Expanding the coverage and reimbursement of medical insurance has become a common way to reduce the burden. This book will elaborate on how medical insurance may increase the burden instead.It explains why the existing medical insurance system results in increased medical costs, where higher costs may offset the benefits of certainty brought by medical insurance, forming the 'paradox of medical care insurance'. This assumption is verified by empirical evidence in China, through a new method developed to find out the actual medical costs, using two parameters: ratio of self-payment of medical insurance and the level of monopoly in the supply of medical services. The book also describes the history, the current situation, and the reform of the health care system in China.

Book excerpt: With its transition to a market-oriented economy, China has gone through significant changes in health care delivery and financing systems in the last three decades. Since 1998, a new public health insurance program for urban employees, called Basic Medical Insurance Program (BMI), has been established. One theme of this reform was to control medical service over-consumption with new cost containment methods. This dissertation attempts to evaluate the effects of the reformed public health insurance on health care utilization, with in-depth studies. In the analytical investigation, I formulate a health care demand model based on the structure of health care delivery and health insurance systems in China. It is assumed in the model that physicians have pure monopoly power in determining patients' health care utilization. The major inference is that the insurance co-payment mechanism can not reduce medical service over-utilization effectively without any efforts to control physicians' behavior. Meanwhile, I use the calibrated simulation to demonstrate my hypothesis in the theoretical model. The main implication is that physicians' incentive to over utilize medical services for their own benefits is significant and severe in China. In the empirical studies, I explore the extent to which the public health insurance status affects individual's medical service utilization and medical expenditure over the period when the new urban public health insurance has been in effect. This study uses the 1997 and 2004 waves of China Health and Nutrition Surveys (CHNS) data with relevant econometric models such as the two-part model and difference-in-difference model. The empirical results provide evidences that (1) public health insurance in China has increasing effect on individual's medical service utilization and medical expenditure, i.e., there exists patients' ex post moral hazard; (2) China's public health insurance reform reduced the increasing effect of public insurance on the probability of utilizing medical services, i.e., reduced patients' ex post moral hazard; (3) China's public health insurance reform enlarged the increasing effect of public insurance on medical service expenditure, implying there exists physicians' severe incentive to over utilize medical services for their own benefits in China, which is consistent with the conclusions in my theoretical analysis.

China’s once praised public health services have deteriorated into a system driven by economic constraints, in which poor people often fail to get access, and middle-income households risk to be dragged into poverty by the rising costs of care. The New Rural Co-operative Medical System (NRCMS) was introduced to counter these tendencies and constitutes the main system of public health insurance in China today. This book outlines the nature of the system, traces the processes of its enactment and implementation, and discusses its strengths and weaknesses. It argues that the contested nature of the fields of health policy and social security has long been overlooked, and reinterprets the NRCMS as a compromise between opposing political interests. Furthermore, it argues that structural institutional misfits facilitate fiscal imbalances and a culture of non-compliance in local health policy, which distort the outcomes of the implementation and limit the effectiveness of insurance. These dynamics also raise fundamental questions regarding the effectiveness of other areas of the comprehensive New Health Reform, which China has initiated to overhaul its healthcare system.

On the other hand through the higher quality of the healthcare system the access is very difficult due rising costs. In that research it will be schematize the misbehaviour of many farmers concerning to health and disease and it will be shown that the present health care system that is working in the shadow of the centralized state which is managed by the government, is not the efficient way concerning to the rising costs. The challenges come with the economic reform and the health care expenditure keeps rising. Because of the demographic changes, the increase of the elderly population in particular, the demand of health care has greatly increased as well. In addition, the decentralization and privatization of the health care system have led to inequity of the accessibility to health care among different groups of people and different regions, and also to the competition among different health care providers. This study aims to explore the current problems in Chinese health care system through ia case study in Shanghai. The case study constitutes of document analysis, review of research literature, and in-depth interviews with medical insurance bureau officials and medical practitioners. It aims at scrutinizing how the health care policy in China is designed and implemented, and how the government departments liaison with concerned organizations in the implementation process. After identifying the strengths and weaknesses of current Chinese health care system, this study also reviews the health care reforms in Australia and Canada. For similar problems had been encountered and resolved in Australia and Canada before and, as two developed countries, their experiences can suggest some lessons that China can learn from . The research findings indicate that the current Chinese health care system has three major problems. Firstly, the government has excessively decentralized the responsibility of health care but invested too little in the system. Secondly, the public sectors have taken up the lion's share of health care provision in urban area and there is little competition in the Chinese health care market. Thirdly, there exists a great gap between the rich and poor in terms of the accessibility to health care. Finally, based on the analysis of different health care systems investigated above, this study concludes by making recommendations for the future reform of Chinese health care system. We construct a theoretical model to predict the exhaustive effects of the reform, and empirically examine the predictions using a large sample of administrative data from a city in Western China, where the insurance payment for inpatient care changed from fee-for-service to a prospective payment model. We find that reimbursable expenditures decreased after the reform, while non-reimbursable expenditures tended to increase. The expenditure reduction could be fully attributed to reduced drug costs. No reduction in non-drug services, including examinations, treatments, and nursing care, was observed. The reduction in drug expenditures was driven by reduced prescriptions for less cost-effective drugs rather than switching to cheaper drugs. Hospital admissions increased, but hospital revenue from inpatient care remained unchanged. The reform also had a spillover effect, increasing the number of outpatient visits.
