustained by transnational flows of expertise and resources. NGOs, universities, pharmaceutical companies and other nonstate actors now play a significant role in medical research and treatment. But as the contributors to this volume argue, these groups have not supplanted the primacy of the nation-state in Africa. Although not necessarily stable or responsive, national governments remain crucial in medical care, both as employers of health care professionals and as sources of regulation, access, and – albeit sometimes counterintuitively - trust for their people. “The state” has morphed into the “para-state” — not a monolithic and predictable source of sovereignty and governance, but a shifting, and at times ephemeral, figure. Tracing the emergence of the “global health” paradigm in Africa in the treatment of HIV, malaria, and leprosy, this book challenges familiar notions of African statehood as weak or illegitimate by elaborating complex new frameworks of governmentality that can be simultaneously functioning and dysfunctional. The Colonial Medical Service was the personnel section of the Colonial Service, employing the doctors who tended to the health of both the colonial staff and the local populations of the British Empire. Although the Service represented the pinnacle of an elite government agency, its reach in practice stretched far beyond the state, with the members of the African service collaborating, formally and informally, with a range of other non-governmental groups. This collection of essays on the Colonial Medical Service of Africa illustrates the diversity and active collaborations to be found in the untidy reality of government medical provision. The authors present important case studies covering former British colonial dependencies in Africa, including Kenya, Malawi, Nigeria, Tanzania, Uganda and Zanzibar. They reveal many new insights into the enactments of colonial policy and the ways in which colonial doctors negotiated the day-to-day reality during the height of imperial rule in Africa. Book excerpt: Infectious disease is the most common cause of illness and death in Africa, yet health practitioners routinely fail to identify causative microorganisms in most patients. As a result, patients often do not receive the right medicine in time to cure them promptly even when such medicine is available, outbreaks are larger and more devastating than they should be, and the impact of control interventions is difficult to measure. Wrong prescriptions and prolonged infections amount to needless costs for patients and for health systems. In Divining without Seeds, Iruka N. Okeke forcefully argues that laboratory diagnostics are essential to the effective practice of medicine in Africa. The diversity of endemic life-threatening infections and limited public health resources in tropical Africa make the need for basic laboratory diagnostic support even more acute than in other parts of the world. This book gathers compelling case studies of inadequate diagnoses of diseases ranging from fevers—including malaria—to respiratory infections and sexually transmitted diseases. The inherited and widely prevalent health clinic model, which excludes or diminishes the hospital laboratory, is flawed, to often devastating effect. Fortunately, there are new technologies that make it possible to inexpensively implement testing at the primary care level. Divining without Seeds makes clear that routine use of appropriate diagnostic support should be part of every drug delivery plan in Africa and that diagnostic development should be given high priority.

This is the first account of Britain’s plans for industrial development in its Caribbean colonies – something that historians have usually said Britain never contemplated. It shows that Britain’s remedy to the poor economic conditions in the Caribbean gave a key role to laboratory research to re-invent sugarcane as the raw material for making fuels, plastics and drugs. Science at the end of empire explores the practical and also political functions of scientific research and economic advisors for Britain at a moment in which Caribbean governments operated with increasing autonomy and the US was intent on expanding its influence in the region. Britain’s preferred path to industrial development was threatened by an alternative promoted through the Caribbean Commission. The provision of knowledge and expertise became key routes by which Britain and America competed to shape the future of the region, and their place in it.

Book excerpt: Between 1935 and 1944 the field of microbiology, and by implication medicine as a whole, underwent dramatic advancement. The discovery of the extraordinary antibacterial properties of sulphonamides, penicillin, and streptomycin triggered a frantic hunt for more antimicrobial drugs that was to yield an abundant harvest in a very short space of time. By the early 1960s more than 50 antibacterial agents were available to the prescribing physician and, largely by a process of chemical modification of existing compounds, that number has more than tripled today. We have become so used to the ready availability of these relatively safe and highly effective 'miracle drugs' that it is now hard to grasp how they transformed the treatment of infection. This book documents the progress made from the first tentative search for an elusive 'chemotherapy' of infection in the early days of the twentieth century, to the development of effective antiviral agents for the management of HIV as the millennium drew to a close. It also offers a celebration of the individuals and groups that made this miracle happen, as well as examining the inexorable rise of the global pharmaceutical industry, and, most intriguingly, the essential input of luck. Infection still maintains a high profile in both medicine and the media, with the current threats of 'superbugs' such as MRSA acquired in hospital, and a potential resistance to antibiotics. This book tracks the history of antimicrobial drugs, a remarkable medical triumph that has provided doctors with an amazing armoury of safe and effective drugs that ensure that reversion to the helpless state of the fight against infection witnessed in the early 1900s is extremely unlikely. This timely compendium acknowledges the agents that have surely led to the relief of more human and animal suffering than any other class of drugs in the history of medical endeavour.

Book excerpt: Total hip replacement effectively began in the UK in 1938 and has led to widely used, commercially successful, mass-produced devices that relieve pain for an ever increasing period. The Witness Seminar, chaired by Mr Alan Lettin, discussed the remarkable postwar collaboration of British surgeons, engineers and manufacturing firms in the development of efficient alloys, surgical procedures, instruments and the implementation of clean, bacteria-reduced air in enclosed operating theatres, as illustrated by successful prostheses and techniques developed in Norwich (Kenneth McKee), Wrightington (Sir John Charnley), Stanmore (John Scales), Redhill (Peter Ring), and Exeter (Robin Ling and Clive Lee). Early failures - such as loosening from infection, osteolysis, and wear debris - stimulated the search for improved materials and fixation methods, as well as the addition of antibiotics to bone cement to reduce infection. National hip registers that record the survival of different implants were adopted in Europe in the 1970s (2003 in the UK), and they pinpoint the successful devices, as measured by survival and low rates of revision. An introduction to the volume by Dr Francis Neary and Professor John Pickstone, and appendices on materials by Professor Alan Swanson; on international standards by Mr Victor Wheble; and of details of selected prosthesis supplement the transcript.
