Book excerpt: Objectifs Déterminer les valeurs normales des dimensions de l’oreillette gauche (OG) et des paramètres de la courbe de remplissage du ventricule gauche (VG) en fonction de l’âge et du sexe. Matériels et méthodes Dans cette étude prospective unicentrique, 173 volontaires sains ont été inclus et divisés équitablement en sept groupes allant de 18 à 70 ans. Tous les volontaires ont passé une IRM cardiaque avec des séquences CINE en coupes grand axe et petit axe du VG. Les images ont été analysées afin de déterminer la planimétrie et la volumétrie de l’OG par la méthode biplan, ainsi que les paramètres de la courbe de remplissage du VG comprenant les pics de remplissage rapide et tardif (PRR et PRT respectivement). Résultats Le PRR diminuait significativement avec l’âge alors que le PRT augmentait significativement. Ainsi, le rapport PRR/PRT décroissait significativement et progressivement avec l’âge et passait de 2.2±0.5 chez les volontaires les plus jeunes à 0.8±0.3 chez les plus âgés. La taille de l’OG ne se modifiait pas de façon significative avec l’âge et la moyenne était de 22.0±4.4 cm2 et 36.3±10.4 mL/m2 dans la population globale de l’étude. Des différences significatives entre les sexes ont été mises en évidence pour la plupart des paramètres étudiés. Conclusion Chez les volontaires sains, la taille de l’OG reste stable avec l’âge pendant que les paramètres de la courbe de remplissage changent significativement. Nous avons fourni des valeurs de références an fonction de l’âge et du sexe, ce qui peut être utile pour une application en routine.

Book excerpt: Objectifs : 1.Évaluer la variabilité de la mesure des paramètres fonctionnels du ventricule droit (VD) en IRM cardiaquechez les patients atteints d'une cardiopathie acquise, avec un accent particulier sur l'effet de l'expérience de l'observateur et des étapes du post-traitement. 2.Évaluer la précision diagnostique et la variabilité de la mesure de 3 paramètres semi-quantitatifs comme outil d'évaluation de la fonction systolique du VD : TAPSE = Excursion systolique du plan de l'anneau tricuspide ; RVFS = Fraction de raccourcissement du VD; RVFAC = Fraction de changement de surface du VD. Matériels et méthodes : Soixante patients consécutifs atteints de cardiopathie acquise ayant une indication clinique à la réalisation d'une IRM cardiaque ont été inclus de façon prospective. Les volumes, fraction d'éjection et masses VD et VG ont été calculés avec des séquences CINE de type balanced steady state free precession. Les paramètres semi quantitatifs ont été calculés sur une coupe ciné 4-cavités. Toutes les analyses ont été effectuées 2 fois par 3 observateurs ayant des niveaux d'expérience différents en IRM cardiaque. La variabilité intra et inter observateur a été évaluée par la méthode de Bland-Altman, le coefficient de variation et le coefficient de corrélation intraclasse. L'effet des étapes de post-traitement suivantes sur la variabilité des paramètres quantitatifs a été évalué: expérience des observateurs, choix des coupes basale et apicale, choix de la phase télésystolique, et contourage de l'observateur. Une corrélation entre les paramètres semi-quantitatifs et la FEVD et une analyse Receiver Operating Characteristic (ROC) ont été réalisés pour chaque paramètre semi-quantitatif. Résultats : Le temps de segmentation moyen variait de 9.8 à 19.0 min pour VD et de 6.4 à 9.2 min pour le VG. La variabilité de la mesure des paramètres fonctionnels du VD en IRM était fortement influencée par l'expérience préalable de l'observateur, le choix de la coupe basale et le contourage de l'observateur. L'évaluation de la fonction VD en IRM restait 2 fois plus longue et 2 à 3 fois plus variable que pour le VG. La corrélation entre le RVFAC et la FEVD était bonne pour tous les observateurs et peu dépendante de l'expérience préalable de l'observateur. Au contraire, le RVFS et le TAPSE étaient moins corrélés avec la FEVD et dépendants de l'expérience préalable. L'analyse ROC a démontré qu'un RVFAC

Book excerpt: The partition of fluid between the vascular and interstitial compartments is regulated by forces (hydrostatic and oncotic) operating across the microvascular walls and the surface areas of permeable structures comprising the endothelial barrier to fluid and solute exchange, as well as within the extracellular matrix and lymphatics. In addition to its role in the regulation of vascular volume, transcapillary fluid filtration also allows for continuous turnover of water bathing tissue cells, providing the medium for diffusional flux of oxygen and nutrients required for cellular metabolism and removal of metabolic byproducts. Transendothelial volume flow has also been shown to influence vascular smooth muscle tone in arterioles, hydraulic conductivity in capillaries, and neutrophil transmigration across postcapillary venules, while the flow of this filtrate through the interstitial spaces functions to modify the activities of parenchymal, resident tissue, and metastasizing tumor cells. Likewise, the flow of lymph, which is driven by capillary filtration, is important for the transport of immune and tumor cells, antigen delivery to lymph nodes, and for return of filtered fluid and extravasated proteins to the blood. Given this background, the aims of this treatise are to summarize our current understanding of the factors involved in the regulation of transcapillary fluid movement, how fluid movements across the endothelial barrier and through the interstitium and lymphatic vessels influence cell function and behavior, and the pathophysiology of edema formation. Book excerpt: The heart and lung are intricately linked. When the heart is affected by disease, the lungs will often show some related pathological or clinical conditions and vice versa. Pulmonary heart disease is by definition a condition when the lungs cause the heart to fail. The left ventricle in combination with the other structures in the “left heart” pumps blood throughout the body. The right ventricle (and structures of the “right heart”) pumps blood to the lungs where it is oxygenated and returned to the left heart for distribution. In normal circumstances, the right heart pumps blood into the lungs without any resistance. The lungs usually have minimal pressure and the right heart easily pumps blood through. However when there is lung disease present, like emphysema, chronic obstructive lung disease (COPD) or pulmonary hypertension- the small blood vessels become very stiff and rigid. The right ventricle is no longer able to push blood into the lungs and eventually fails. This is known as pulmonary heart disease. Pulmonary heart disease is also known as right heart failure or cor pulmonale. The chief cause of right heart failure is the increase in blood pressure in the lungs (pulmonary artery).
