There are already a few extremely well-written medical textbooks that provide comprehensive, state-of-the-art technical reviews regarding vaccine science. Additionally, in the past decade alone, a number of engrossing, provocative books have been published on various related issues ra- ing from vaccines against specific diseases to vaccine safety and policy. Yet there remains a significant gap in the literature – the history of vaccines. Vaccines: A Biography seeks to fill a void in the extant literature by focusing on the history of vaccines and in so doing, recounts the social, cultural, and scientific history of vaccines; it places them within their natural, historical context. The book traces the lineage – the “biography” – of individual vaccines, originating with deeply rooted medical problems and evolving to an eventual conclusion. Nonetheless, these are not “biographies” in the traditional sense; they do not trace an individual’s growth and development. Instead, they follow an idea as it is conceived and dev- oped, through the contributions of many. These are epic stories of discovery, of risk-takers, of individuals advancing medical science, in the words of the famous physical scientist Isaac Newton, “by standing on the shoulders of giants. ” One grant reviewer described the book’s concept as “triumphalist”; although meant as an indictment, this is only partially inaccurate.

Book excerpt: This report presents the recommendations of a WHO Expert Committee commissioned to coordinate activities leading to the adoption of international recommendations for the production and control of vaccines and other biological substances, and the establishment of international biological reference materials. Following a brief introduction, the report summarizes a number of general issues brought to the attention of the Committee. The next part of the report, of particular relevance to manufacturers and national regulatory authorities, outlines the discussions held on the development and revision of WHO Guidelines for a number of vaccines, blood products and related substances. Specific discussion areas included WHO guidance on the production and evaluation of the quality, safety and efficacy of monoclonal antibodies as similar biotherapeutic products (SBPs); blood and blood components as essential medicines; estimation of residual risk of HIV, HBV or HCV infections via cellular blood components and plasma; snake antivenom immunoglobulins; human pandemic influenza vaccines in non-vaccine-producing countries; and clinical evaluation of vaccines: regulatory expectations. In addition, the following WHO guidance documents were also adopted: WHO manual for the preparation of secondary reference materials for in vitro diagnostic assays designed for infectious disease nucleic acid or antigen detection: calibration to WHO International Standards; and Human challenge trials for vaccine development: regulatory considerations. One WHO addendum document "Labeling information of inactivated influenza vaccines for use in pregnant women" was also adopted. Subsequent sections of the report provide information on the current status, proposed development and establishment of international reference materials in the areas of: biotherapeutics other than blood products; blood products and related substances; cellular and gene therapies; in vitro diagnostics; and vaccines and related substances. A series of annexes are then presented which include an updated list of all WHO Recommendations, Guidelines and other documents on biological substances used in medicine (Annex 1). The above nine WHO documents adopted on the advice of the Committee are then published as part of this report (Annexes 2-10). Finally, all additions and discontinuations made during the 2016 meeting to the list of International Standards, Reference Reagents and Reference Panels for biological substances maintained by WHO are summarized in Annex 11. Book excerpt: The adulteration and fraudulent manufacture of medicines is an old problem, vastly aggravated by modern manufacturing and trade. In the last decade, impotent antimicrobial drugs have compromised the treatment of many deadly diseases in poor countries. More recently, negligent production at a Massachusetts compounding pharmacy sickened hundreds of Americans. While the national drugs regulatory authority (hereafter, the regulatory authority) is responsible for the safety of a country's drug supply, no single country can entirely guarantee this today. The once common use of the term counterfeit to describe any drug that is not what it claims to be is at the heart of the argument. In a narrow, legal sense a counterfeit drug is one that infringes on a registered trademark. The lay meaning is much broader, including any drug made with intentional deceit. Some generic drug companies and civil society groups object to calling bad medicines counterfeit, seeing it as the deliberate conflation of public health and intellectual property concerns. Countering the Problem of Falsified and Substandard Drugs accepts the narrow meaning of counterfeit, and, because the nuances of trademark infringement must be dealt with by courts, case by case, the report does not discuss the problem of counterfeit medicines.

This process is predicated upon a reproducible data base which explains/diagnoses the nature of bony alterations and a secure review of the literature. Thereby hangs the rub. The herpetological literature are difficult to access (let alone read) and are scattered through many prominent and eclectic journals and in the lay literature. While older diagnoses often have not stood the test of time, the clarity of report descriptions usually allows confident identification of the underlying pathology.
