Book excerpt: Contexte : En 2012, environ 1% de la population française était traitée par anti-vitamine K (AVK). Les accidents hémorragiques iatrogènes sont fréquents et les AVK constituent la première cause d'hospitalisation iatrogène en France. Une maîtrise insuffisante du traitement augmente le risque iatrogène, risque pouvant être limité par un programme éducatif. L'objectif était d'évaluer les connaissances des patients sous AVK, puis de pointer des pistes d'amélioration. Méthode : Cette étude quantitative transversale multicentrique a été réalisée par diffusion d'un auto-questionnaire dans des cabinets de médecine générale d'Ille-et-Vilaine tirés au sort. Résultats : 149 patients ont été inclus. 93,3% nommaient l'INR comme marqueur de suivi biologique. Seule la gestion du traitement par le patient impactait positivement la connaissance de cet item. L'indication, l'objectif du traitement, la fréquence de surveillance, les bornes de l'INR, l'automédication et le régime alimentaire étaient respectivement maîtrisés par 68,5%, 66,4%, 76,5%, 69,1%, 98% et 71,1%. Près de la moitié des patients ignoraient que le traitement permet d'éviter un accident vasculaire cérébral. La participation à un programme d'éducation thérapeutique était faible (13,8%) ainsi que l'utilisation du carnet de suivi AVK (32,2%). Conclusion : La connaissance globale des patients concernant leur traitement par AVK est insuffisante. L'analyse a permis de faire ressortir les connaissances et les lacunes ; lacunes qui ont permis ensuite de mieux cibler les besoins d'éducation thérapeutique ainsi que des pistes d'amélioration de la prescription du médecin traitant afin de mieux la sécuriser.

Les AVK sont la première cause d’accidents iatrogènes graves. De manière concomitante, on observe un manque de connaissance des patients sur ce traitement efficace, mais dangereux. Ce fait représente une des causes principales de la survenue des ces accidents. Dans notre étude portant sur 40 patients hospitalisés dans l’Unité d’Hospitalisation de Courte Durée du CHU de Caen, la moitié des patients connaissait le nom de leur traitement, 52,6% en ignorait l’indication, 89,2% ne connaissait pas le temps d’action de leur médicament, 65,8% ne savait pas quel dosage sanguin permet le suivi de l’efficacité de leur traitement et 71,1% ne connaissait pas la zone cible de l’INR. Lors de l’initiation d’un traitement par AVK, l’accent n’est pas suffisamment mis sur l’éducation thérapeutique, les patients n’ont alors qu’une connaissance partielle et insuffisante de leur traitement. Dans de nombreux pays, les deux axes choisis pour diminuer l’incidence des accidents sont l’accès à l’automesure de l’INR et l’ouverture de cliniques d’anticoagulants. La France est très en retard sur l’implantation des cliniques d’anticogulants, dont le principe de fonctionnement serait tout à fait adaptable au système de soin français. Le retard est tout aussi important pour les appareils d’automesure de l’INR, qui outre leur utilité en ambulatoire, pourraient occuper une place essentielle dans les services d’urgence où ils auraient un intérêt majeur dans la détermination de l’INR en urgence : traitement fibrinolytique des accidents vasculaires cérébraux, prise en charge d’un accident hémorragique sous AVK.

