Book excerpt: The need for esthetic restorative materials that exhibit longevity and offer ease of placement for the restoration of posterior primary teeth is becoming markedly more important. The development of the "self-etch" dentin bonding agent provides the dental practitioner with a product that is designed to eliminate the rinse and dry step which is one of the most difficult steps in placement for pediatric patients. The purpose of this study was to (1) assess the clinical performance of Class II composite restorations placed in primary molars and to determine which if any pre-treatment regimen ("self-etch" or "total-etch") provided a superior adhesive bond, (2) correlate clinical assessments with chemical and morphologic analyses of the Class II composite restorations, and determine whether the "self-etch" or "total-etch" adhesive provided enhanced structural integrity of the dentin/adhesive/composite bond, and (3) determine whether differences in the morphologic and chemical characteristics of caries-affected and caries-free dentin affects the integrity of the dentin/adhesive bond. The clinical component of the study determined that composite resin restorations placed in children remained intact and clinically acceptable for the duration of the study, with some restorations serving for more than 36 months. There were no differences in the clinical performance of the "self-etch" dentin bonding agent compared to the "total-etch" dentin bonding agent. Although the dentin bonding agents and the resulting restorations were clinically acceptable and remained intact for the duration of the study, the morphologic and chemical analyses of the dentin/bonding interfaces created by both types of dentin bonding agents revealed significant weaknesses. In summary the results of this investigation suggest that (1) there are were no differences in the clinical performance of restorations placed with the "self-etch" bonding agents as compared with "total-etch" bonding agents, (2) the "total-etch" bonding agent penetrated conditioned dentin better than the "self-etch" bonding agent, (3) neither bonding agent produced an ideal dentin/adhesive interface impervious to breakdown and (4) there is a difference in the morphologic and chemical characterization of caries-free and caries-affected dentin that impacts the integrity of the dentin/adhesive interface.
