Book excerpt: The 2001 outbreak of Foot and Mouth was set out in a report published in 2002, providing an analysis of what happened with recommendations (HCP 888, session 2001-02, ISBN 9780102976243). Another FMD outbreak occurred in the Summer of 2007 and the Government again asked Dr Iain Anderson to conduct a review and find out if the lessons of 2001 had been learned and whether new recommendations are needed. This report is built around the findings of 2001, with a particular look at the Pirbright facility, the source of the virus incriminated in this recent outbreak. Dr Anderson's states that he found much to applaud in the handling of the 2007 outbreak along with some deficiencies, but states the positive outweighs the negative. The report commends the strong leadership of the Prime Minister, as well as the contingency planning and better integration of scientific advice and the capabilities at the centre of disease control strategies. External communication was also greatly improved. Less encouraging were the inadequate infrastructure and governance of the Pirbright facility where the Institute for Animal Health is located and the poor regulatory regime in place to licence work with dangerous animal pathogens. Defra's information systems too were found wanting. The report also recommends that the Institute of Animal Health should become the new National Institute of Infectious Diseases, supported by multiple sources of funding from government and elsewhere. Also recommended is the creation of an Independent Advisory Committee on Animal and Emerging Infectious Diseases, combining a high level group of experts along with other stakeholders with a mission to provide independent advice to government and link closely with the Government's and departmental Chief Scientific Advisors and include the Chief Veterinary Office and Chief Medical Officer amongst its members. The publication is divided into five sections: Section 1: Foreword; Section 2: Summary and recommendations; Section 3: Lessons learned, the lessons are - Lesson 1: Maintain vigilance; Lesson 2: Be prepared; Lesson 3: React with speed and certainty; Lesson 4: Explain policies, plans and practices; Lesson 5: Respect local knowledge; Lesson 6: Apply risk assessment and cost benefit analysis; Lesson 7: Use data and information management systems; Lesson 8: Have a legislative framework; Lesson 9: Base policy decisions on the best available science; Section 4: Pirbright; Section 5: Appendices.

Book excerpt: Globalization of the food supply has created conditions favorable for the emergence, reemergence, and spread of food-borne pathogens-compounding the challenge of anticipating, detecting, and effectively responding to food-borne threats to health. In the United States, food-borne agents affect 1 out of 6 individuals and cause approximately 48 million illnesses, 128,000 hospitalizations, and 3,000 deaths each year. This figure likely represents just the tip of the iceberg, because it fails to account for the broad array of food-borne illnesses or for their wide-ranging repercussions for consumers, government, and the food industry-both domestically and internationally. A One Health approach to food safety may hold the promise of harnessing and integrating the expertise and resources from across the spectrum of multiple health domains including the human and veterinary medical and plant pathology communities with those of the wildlife and aquatic health and ecology communities. The IOM's Forum on Microbial Threats hosted a public workshop on December 13 and 14, 2011 that examined issues critical to the protection of the nation's food supply. The workshop explored existing knowledge and unanswered questions on the nature and extent of food-borne threats to health. Participants discussed the globalization of the U.S. food supply and the burden of illness associated with foodborne threats to health; considered the spectrum of food-borne threats as well as illustrative case studies; reviewed existing research, policies, and practices to prevent and mitigate foodborne threats; and, identified opportunities to reduce future threats to the nation's food supply through the use of a "One Health" approach to food safety. Proper disposal of animal carcasses that die or are culled during the outbreak is a key component of a successful response to a disease outbreak because it helps prevent or mitigate the further spread of pathogens and in case of zoonotic disease, to further protect human health. The practical guidelines presented hereby provide carcass and related waste management considerations and recommended procedures for use by Veterinary Services and other official response authorities when developing animal disease outbreak containment and eradication plans. The guidelines apply to animal disease outbreaks of varying sizes, whether the outbreak is isolated to a single premise or spans a region to cover numerous premises. However, they are focused on small to medium-sized holdings in countries without access to engineered landfills, rendering plants or controlled incinerators. The guidelines are written in the spirit of “keep it simple and doable”, considering the limited human and financial resources that many countries are constrained with. Its presentation and practical approach ensure that countries will find it very useful for their emergency operation procedures toolbox. Book excerpt: The Department for Environment, Food and Rural Affairs and its Animal Health agency successfully contained limited outbreaks of Avian Influenza and Foot and Mouth Disease in 2007. The estimated £33 million expenditure by Animal Health in 2007-08 on dealing with these exotic disease outbreaks has represented good value for money when compared to the economic costs of these diseases becoming more widespread. The control of some of the more serious endemic diseases has been managed less successfully. Good progress has been made with the control of Bovine Spongiform Encephalopathy (BSE), Scrapie and Salmonella, but Bovine Tuberculosis has continued to spread. In 2007-08, tackling Bovine Tuberculosis accounted for 39 per cent of Animal Health's total expenditure. Herd restrictions are applied immediately when disease is identified, but compliance with the requirements for routine testing to detect disease is not rigorously enforced. There are no national standards on farm biosecurity to minimise the risk of diseases spreading. The Department, Animal Health and other inspection bodies, such as local authorities, do not systematically collect and share information about biosecurity risks. Beekeepers have reported unusually high losses of honeybees in recent years and, now that the Varroa parasite is endemic, honeybee colonies are more vulnerable to other diseases. Controlling Varroa and monitoring of other diseases is hampered by the limited inspections of colonies carried out by the Department's National Bee Unit. An estimated 20,000 beekeepers are not known to the Unit's inspectors and are less likely to notify the Department of any diseases.