Ils exposent les patients à deux risques principaux : l'hémorragie ou la thrombose. Pour limiter ces risques, les pharmaciens sont autorisés à proposer des entretiens pharmaceutiques à ces patients. Nous avons souhaité évaluer l'impact de ces entretiens sur le savoir, savoir-faire et savoir être des patients de plus de 75 ans vis-à-vis de leur traitement AVK. Méthode : Cette étude épidémiologique observationnelle a été réalisée sur une cohorte de 30 patients à l'hôpital et de 30 patients à l'officine. Un questionnaire de 20 questions a été posé à nos patients à 3 reprises : En pré-entretien, en post entretien précoce (dans la journée de l'entretien) et en post entretien tardif (1 mois après l'entretien). L'entretien était adapté aux lacunes constatées lors du questionnaire réalisé avant l'entretien. Une analyse des 2 INR précédents et suivants l'entretien a été également réalisé. Résultats : La cohorte en Officine était composée à 53% de patients de plus de 80 ans et d'une majorité de femmes (60%). La cohorte à l'hôpital était composée à 73% de patients âgés de plus de 80 ans et également d'une majorité de femmes (53%). En Officine, les notes obtenues au questionnaire juste après l'entretien étaient meilleures qu'avant l'entretien (p=4,73 E-15). De plus les notes obtenues 1 mois plus tard n'ont pas diminuées (p= 0,81). A l'hôpital, les notes obtenues au questionnaire juste après l'entretien étaient meilleures qu'avant l'entretien (p=1,2 E-12) et les notes obtenues 1 mois plus tard n'ont pas diminuées (p=0,10). Nous n'avons pas constaté l'impact de l'entretien sur l'INR des patients ayant un INR cible entre 2 et 3, que ce soit en Officine (p= 0,25) ou à l'hôpital (p= 0,27). Conclusion : D'après notre étude, à l'hôpital comme à l'officine, l'entretien pharmaceutique a permis une amélioration significative des connaissances des patients vis-à-vis de leur traitement AVK. Cette amélioration est plus marquée en Officine qu'à l'hôpital. Cependant certaines notions restent difficilement assimilées ce qui justifie le fait d'insister sur ces notions lors d'un entretien pharmaceutique ultérieur.

Ce traitement est responsable de plus de 4000 décès par an. L’insuffisance d’éducation thérapeutique des patients est aujourd’hui considérée comme un facteur de risque non négligeable de complication. Objectif : Etudier les patients qui appellent le Centre 15 du 77 pour un problème lié à leur traitement, et comparer leur niveau d’éducation thérapeutique à celui de patients non sélectionnées dans des cabinets de médecine générale. Méthode : Etude descriptive prospective à l’aide d’un questionnaire proposé par l’AFSSAPS, sur le niveau d’éducation thérapeutique des patients. Analyse complémentaire des motifs d’appels au 15, et de la relation des patients avec leur médecin traitant. Résultats : 22 patients ayant appelé le 15 ont été comparés à 17 patients de médecine générale. Les patients qui appellent le 15 ont un niveau de connaissance inférieur et un traitement anticoagulant plus récent. Les appels au 15 sont reliés à une indisponibilité du médecin traitant, et celui ci n’est pas le prescripteur du traitement. Discussion : Les patients sous traitement antivitamine K qui appellent le 15 pour un problème lié à ce traitement sont une population à risque. Ces patients devraient faire l’objet d’un suivi particulier afin de réduire le risque de complications liées au traitement anticoagulant.

Résultats : 325 Adhérents, 312 Non-adhérents, 152 médecins, et 202 pharmaciens ont été inclus. Les patients adhérents étaient âgés en moyenne de 76 ans, plutôt citadins, seuls, et de niveau scolaire élevé. Cette étude, réalisée au sein du Groupement Hospitalier Est de Bron (HCL), a pour but d'évaluer le niveau de connaissance de patients hospitalises, pour lesquels un traitement AVK a été instaure, et ayant ainsi bénéficie d'un entretien pharmaceutique lors de leur séjour. Le niveau de connaissance a été évalué deux semaines après leur sortie par un entretien téléphonique semi-directif. Parallèlement à cette expérimentation, l'implication de l'équipe officinale lors de la première délivrance du traitement au patient a été appréciée lors de ce même entretien. Ceci a permis de savoir si le nouveau traitement a été expliqué au patient mais aussi de savoir si un entretien pharmaceutique lui a été proposé à l'officine. Les patients ayant eu un entretien à l'hôpital et une explication de la part de l'équipe officinale possèdent de meilleures connaissances que les patients ayant bénéficié uniquement de l'entretien pharmaceutique a l'hôpital. Le nombre d'entretiens pharmaceutiques AVK propose par les équipes officinales apparait faible par rapport au nombre de pharmacies ayant intégré ce dispositif.
