Many studies have been con ducted with the aim of discovering the general mechanisms which produce and maintain the ECM in tissues. Wound re pair in animals has always been a suitable "tool" for exploring these mechanisms. In such a pathologic event, the different components of the healing system can be readily observed since they are exaggerated. It has now become clear that the regulation of protease/inhibitor activity is one of the most im portant steps in maintaining the integrity of ECM in the normal as well as in the pathologic state. As far as the repair process is concerned, a failure in such regulation may result in abnormal wound healing ranging from nonhealing wounds to hyper trophic scarring. Experimental findings have been directly applied in clinical practice. For example, the importance of debridement in nor mal and especially in poorly healing wounds has become much clearer in the recent past. Different methods have been proposed to obtain an optimal wound bed in order to perform skin transplantation. Both mechanical and physical debridement procedures un doubtedly offer some advantages and are essential when extensive third degree burns have to be treated. On the other hand, pain, excessive blood loss, and overall broad and indi scriminate debridement are common negative side effects. Thus, many enzymic formulations have been proposed and used in practice by clinicians all over the world.

Interestingly, many of these treat ments are now being rediscovered or re-used in modern-day clinical practice. In addition to the diverse nature of treatments that have been suggested as being beneficial for wounds, over many centuries there has been a range of clinicians who have cared for patients with wounds, and in many parts of the world today it is not doctors but other professional colleagues - e. g. nurses or podiatrists - who are the most knowledgeable persons on wounds and wound treatments. Finally, the nature of wounds seen in clinical practice is changing. Increasing destructive power is resulting in more severe injuries occurring in patients involved in conflict. Developments in surgical techniques have enabled new surgical procedures to be offered to patients, but novel and more severe complications can result from such technical advances. In addition, many societies around the world are having to deal with an increasing ageing population, and huge increases in the number of patients suffering from chronic wounds are being seen. In summary, expanding treatment options, diverse groups of clinicians caring for patients and changes in the nature of wounds seen in clinical practice are real issues that have to be faced, and solutions have to be found if patients are to receive optional care for their wounds.

The work was presented as a first proof in multiple peer reviewed international conferences, in the United Kingdom, United States, most of Europe, Asia, and the Middle East. The pre-requisite for planning new and novel research experiments and coping with future trends in science is a thorough analysis of the present state of knowledge and in finding new ways of thinking and planning. On this basis, new research hypotheses and strategies will have to be built. This book aims to present a short review of key concepts around Dr MacFarland’s research experiments on signal transduction in diverse systems, such as cancer blood brain barrier studies, coronary heart disease, mechanotranduction mechanisms in connective tissues mainly articular cartilage and meniscus with special attention paid to the reactions of these two tissues, as responses in vitro loading model regimens. The current interest in loading stems from how little we know of the aetiology of osteoarthritis a degenerative joint disease, the most crippling of musculoskeletal disorder and diseases. Signal transduction mechanisms are also examined in endothelial and smooth muscle and the role of cholesterol ester transfer protein in coronary heart disease. The object of Dr MacFarland’s scientific and medical research in signal transduction is ambitious, as both scientific research leading to therapeutic remedies needed high ideals setting goals; to give novel relevant results as was the case of the discovery of the first signalling molecule in her research in a unique biological system, as well as the response of connective tissue cells to mechanical stimulus. Therefore, far off as it may be, the prevention of diseases must be the ultimate objective and the overwhelming character of the problems, compels scientists in specific in the age of nano technology to seek cures and create the means of prevention of the disease. Aspects of her studies show the relevance in high impact sports injuries and daily working conditions, whether they degrade or maintain the health of joints in general, and benefit or harm joints and health.