In addition to the number of patients infected with the SARS virus, the disease had profound economic and political repercussions in many of the affected regions. Recent reports of isolated new SARS cases and a fear that the disease could reemerge and spread have put public health officials on high alert for any indications of possible new outbreaks. This report examines the response to SARS by public health systems in individual countries, the biology of the SARS coronavirus and related coronaviruses in animals, the economic and political fallout of the SARS epidemic, quarantine law and other public health measures that apply to combating infectious diseases, and the role of international organizations and scientific cooperation in halting the spread of SARS. The report provides an illuminating survey of findings from the epidemic, along with an assessment of what might be needed in order to contain any future outbreaks of SARS or other emerging infections.

The international workshop examined biosafety and biosecurity issues related to the design, construction, maintenance, and operation of high-containment biological laboratories- equivalent to United States Centers for Disease Control and Prevention biological safety level 3 or 4 labs. Although these laboratories are needed to characterize highly dangerous human and animal pathogens, assist in disease surveillance, and produce vaccines, they are complex systems with inherent risks. Biosecurity Challenges of the Global Expansion of High-Containment Biological Laboratories summarizes the workshop discussion, which included the following topics: Technological options to meet diagnostic, research, and other goals; Laboratory construction and commissioning; Operational maintenance to provide sustainable capabilities, safety, and security; and Measures for encouraging a culture of responsible conduct. Workshop attendees described the history and current challenges they face in their individual laboratories. Speakers recounted steps they were taking to improve safety and security, from running training programs to implementing a variety of personnel reliability measures. Many also spoke about physical security, access controls, and monitoring pathogen inventories. Workshop participants also identified tensions in the field and suggested possible areas for action.

It makes the point that given the spread of the disease before its detection, the impact of the outbreak was bound to be severe, even if everything had been handled perfectly. However there were problems and the initial response was neither fast enough nor effectively co-ordinated. MAFF took the lead but came under severe resource pressure, not only in the number of vets but also in the managerial and logistical skills that had to be later provided by the army. Because disease control policy had not been fully debated before the outbreak, arguments took place whilst the disease was raging and changes were made at short notice and were poorly communicated. In addition the full impact on tourism and the rural economy was not clearly recognised. However in Scotland, with a different management structure and closer relationships between central government, local government and the farming industry, the outbreak was better managed. As well as analysing the conduct of the Government during the epidemic, the report contains a series of recommendations to help ensure that the chances of exotic animal diseases entering the country are reduced, the farming industry is less vulnerable to outbreaks and, that if such outbreaks do occur, the impact is minimised.

Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.
