Book excerpt: When children and adults apply for disability benefits and claim that a visual impairment has limited their ability to function, the U.S. Social Security Administration (SSA) is required to determine their eligibility. To ensure that these determinations are made fairly and consistently, SSA has developed criteria for eligibility and a process for assessing each claimant against the criteria. Visual Impairments: Determining Eligibility for Social Security Benefits examines SSA's methods of determining disability for people with visual impairments, recommends changes that could be made now to improve the process and the outcomes, and identifies research needed to develop improved methods for the future. The report assesses tests of visual function, including visual acuity and visual fields whether visual impairments could be measured directly through visual task performance or other means of assessing disability. These other means include job analysis databases, which include information on the importance of vision to job tasks or skills, and measures of health-related quality of life, which take a person-centered approach to assessing visual function testing of infants and children, which differs in important ways from standard adult tests.

Book excerpt: The ability to see deeply affects how human beings perceive and interpret the world around them. For most people, eyesight is part of everyday communication, social activities, educational and professional pursuits, the care of others, and the maintenance of personal health, independence, and mobility. Functioning eyes and vision system can reduce an adult's risk of chronic health conditions, death, falls and injuries, social isolation, depression, and other psychological problems. In children, properly maintained eye and vision health contributes to a child's social development, academic achievement, and better health across the lifespan. The public generally recognizes its reliance on sight and fears its loss, but emphasis on eye and vision health, in general, has not been integrated into daily life to the same extent as other health promotion activities, such as teeth brushing; hand washing; physical and mental exercise; and various injury prevention behaviors. A larger population health approach is needed to engage a wide range of stakeholders in coordinated efforts that can sustain the scope of behavior change. The shaping of socioeconomic environments can eventually lead to new social norms that promote eye and vision health. Making Eye Health a Population Health Imperative: Vision for Tomorrow proposes a new population-centered framework to guide action and coordination among various, and sometimes competing, stakeholders in pursuit of improved eye and vision health and health equity in the United States. Building on the momentum of previous public health efforts, this report also introduces a model for action that highlights different levels of prevention activities across a range of stakeholders and provides specific examples of how population health strategies can be translated into cohesive areas for action at federal, state, and local levels.

Book excerpt: This study examined the practical, personal, social, economic and technical experiences of visually handicapped students and workers in Ontario schools and workplaces. The examination was based on the data generated from a series of semi-structured interviews with eleven visually impaired participants who have widely different educational and work experiences. The data from the interviews served as a basis for evaluating the appropriateness of current needs assessment tools and practices in Ontario and other jurisdictions. In order to reflect the above mentioned experiences accurately, a qualitative research paradigm, using a series of case studies, was chosen as the most appropriate strategy for investigation. A somewhat unusual feature of this approach was the opportunity it provided to explore in depth as well as in breadth the lives of the participants. In this case, the persons with visual impairments who provided the information to the study were treated as research partners. They had direct, timely, and meaningful input into the many and varied activities of this study in order to establish mutual trust, thereby enhancing the trustworthiness of the data. The results of this study indicated that there are clearly significant barriers for people with visual impairment in their educational, social and work lives. The kinds of inadequacies cited by the participants included problems with the availability of information in an accessible format and the psychological, technical, and academic preparedness of educators, employers and support staff to create a level playing field for students and workers with visual disabilities and their sighted counterparts. A somewhat surprising finding was that participants felt that both mainstream institutions and those institutions designed to provide education exclusively to persons with visual impairments had similar instructional and employment barriers. In the workplace, they were particularly concerned about the difficulties they had in work discussions because they were not given in accessible format the background material that sighted workers had. In classrooms, they were concerned about teachers who did not provide educational materials in accessible format in a timely manner. In addition, they had limited opportunities for interacting with the community at large both on the intellectual and social levels.

Often, many health and welfare workers are involved in this process. Occupational therapists however have a particular role to play. Their training is such that they are able to assess and treat the physical, psychological and social aspects of a disabling condition. This enables them to help disabled people to achieve their maximum ability in the environment in which they live. Whether the occupational therapist is hospital or community based, employed by the health or social service depart ment, her caseload is often vast. The occupational therapist may specialize in a particular age group or disability, but it is more likely that she will have to work with a wide age range from the very young to the very old. Assessment of needs and the subsequent provision of equipment and adaptations is the greater part of the OT's job. Other aspects include counselling the disabled person and his family and planning future intervention must often, however reluctantly, be given a low priority. In spite of staff shotages occupa tional therapists are frequently called on to provide the client and other groups of workers with advice and information on all aspects of living with any disability.
