Book excerpt: Introduction : l'obésité infantile constitue l'un des plus grands défis pour la santé publique du 21eme siècle en raison de sa forte prévalence et des conséquences à l'âge adulte. En France métropolitaine, d'après l'étude ESTEBAN menée de 2014 à 2016, la prévalence du surpoids chez les enfants et adolescents de 6 à 17 ans est estimée à 17 % dont 4 % d'obèses. Les SSR proposent une prise en charge multidisciplinaire pour les enfants en surpoids au cours d'une hospitalisation thérapeutique et éducative. Notre étude porte sur le développement d'une nouvelle structure de soin en région PACA : le SSR Val Pré Vert qui a ouvert ses portes en 2014. L'objectif de notre étude était d'évaluer l'impact de cette hospitalisation sur des adolescents à savoir une modification de leurs habitudes de vie. Matériel et méthodes : dans cette étude épidémiologique prospective, ont été inclus les adolescents âgés de 12 à 18 ans présentant un IMC>97eme percentile et hospitalisés sur l'année 2016-2017. Les données psychopathologiques, pondérales, physiques, nutritionnelles et socio-familiales en début et fin de prise en charge ont été extraites à partir de dossiers informatisés. Les données à distance (≥ 1 an) ont été récoltées à l'aide d'un questionnaire proposés aux adolescents et à leur famille. Book excerpt: Introduction : L'obésité de l'enfant et de l'adolescent est aujourd'hui un problème de santé publique. La prise en charge de l'obésité infantile doit s'articuler autour d'une rééducation diététique, d'une remise à l'activité physique et d'une prise en charge psychologique. Le centre Marc Sautelet, centre de soins de suite et rééducation pédiatrique situé à Villeneuve d'Ascq, effectue cette prise en charge multidisciplinaire depuis 2010. Néanmoins, à 2 ans du séjour, 70% des adolescents ont arrêté le suivi. L'objectif de cette étude est d'évaluer les raisons ayant entrainé cet arrêt de suivi ainsi que le devenir des adolescents ayant bénéficié d'un séjour de prise en charge de l'obésité. Méthodes : Un questionnaire de 12 questions portant notamment sur leur suivi, leurs raisons d'arrêt de suivi le cas échéant et leur devenir pondéral à 2 ans, était posé par téléphone, sur entretien dirigé, auprès des adolescents ayant bénéficié d'un séjour de prise en charge de l'obésité au centre Marc Sautelet dans la période de septembre 2014 à aout 2017. Les patients étaient considérés comme « perdus de vus » après 10 appels infructueux ou refus de consentement. Résultats : 76 adolescents étaient interrogés, soit 62,3% des adolescents ayant bénéficié d'un séjour sur la période étudiée. Le suivi auprès du centre Marc Sautelet avait été interrompu avant 2 ans dans 72,4% des cas. Les raisons motivant l'arrêt du suivi étaient principalement un manque de motivation à la poursuite de perte de poids chez 27,8% des patients en arrêt de suivi et des problèmes d'ordre familial ou personnel chez 25,9% des sujets en arrêt de suivi. On notait dans l'étude de meilleurs résultats concernant le suivi des mesures hygiénodiététiques, l'activité physique actuelle et la diminution du Z-score de l'Indice de Masse Corporelle chez les patients ayant poursuivi leur suivi au-delà de deux ans (Z-score : - 0,105 vs - 0,073 ; p = 0,028) Conclusion : Cette étude montre qu'il est nécessaire de suivre l'enfant obèse sur le plan psychologique afin d'entretenir la motivation. Elle montre aussi qu'un suivi prolongé améliore les résultats de la prise en charge.

Book excerpt: En Ille-et-Vilaine, le centre de soins de suite et de réadaptation Rey-Leroux, propose une hospitalisation thèrapeutique et éducative aux adolescents en situation d'obésité sévère, dans le but de modifier durablement les habitudes de vie afin de diminuer la corpulence, et ainsi limiter l'incidence des comorbidités et améliorer la qualité de vie de l'adolescent. Néanmoins le centre n'organise pas de consultation de suivi post séjour, on ne peut donc pas juger de l'efficacité à terme de cette intervention. Cette étude observationnelle a été réalisée, auprès des 194 adolescents pris en charge au centre entre septembre 2007 et avril 2014, afin d'évaluer le devenir de ces adolescents à distance du séjour. Le recueil des données était rétrospectif par l'intermédiaire de questionnaires envoyés à l'adolescent. Le taux de réponse après relance téléphonique était de 42,8 % pour un délai médian depuis la fin du dernier séjour de 3,2 ans. La majorité des sujets (59%) n'ont pas de suivi médical alors même que 60 % se sentent exposés à des problèmes de santé. L'évolution de la corpulence était évaluée par le Z-score IMC. Celle-ci était maintenue dans 22 % des cas, diminuée dans 30 % des cas et augmentée dans 36 % des cas (10 données manquantes). Le sexe féminin (p=0,03), une durée de séjour longue (p=0,04), le fait d'avoir une activité physique faible (p=0,047) et certains comportements alimentaires sont associés à une reprise pondérale. La présence d'un suivi post séjour n'influence pas l'évolution de la corpulence. Le niveau de corpulence est significativement associé à une qualité de vie liée à la santé moins bonne (p=0,0003)

Instances of these impairments are expected to double by the year 2050, making low vision a significant public health issue. This Practice Guideline includes occupational therapy assessment and intervention guidelines for older adults who have visual acuity impairments, visual field impairments, or both as a result of a low vision diagnosis.
