They differ in price, in place of origin, in color, shape, and size, in the dyes, binders, fillers, and coatings used, and in a host of other ways. Claims of generic equivalence, as physician-historian Jeremy Greene reveals in this gripping narrative, are never based on being identical to the original drug in all respects, but in being the same in all ways that matter. How do we know what parts of a pill really matter? Decisions about which differences are significant and which are trivial in the world of therapeutics are not resolved by simple chemical or biological assays alone. While these two types of medications may have differences in their packaging appearance, they are required to be equivalent in their pharmacological effect. However, some patients and healthcare professionals perceive branded medicines to be of better quality, safer and more effective than generics. These beliefs can impact the therapeutic effect of a medication, resulting in generics performing poorly compared to their branded counterparts; a situation referred to as the placebo and nocebo effects. This study investigated whether enhancing the appearance of a generic medicine could improve its efficacy and reduce side effects. Eighty-seven participants were randomised to receive either a branded beta-blocker, a plainly packaged generic or an enhanced generic ostensibly to treat preexam anxiety. In reality all medications were placebos. Changes in blood pressure, heart rate and anxiety were measured, along with the side effects attributed to each medication. While a general placebo and nocebo effect was found, all three medications performed similarly. There were no differences in the effect of the three medications on systolic and diastolic blood pressure (p = .393, p = .130, respectively), mean arterial pressure (p = .143), heart rate (p = .326), anxiety (p = .848), and the number of side effects reported post-exam (p = .729) or 24 hours later (p = .057). From these results, one might conclude that enhancing the appearance of a generic medicine does not affect its therapeutic outcomes. However, the fact that the branded and plain generic medicines also did not differ in effect challenges this interpretation as this contradicts previous placebo research showing that branded medicines consistently perform better than generics. There are several theories that could account for these findings. The results may be due to participants' lack of familiarity with the medicines used in this study. Aspects of the sample or issues with the experimental design of the study may have also contributed to the results. From this study alone, it cannot yet be determined whether enhancing the appearance of a generic medicine improves its therapeutic outcomes. Nevertheless, the findings of this study demonstrate the placebo and nocebo effects, add to the literature on medicine branding, and offer directions for future research.
