Unit medical trainers and medical directors should evaluate Critical Care Flight Paramedics (CCFP) ability to follow and execute the medical instructions herein. These medical guidelines are intended to guide CCFPs and prehospital professionals in the response and management of emergencies and the care and treatment of patients in both garrison and combat theater environments. Unit medical providers are not expected to employ these guidelines blindly. Unit medical providers are expected to manipulate and adjust these guidelines to their unit’s mission and medical air crew training / experience. Medical directors or designated supervising physicians should endorse these guidelines as a baseline, appropriately adjust components as needed, and responsibly manage individual unit medical missions within the scope of practice of their Critical Care Flight Paramedics, Enroute Critical Care Nurses, and advanced practice aeromedical providers. The medication section of this manual is provided for information purposes only. CCFPs may administer medications only as listed in the guidelines unless their medical director and/or supervising physician orders a deviation. Other medications may be added, so long as the unit supervising physician and/or medical director approves them. This manual also serves as a reference for physicians providing medical direction and clinical oversight to the CCFP. Treatment direction, which is more appropriate to the patient’s condition than the guideline, should be provided by the physician as long as the CCFP scope of practice is not exceeded. Any medical guideline that is out of date or has been found to cause further harm will be updated or deleted immediately. The Medical Evacuation Concepts and Capabilities Division (MECCD) serves as the managing editor of the SMOG and are responsible for content updates, managing the formal review process, and identifying review committee members for the annual review. The Standard Medical Operating Guidelines are intended to provide medical procedural guidance and is in compliment to other Department of Defense and Department of the Army policies, regulatory and doctrinal guidance. Nothing herein overrides or supersedes laws, rules, regulation or policies of the United States, DoD or DA.

Book excerpt: This research examines the Air Force Aeromedical Evacuation, Patient Movement Items (PMI) Program. This thesis analyzes the primary question of cost savings or equipment deferment based on projected casualty rates. It uses a simple linear program, focused on minimizing beginning inventory, and maps an optimal order plan based on manufacturer capacity and lead time. This thesis suggest updating demand requirements for variability from projected demand using an exponentially weighted moving average calculation. This thesis illustrates that initial deferment can generate substantial savings. This thesis recommends increasing readiness capabilities and cost avoidance by implementing the deferment plan. This thesis recommends additional areas of further research to include consolidating patient movement and inventory tracking systems and utilizing advanced simulation software to determine medical requirements in theaters of operation. These initiatives, if analyzed more thoroughly, could provide DoD policy makers clearer insight for potential system-wide savings.

Book excerpt: The Department of Defense, USAF, and Air Mobility Command (AMC) rely on the Civil Reserve Air Fleet (CRAF) to provide supplemental airlift in times of national emergency. AMC oversees the CRAF program which contains three segments: international, national, and aeromedical evacuation. The CRAF aeromedical Evacuation (AE) program involves commercial B-767 aircraft, Aeromedical Evacuation Shipsets (AESS), a Casualty Transfer System, and a large number of support personnel and equipment. Wartime planning in the early 198Os revealed a significant shortage of military airlift to accomplish the aeromedical evacuation mission. Consequently the CRAF AE program was initiated. Planning for Major Regional Conflict scenarios has currently set a requirement for 25 and 44 B-767 aircraft for CRAF Stages II and III, respectively. Unfortunately AMC has not been able to acquire full commitment to the CRAF AE program. In addition, the CRAF AE program has been beleaguered by a number of problems which have continually kept capability of the overall system low. This graduate research paper provides a comprehensive review of the AE shortfall issue from beginning to the present with recommendations and conclusions for the near future.

Book excerpt: This report devises a new metric for measuring expeditionary medical support (EMEDS) and develops a framework for applying it across three Air Force medical mission areas: deployed support to the warfighter, humanitarian relief, and defense support to civil authorities. The operational emphasis of EMEDS is on patient flow. An injured patient receives limited treatment locally and is then moved from the point of injury to an EMEDS facility as quickly as possible. There, the patient is further evaluated, stabilized, triaged, treated, and evacuated to a higher level of care. Each level of care is designed to be sufficient for immediate needs, not to provide definitive care. This emphasis on flow streamlines capabilities that need to be deployed and places the definitive care in the most capable facilities. The current measure of EMEDS capabilities, the number of available beds, is inadequate because the components of the expeditionary medical system are not intended to hold patients. The new metric developed in this study focuses on the rate at which each component of the deployment system can evaluate, stabilize, triage and treat, and evacuate patients, or the medical STEP rate. This measure more closely captures the requirement at the time of need and holds the promise of providing a more agile, responsive, and effective medical deployment capability.
