Book excerpt: Contexte : Le diabète de type 1 est une maladie chronique caractérisée par une production insuffisante d'insuline. C'est une pathologie dont la découverte se fait majoritairement chez l'enfant et l'adulte jeune. Son diagnostic est une urgence, afin d'éviter le risque d'acidocétose et de décès. Objectif : l'objectif principal de cette étude était d'évaluer la connaissance des parents sur les signes inauguraux du diabète de type 1 en Ille-et-Vilaine. Nous avons ensuite comparé cela avec les caractéristiques de notre population. Nous voulions également sensibiliser les parents aux autres caractéristiques de cette pathologie, raison pour laquelle d'autres items ont été abordés. Méthode : étude quantitative, descriptive, transversale par auto-questionnaire réalisée entre mars et juin 2022 en Ille-et-Vilaine. Les questionnaires étaient accessibles en ligne via un QR code affiché en salle d'attente des cabinets de médecine générale, pédiatrie, PMI et centres de soins non programmés. Au total, 121 questionnaires ont été analysables. Résultats : la note moyenne concernant les signes inauguraux était de 12,6/20. Une note d'autant plus encourageante que le syndrome polyuro-polydipsique était admis pour la majorité de l'échantillon. Un niveau d'étude important et la connaissance d'un enfant diabétique de type 1 semblaient augmenter significativement cette moyenne. L'urgence d'une consultation le jour même était plutôt mal perçue car la majorité de l'échantillon aurait consulté dans la semaine suivant l'apparition de ces symptômes. Conclusion : cette étude montre une connaissance plutôt bonne des parents concernant les symptômes initiaux du diabète de type 1. Cependant des lacunes persistent comme la méconnaissance de l'énurésie comme symptôme précoce, et le délai long de consultation que les parents accorderaient face à l'apparition de ces symptômes.

Book excerpt: L'objectif était d'évaluer les connaissances des médecins généralistes (MG) d'Ille-et-Vilaine concernant le diagnostic de diabète de type 1 (DT1) chez l'enfant.Méthode : Cette étude quantitative observationnelle soutenue par l'Aide aux Jeunes Diabétiques s'est déroulée fin 2014. Un questionnaire a été posté à 888 MG d'Ille-et-Vilaine. Résultats : Le taux de réponse était de 31%. Tous les MG évoquaient la polyurie polydipsie, 85% la perte de poids et 45% l'énurésie comme symptômes évocateurs du DT1. D'après 65% des MG, le diagnostic de DT1 pouvait se faire sur une simple analyse d'urine. Si le diagnostic était fait au cabinet, 70% déclaraient adresser l'enfant immédiatement aux urgences pédiatriques. Le caractère fréquent de l'acidocétose (ACD) au diagnostic était mal connu et 2/3 des MG citaient un signe non associé au DT1. Conclusion : Les MG devraient être mieux informés des spécificités du DT1 et de l'urgence à la prise en charge hospitalière dans le but de prévenir l'ACD au diagnostic.

Book excerpt: Le diabète est une maladie chronique caractérisée par la présence d'un excès de glucose dans le sang appelé hyperglycémie. On distingue 2 types de diabète : le diabète de type 1, dit insulinodépendant et appelé diabète juvénile, et le diabète de type 2 dit non insulinodépendant et considéré comme diabète de l'adulte. En France, en 2016, on comptait plus de 3,3 millions de personnes traitées pharmacologiquement pour un diabète tous types confondus, soit 5% de la population. Le diabète de type 1 est l'une des maladies chroniques les plus fréquentes chez l'enfant. Il représente environ 10% des cas de diabètes en France et dans le monde. La moitié des cas se déclare avant l'âge de 20 ans. Actuellement en France, l'incidence du diabète de type 1 est d'environ 15 cas pour 100 000 enfants de moins de 15 ans. C'est ainsi qu'entre 2013 et 2015, 6 424 enfants ont été identifiés comme nouvellement atteints de diabète de type 1. Les prévalences chez les jeunes atteints de diabète de type 1 sont donc élevées, soulignant la nécessité de mettre en place des dispositifs de prévention. Les outils d'information existants ne sont pourtant pas adaptés aux enfants et aux adolescents. Afin de mettre en place un outil d'information adapté pouvant être intégré à leur parcours d'éducation thérapeutique, cette thèse avait pour objectif d'évaluer les connaissances des adolescents diabétiques de type 1 et de leurs parents sur l'impact et la gestion des risques bucco-dentaires liés à cette maladie. Enfin, une fiche d'information pédagogique adaptée à ce public a donc été produite et distribuée aux participants à l'étude.
