Book excerpt: Andrology, a counterpart to gynecology, deals with the study of the male reproductive organs. Clinical andrology has been neglected primarily because of the lack of relevant, accurate laboratory methods for functional analysis, but in the last decade substantial progress has been made in the understanding of male reproductive biology. This progress has resulted from modern tech niques and instrumentation in microanatomy, immunology, neurophysiology, pathology, genetics, endocrinology, biochemistry, biophysics, urology and surgery. These studies are scattered in such a wide spectrum of journals that andrologists can hardly keep abreast of the advances. There have been numerous textbooks on the testes, male accessory organs and semen but the clinical aspects of andrology have not received similar emphasis. Since literature concerning clinical andrology is extensive and widely scattered in many different publica tions, we hope that a useful purpose will be served by summarizing the more pertinent material in a series of volumes which can be made readily available to students of andrology. It was decided that a series of specialized monographs should be devoted to clinics in andrology. These ten volumes of the series are an attempt to coordinate anatomical, physiological, biochemical, endocrinological, pharmacological and immunological aspects of the spermatozoa, testes, epi didymis and other accessory genital organs. Little is known about the effect of diet, diseases, environmental factors and drugs on male reproduction. The interest in developing new male contraceptive methods will stimulate research in andrology.

The correct number of human chromosomes was not deter mined until the mid-1950s, and Down syndrome was recognized only in 1959. It was not until in 1968 that the exact location of a gene was determined on an autosomal chromo some, and the study of genes, rather than their protein products, has been possible for barely a decade.

The laws permitting sterilization of the mentally re tarded~ and those proscribing consanguineous marriages are but two examples. Indeed as far back as the 5th-10th centuries, B.C.E., consanguineous unions were outlawed (Leviticus XVIII, 6). Case law has traditionally tended toward the conservative. It is reactive rather than directive, exerting its influence only after an individual or group has sustained injury and brought suit. In contrast, state legislatures have not been inhibited in enacting statutes. Many of their products can be characterized as hasty, unnecessary, ill-conceived, and based on the heart rather than the head. Moreover the lack of expert consultation sought has also been remarkable. One state legislature, for example, has advocated immunization for sickle cell anemia! Many others have enacted laws for the screening of inborn errors of metabolism, e.g., phenylketon uria, but have poorly defined the lines of responsibility to secure compliance. A spate of specific disease-related bills has emerged in the u.S. Congress, each seeking recognition and appropriations. Sickle cell anemia, hemophilia, Cooley's anemia and Tay-Sachs disease have been among the front-runners for support. Finally, in 1975, Congress has begun to examine an omnibus bill concerning all forms of genetic disease. The bill, termed the National Genetic Diseases Act is, however, still far from being enacted.

Human embryos whose genomes have been edited should not be used to create a pregnancy until it is established that precise genomic changes can be made reliably and without introducing undesired changes - criteria that have not yet been met, says Heritable Human Genome Editing. From an international commission of the U.S. National Academy of Medicine, U.S. National Academy of Sciences, and the U.K.'s Royal Society, the report considers potential benefits, harms, and uncertainties associated with genome editing technologies and defines a translational pathway from rigorous preclinical research to initial clinical uses, should a country decide to permit such uses. The report specifies stringent preclinical and clinical requirements for establishing safety and efficacy, and for undertaking long-term monitoring of outcomes. It should not come as a shock that scientists and physicians often prefer the mirror at times when society seems to be demanding a mandate. This may be especially true in the rapidly advancing field of medical genetics, where recent discoveries leading to potentially startling applications have raised old questions of law in a new light. Nevertheless, we believe that in general the conflict between the law and science, as illustrated in the field of genetics, is embroi dered with exaggeration. The Chief Justice of the United States Supreme Court, Warren Burger, has noted that "the prime function of the law is to protect basic human values--individual human values--sometimes even at the expense of scientific progress"; and that "it is not the function of the law to keep pace with science." While both of these statements are true as far as they go, we believe the law must make an affirmative effort to anticipate scientific developments so that those beneficial to society can be nurtured rather than stultified. It was to nurture cooperation and understanding that we brought together a distinguished faculty of internationally known experts on law and genetics to discuss their fields in 1975.

Whatever may happen thereafter, the child’s parents are legally bound to provide for their child, and if they don’t, they’re held accountable by law. But what about children created by artificial insemination? When it comes to paternity, the law is full of gray areas, resulting in many cases where children have no legal fathers. In Papa’s Baby, Browne C. Lewis argues that the courts should take steps to insure that all children have at least two legal parents. Additionally, state legislatures should recognize that more than one class of fathers may exist and allocate paternal responsibility based, again, upon the best interest of the child. Lewis supplements her argument with concrete methods for dealing with different types of cases, including anonymous and non-anonymous sperm donors, married and unmarried women, and lesbian couples. In so doing, she first establishes different types of paternity, and then draws on these to create an expanded definition of paternity.
