A major goal of this textbook is to narrow the gap between the two aforementioned management dichotomies in the virally infected pregnant woman. Weare at the infancy of our understanding of viral infections in pregnancy. The current and anticipated advancements are due in large part to a burgeoning oftechnological achievements in the areas of immunodiagnostics, molecular biology, and pharmacotherapeutics. Our in utero diagnostic capabilities, both invasive and noninvasive, have also allowed us new opportunities to study the effects of various maternal infectious disease processes on the developing fetus. New insights have been recognized pertaining to the maternal-fetal interface, the placenta, in that this structure is now acknowledged to function as both a mechanical and an immunological barrier to vertical transmission of infection. These observations suggest that there will be an outpouring of new data in the next several years that clinicians will need to master to maintain an appropriate level of expertise in the care of their patients.

Book excerpt: Abstract: The treatment and prevention of viral diseases during pregnancy are common concerns for medical providers and pregnant women, yet evidence regarding risks associated with these interventions is sparse. In study 1 we evaluated the association between antiherpetic medication use and the risk of gastroschisis, an abdominal wall birth defect, among women who participated in the National Birth Defects Prevention Study from 1997-2007. The mothers of 0.7% (n=7) of 941 gastroschisis cases and 0.3% (n=22) of 8,339 non-malformed controls reported antiherpetic medication use during the first trimester of pregnancy; after adjustment for confounders, the risk of gastroschisis was twice as high among women using antiherpetic medication during the first trimester compared with those reporting no use of antiherpetics during pregnancy. Our study raises the possibility of a modest adverse effect of either antiherpetic medication use during the first trimester or the herpes infection for which it was presumably indicated. In study 2 we examined the association between seasonal influenza vaccination during pregnancy and the risk of preterm birth ( 37 weeks' gestation) among a retrospective cohort of 2,279 mothers of non-malformed infants who participated in the Slone Birth Defect Study from 2006-2011. One third of women reported influenza vaccination during pregnancy and approximately 7% (n=164) experienced a preterm birth. We observed a null association between influenza vaccination at any time during pregnancy and the risk of preterm birth, after adjustment for confounders. For study 3 we evaluated the association between seasonal influenza vaccination during pregnancy and the risk of small infant size, also among mothers of non-malformed infants participating in the Slone Birth Defect Study from 2006-2011. Their small size and simple structure have made them ame nable to detailed physiochemical analysis, and from this work relatively detailed information has resulted that has signficantly increased our un derstanding of the biology of these viruses. It has become clear that the Parvoviridae are of interest not only for their own sake, but also because their relative simplicity renders them useful probes in the study of the biology of host cells and of other DNA viruses with which they interact. The Dependovirus genus, for instance, contains the defective adeno-as sociated viruses (AA V), which require a coinfection with either an ad enovirus or a herpesvirus for productive multiplication. Studies of AA V, therefore, necessarily impinge on our understanding of the control of macromolecular synthesis by the helper virus. Similarly AA V has been reported to inhibit the oncogenicity of both adeno-and herpesviruses and has been used as a probe of mechanism in these instances as well. Finally, AA V establishes latent infections in vivo and is the only mammalian DNA virus where a comparable model system has been established in cell culture. This system has allowed study of the mechanism of latent infection at the molecular level."
