Book excerpt: "Multiple Sclerosis (MS) is a chronic degenerative autoimmune disease of the central nervous system that affects approximately 2.1 million people world-wide (National Multiple Sclerosis Society, 2005). Symptoms include limitations with both physical (coordination, muscle weakness, vision problems, etc.) and cognitive functioning and vary by individual. There is currently no cure for MS and treatment is based around symptom management (Coyle & Hamaad, 2006). One means for symptom management is physical activity (PA). PA has been shown to effectively aid symptom management by reducing the number, length, and duration of disease flare-ups (Motl, McAuley, & Snook, 2005), as well as increasing overall quality of life (Stuifbergen, Blozis, Harrison, & Becker, 2006). In spite of this evidence, the MS population is one of the most inactive segments of the population, even among patients with chronic diseases (Motl & Snook, 2008). Understanding what motivates this population to be physically active is the first step in developing an effective, sustainable, PA intervention for disease management. Using Path Analysis, this study examined potential predictors of motivation for PA in individuals with MS (n = 215) finding self-determined motivation, in conjunction with self-efficacy, as predictors of PA participation, and self-efficacy and PA participation as a predictors of quality of life. In the model, self-efficacy and identified regulation predicted PA participation, and PA participation predicted quality of life,?2(1) = .02, p = .867; RMSEA = .00; CFI = 1.0; SRMR = .002. Open-ended responses from participants supported the model, indicating that self-efficacy and identified regulation were predictors of PA, and PA was a predictor of increased overall quality of life. The findings and resulting model may be used to guide future interventions to promote PA participation in individuals with MS and consequently enhance long-term quality of life."--Abstract from author supplied metadata.

Book excerpt: Objective: Unemployment is common among individuals with Multiple Sclerosis (MS) and is associated with significant socioeconomic burden. Several MS-related factors have been found to be associated with unemployment status including fatigue, depression, cognitive problems, and motor difficulties. However, few studies have examined these factors collectively in predicting employment. The present study aimed to explore these variables together in predicting employment status in MS. Patients and Methods: Fifty-three individuals with MS participating in a research study of cognitive, emotional, and social factors related to MS were examined. Construct scores were created using factor analysis that represented cognition, fatigue, depression, and motor function. These construct scores, along with age and measures of disease burden, were explored as predictors of unemployment status (not working, working) via logistic regression. Models of mediation were also investigated. Results: A model including construct scores of motor function, cognition, depression, and fatigue significantly distinguished those who are unemployed from employed. However, only the cognitive, motor, and fatigue construct scores were found to be significantly associated with unemployment individually. Results of a mediation analysis indicated that the cognitive and fatigue construct scores significantly mediated the relationship of disability (EDSS) on work status. More specifically, processing speed and memory mediated the relationship of overall disability on work status. Conclusion: Cognitive function and fatigue mediate the effect of MS disability on employment status. Interventions targeting cognitive difficulties and fatigue in MS may be effective in helping individuals to maintain employment.

Book excerpt: Many individuals report using religion and spirituality as a means of coping with multiple sclerosis (MS). However, there has been little research to investigate how various aspects of religious and spiritual belief systems relate to health outcomes in this clinical population. The present study sought to identify how specific facets of religion and spirituality relate to physical and mental health outcomes in a sample of adults with MS. Furthermore, the effect of disability status (high or low disability) on the relationship between religion/spirituality and health outcomes was also explored.The sample included 80 adults with multiple sclerosis. Participants subjectively reported on their religious/spiritual beliefs and psychosocial resources as well as their current physical and mental health. Objective health outcomes were obtained using a clinician-rated measure of disability status and a measure of ambulation capacity. Most of the participants were female (74%), Christian (73%), and had relapsing-remitting MS (76%).The results indicate that existential well-being (a sense of meaning and purpose in life) was positively associated with mental health outcomes after accounting for demographic and psychosocial characteristics. Among individuals with high disability due to MS, negative religious coping (having punitive attitudes about a divine being0́9s role in one0́9s life situation) was inversely related to mental health, and positive religious coping was positively related to objective physical health. Religious well-being (a sense of connectedness to a divine being) was not substantially related to any physical or mental health outcomes. Among individuals with low disability, religious involvement (commitment to and participation in religious practices) was inversely associated with physical health. Individuals with low disability reported experiencing more severe anxiety than individuals with high disability.The findings indicate that specific facets of religious and spiritual belief systems are substantially related to health outcomes among individuals with MS. Furthermore, the findings suggest that the relationship between facets of religion, spirituality, and health outcomes may differ based on the severity of MS disability. The findings highlight the important influence that religion and spirituality have on health outcomes and coping with MS.
