Book excerpt: This volume contains the proceedings of the Fourth International Symposium on Coronary Heart Disease, held in Frankfurt/Federal Republic of Germany. In 1970, when the fIrst symposium was held, the whole range of diagnostic and therapeutic measures was covered, but since then interest in coronary heart disease has increased so considerably that the fourth symposium was restricted to only two therapeutic aspects, angioplasty and intracoronary thrombolysis. It can be noted with pleasure that European cardiologists, namely Andreas Griintzig and Peter Rentrop, have made essential contributions to the applicability of these modes of treatment. When angioplasty was fIrst discussed at the Third International Symposium (Coronary Heart Disease, Stuttgart, 1978) a great deal of astonishment was expressed. Today the procedure has gained an important place in the treatment of angina pecto ris. The proportion of candidates for coronary surgery in whom angioplasty can be attempted instead of operation is around 10%, in certain subgroups even higher. The most remarkable fmding of our 5-year experience with angioplasty are the extre mely good long-term results. Recurrences happen in about 15%-20% of cases within the fIrst 3 months; after this period of time, recurrences are very unusual and mostly due to progression of the disease at other sites. Angioplasty has thus also contributed to our understanding of the dynamics of the atherosclerotic process.

Book excerpt: Percutaneous Transluminal Coronary Angioplasty is presently the widest used non-surgi cal method for the treatment of stenotic lesions in coronary artery disease. Continuous development of the procedure and equipment has made complex and multi-vessel-inter ventions possible and has led to other techniques of intravascular angioplasty. In reviewing current results, this volume pays particular attention to causes, incidence, circumstances, recognition, treatment and outcome of complications of PTCA; it also considers restenosis, which still occurs at an unsatisfactory high rate after primarily successful procedures. To deal with these topics, experts on PTCA met to discuss their experiences and insights into peculiar aspects of complications in PTCA. The discussion was subdivided into: Recognition of complications; Complications in new interventional techniques; Management of complications; and Restenosis after angioplasty. The question of whether angiographic and pathological characteristics of sclerotic lesions allow the identification of complication-prone lesions in advance is discussed in the first chapter. One paper deals with the state of the art of digital cardiac imaging, which enables the operator to have an early knowledge of and adequate reaction to possible complications occurring during angioplasty. Special attention is directed to the so-called "high-risk"-PTCA, detailing the clinical variables, in order to have better knowledge about patients with a higher probability of unsuccessful or complicated procedures.

Book excerpt: Since coronary angioplasty was first practiced by Andreas Gruntzig in 1977, it has rapidly developed into a technique widely used on patients with chronic and acute coronary heart disease. The meeting described in this book was held under the auspices of the working group of our national society, chaired by Prof. Kaltenbach, Frankfurt, and by Prof. Meyer, Mainz, in cooperation with Stanford University. It is an attempt to present various cardiologists' appraisals of the current position ofPTCA in clinical medicine. PTCA is far from easy, as its mechanism is critically balanced between success and fail ure. Therefore the experience of more than one hundred cases is generally regarded as ne cessary for cutting down complications and achieving a high rate of sli'ccess. Furthermore, success and complications are a result not only of technical expertise, but also of patient selection. However, the transition from the diagnostic to the therapeutic intervention is not always easily accomplished. Certain prerequisites concerning technique, the physician, technical equipment, and program organization are necessary for safe and effective performance. The consequences of unsuccessful coronary angioplasty range from waste of time and money to severe complications for the patient; the decision to perform PTCA needs to be based on sound indications and techniques according to the best current scientific evidence. Techniques and technology for coronary angioplasty continue to evolve. Scientific evidence of its short-and long-term value continues to accumulate, and applications of the procedure are still being extended. This volume is obviously no substitute for well-guided hands-on experience with the procedure; it is intended rather to present current policies of patient selection and patient management; to describe standard techniques (femoral and brachial approaches) and potential applications; to point out problem areas and limitations of the proce dure; and to provide tips that may be useful even for the more experienced angioplasty operator.

While long-term morbidity can only be improved by changing the patient's life-style and removing any risk factors that may be present(hypertension, hyperlipidemia,obesity,diabetesmellitus, nicotine abuse), symptomatic relief may be obtained by various procedures. Besides conservative treatment of arterial occlusive disease (physical therapy, medical treatment) and surgical vascular interventions (thromboendarterectomy, bypass procedures), balloon catheters introduced percutaneously under local anesthesia have found increased application in virtually all areas for the dilatation and recanalization of obstructive lesions in the past few years. The technique of balloon dilatation, as introduced by Griintzig as a further development of the percutaneous interventions by Dotter and Judkins, is no longer the decisive issue. The main objective of present clinical research is to de termine the proper indications and patient selection for this procedure, which has found its place between conservative treatment and surgery. The contents of this Symposium should provide some guidelines for the indi cations and postprocedural therapy for the referring physician, the angiologist, and for the radiologist and cardiologist performing the dilatation. We owe our thanks to all the specialists involved, who have provided us with the benefits of their experience. Mrs. H. Beilmann and Dr. M.
