Book excerpt: Although prostaglandin El (PGE ) has been clinically available for a long 1 time, only in recent years has its effectiveness in peripheral arterial occlusive disease been confirmed in controlled studies. Not surprisingly, the favour able results achieved both in patients with critical limb ischaemia and in those with intermittent claudication has stimulated research activities into the clinical pharmacology of this prostaglandin. As a consequence of these efforts, exciting new findings have revealed that PGE has anti thrombotic , endothelium-stabilizing and leucocyte-stabi 1 lizing properties as well as effects on lipid metabolism, all of which, quite apart from its well-known anti-aggregating and vasodilator effects, may add to the clinical efficacy of the substance. New data have also been gathered on the metabolism of PGE most b notably the detection of 13,14-dihydro-PGE a metabolite which was b recently isolated in humans following the administration of PGE . Being 1 biologically active, the pharmacodynamic spectrum of 13,14-dihydro-PGE 1 very closely resembles that of PGE . This finding may help to explain the 1 efficacy of PGE despite its rapid metabolization when given intravenously.

While long-term morbidity can only be improved by changing the patient's life-style and removing any risk factors that may be present(hypertension, hyperlipidemia,obesity,diabetesmellitus, nicotine abuse), symptomatic relief may be obtained by various procedures. Besides conservative treatment of arterial occlusive disease (physical therapy, medical treatment) and surgical vascular interventions (thromboendarterectomy, bypass procedures), balloon catheters introduced percutaneously under local anesthesia have found increased application in virtually all areas for the dilatation and recanalization of obstructive lesions in the past few years. The technique of balloon dilatation, as introduced by Griintzig as a further development of the percutaneous interventions by Dotter and Judkins, is no longer the decisive issue. The main objective of present clinical research is to de termine the proper indications and patient selection for this procedure, which has found its place between conservative treatment and surgery. The contents of this Symposium should provide some guidelines for the indi cations and postprocedural therapy for the referring physician, the angiologist, and for the radiologist and cardiologist performing the dilatation. The proceedings of this symposium - organized by the department of general surgery of the University of Limburg - cover the field thoroughly with contributions from outstanding specialists from all over the world. Rheumatological, neuro surgical, orthopedic, and traumatological aspects are covered. Vascular surgeons considering both venous and arterial problems entwine with thoracic surgeons to review the thoracic outlet problem. Establishing a diagnosis in this multi-disciplinary field is extremely important and encom passes a combination of disciplines including neurology, orthopedics, sur gery, and physical medicine. Likewise, the role of each contributing factor must be assessed when multiple sources were responsible for pain. In addition to adequate diagnosis, it is important to institute the appropriate therapy and to establish the proper priority and timing of such therapies. The follow-up techniques particularly regarding thoracic outlet syndrome are extremely important to provide an objective basis for evaluation. Although many advances have been made in the field of objective diagnosis and therapy, there still is a great deal of overlap and confusion which has to be clarified. Professor Greep and his organizing committee have done a wonderful job in composing a program covering all aspects of this field. They are to be congratulated on bringing us a "timely current status" report on pain in the shoulder and arm.
