Book excerpt: "The public health care state has developed as completely decentralized, in collaboration with voluntary organizations, and under the banner of "non-political" scientific agencies. The early history of this system explains how and why public health leaders were able to hide its growth in later periods. Understanding this foundational history is important for three reasons. First, the state-voluntary collaboration shaped the U.S. health care system, leaving it fragmented and unequal. Second, leaders in the public health coalition characterized the state's close collaboration with the voluntary sector as "private provision," abetting the beginning of the American Myth and setting the stage for grow-and-hide. And third, this formative history provides insight as to why the mixture of public and private "has been so ubiquitous in American history as to be almost invisible.""--

Book Voluntary Agencies in the Welfare State

Download or read book Voluntary Agencies in the Welfare State written by Ralph M. Kramer and published by Univ of California Press. This book was released on 2023-04-28 with total page 364 pages. Available in PDF, EPUB and Kindle. Book excerpt: The rise of the welfare state threatens the autonomy and survival of nonprofit voluntary agencies as providers of social services. Or does it? In this cross-national, empirical study of the workings of voluntary agencies, Ralph M. Kramer cuts through the conceptual confusion surrounding voluntarism and the boundaries between the public and private sectors. He draws on a survey of voluntary agencies helping disabled people in four welfare democracies (the United States, England, Israel, and the Netherlands) to explain the virtues and flaws of different patterns of government-voluntary relationships in coping with the growing demand for human services. Kramer concludes that many of the most cherished beliefs about the voluntary sector have little basis in fact. The most innovative agencies, for example, are not the smallest, but rather among the largest, most bureaucratized, and most professionalized. Government funding does not necessarily constrain agency autonomy. And giving voluntary agencies the primary responsibility for social services can reduce, not increase, citizen participation. This comparative analysis of the distinctive competence, vulnerability, and potential of the voluntary agency should replace some of the myths that guide public policy and the day-to-day activities of social service agencies. Book National Health Systems of the World

Download or read book National Health Systems of the World written by Milton I. Roemer and published by Oxford University Press. This book was released on 1993-01-14 with total page 370 pages. Available in PDF, EPUB and Kindle. Book excerpt: This is the companion to the comprehensive review of national health systems presented in Volume I. In that volume, the author analyzed the resources, organization, financing, management, and delivery of health services in 68 countries at diverse levels of economic development and political ideology. In Volume II, the principal issues in health systems across countries are carefully examined. These issues are categorized according to the several components by which national health systems may be analyzed. In the general field of health resources, Roemer discusses physicians and traditional healers, nurses, pharmacists, auxiliary health personnel, the background and distribution of hospitals and health centers, and the production and consumption of drugs. The sections of this important work elucidate the various issues surrounding the world's health systems. The scope and functions of Ministries of Health and social security programs for health care in different types of health systems are reviewed. The book recognizes the contributions of voluntary health agencies, as well as the characteristics of major services in the private sector of national economies. The serious implications of private profit in health systems and the benefits and difficulties of private/public sector relationships are also examined. This authoritative volume presents a global analyzation of economic and management support for national health systems including a discussion of world-wide expenditures according to the source of financing and the purposes for which money is spent. Special attention is also given to experiences in the delivery of health service including ambulatory and primary care, and equivalent analyses are made of world-wide delivery of hospital services, regionalization, and long-term care. Volume II concludes with a review of international health activities from the 19th century International Sanitary Conferences up to the present era of the World Health Organization. This critical work probes the political factors involved in this evolution. The last chapter summarizes major social trends in society, along with major trends in the health system components: resources, programs, economic support, management, and delivery of services.
