Book excerpt: For more than two decades, the practice of risk assessment has been applied to human public health issues, and policy makers have used the results of risk assessments in their decision-making process. Approaches for risk assessment have been developed for nonnutrients such as drugs, food additives, and pesticides, but approaches for risk assessment have received less attention in the nutrition area. Some aspects of the risk assessment approach used for nonnutrients are applicable to the assessment of risks related to nutrition. The overall approach, however, must be adapted and modified to take into account the unique aspects of nutrients, including the fact that both high and low nutrient intakes are associated with risk. Experience with the application of a risk assessment process to the setting of upper levels of intake for essential nutrients, for example, has uncovered a number of challenges. Adapting and developing risk assessment strategies for application in nutrition science could lead to improved approaches to the development of dietary and nutritional recommendations and thus is a topic of considerable interest. One nonscientific but overall challenge to nutritional risk assessment relates to increasing and improving communication among experts from key disciplines in ways that could inform the nutritional risk assessment process. Among these key disciplines are nutrition, toxicology, dietary exposure assessment, economics, risk analysis, and epidemiology. How can the perspectives and methods of these diverse fields be brought together to develop more effective approaches for quantitative nutritional risk assessment? How can they be applied to a spectrum of topics related to food and nutrition-micronutrients, macronutrients, dietary supplements, whole foods, food groups, and dietary patterns? How can they help overcome the data challenges that confront nutritional risk assessors? As a step toward improving the communication and sharing methods and information across disciplines, members of the Interagency Risk Assessment Consortium, the U.S. Health and Human Services Office of the Assistant Secretary for Planning and Evaluation, the Institute of Medicine's Food Forum, and the International Life Sciences Institute planned the Nutritional Risk Assessment Workshop. The workshop was held on February 28 and March 1, 2007, in Washington, D.C. This workshop, which was envisioned as one in a series, focused on opening a dialogue to explore the unique questions and challenges faced by nutritionists and the potential use of risk assessment methodologies to answer them. Nutritional Risk Assessment : Perspectives, Methods, and Data Challenges, Workshop Summary summarizes the happenings of this workshop.

Book excerpt: "Health disparities are prevalent in the U.S, with low-income African American children suffering from higher rates of obesity and chronic diseases compared to White children. Currently, little is known about parental perceptions of healthy eating, and concerns related to child health and weight in this at-risk population. The main purpose of this study was to examine perceptions, motivating factors and barriers to healthier eating in a sample of low-income African American parents of 3-5 year old children. The second purpose was to assess parental concerns about health and/or weight and to examine the accuracy of parental perceptions of child weight status. Participants were recruited from Head Start Programs in North Carolina. The Social Cognitive Theory constructs guided focus groups and a survey administered to the participants. Content analysis of 8 focus groups generated the following themes: 1) lack of nutrition knowledge and misconceptions; 2) healthy meals are home-cooked, include meat and starch; 3) family members, lack of maternal modeling, and child pickiness are main barriers to healthier eating; 4) strong awareness of family history of chronic disease, including obesity; 5) lack of concern about child's current weight. Over 25% of mothers underestimated their child's weight status. Our findings highlight important maternal perspectives influencing children's diet quality and long-term health outcomes among low-income at-risk preschoolers. Nutrition educators should be aware that low-income African American mothers may be aware of chronic disease risks, but do not perceive food choices in early childhood as having strong impact on the child's future health."--Abstract from author supplied metadata.

Risk behaviors, such as tobacco use and physical inactivity, modify the result for better or worse. All this takes place in a social, cultural, political and economic environment that can aggravate the health of populations unless active measures are taken to make the environment a health-promoting one. Although this report has taken a disease approach for convenience, the Expert Consultation was mindful in all its discussions that diet, nutrition and physical activity do not take place in a vacuum. Since the publication of the earlier report in 1990 (1), there have been great advances in basic research, considerable expansion of knowledge, and much community and international experience in the prevention and control of chronic diseases. At the same time, the human genome has been mapped and must now enter any discussion of chronic disease. Concurrently there has been a return to the concept of the basic life course, i.e. of the continuity of human lives from fetus to old age. The influences in the womb work differently from later influences, but clearly have a strong effect on the subsequent manifestation of chronic disease. The known risk factors are now recognized as being amenable to alleviation throughout life, even into old age. The continuity of the life course is seen in the way that both under nutrition and over nutrition (as well as a host of other factors) play a role in the development of chronic disease. The effects of man-made and natural environments (and the interaction between the two) on the development of chronic diseases are increasingly recognized. Such factors are also being recognized as happening further and further ''upstream'' in the chain of events predisposing humans to chronic disease. All these broadening perceptions not only give a clearer picture of what is happening in the current epidemic of chronic diseases, but also present many opportunities to address them. The identities of those affected are now better recognized: those most disadvantaged in more affluent countries, and --- in numerical terms far greater --- the populations of the developing and transitional worlds. There is a continuity in the influences contributing to chronic disease development, and thus also to the opportunities for prevention. These influences include the life course; the microscopic environment of the gene to macroscopic urban and rural environments; the impact of social and political events in one sphere affecting the health and diet of populations far distant; and the way in which already stretched agriculture and oceanic systems will affect the choices available and the recommendations that can be made. For chronic diseases, risks occur at all ages; conversely, all ages are part of the continuum of opportunities for their prevention and control. Both under nutrition and over nutrition are negative influences in terms of disease development, and possibly a combination is even worse; consequently the developing world needs additional targeting. Those with least power need different preventive approaches from the more affluent. Work has to start with the individual risk factors, but, critically, attempts at prevention and health promotion must also take account of the wider social, political and economic environment. Economics, industry, consumer groups and advertising all must be included in the prevention equation.

The Dietary Guidelines for Americans (DGA) provide evidence-based recommendations for making healthy dietary choices and preventing chronic disease. Millennials are the largest and most diverse generation in the United States (US). Some research shows that millennials have poor diet quality and there is a dearth of information regarding their adherence to MyPlate recommendations and the factors associated with their self-reported diet quality (SRDQ). Objective: To examine the associations between millennials’ SRDQ, adherence to MyPlate recommendations, food security, consumer behavior and mental health outcomes millennials and to explain the knowledge and perceptions of millennials regarding the DGA and MyPlate recommendations. Methods: Millennials (24-38 years of age) residing within the US were recruited via ResearchMatch.org for a cross-sectional survey-based study. Questions derived from validated national surveys measured demographics, SRDQ, consumer behavior, food security, and mental health. A subset of participants participated in focus groups. Descriptive statistics characterized the sample. Pearson’s Chi-squared tests explored the relationships among SRDQ and variables of interest. Results: Respondents (n = 233) were mostly female (76%), non-Hispanic white (74%), employed (83%), college educated (83%), and one-third of individuals with incomes >$100,000 (32%).
