Un risque accru de fracture a été observé chez les patients diabétiques, alors que la DMO est 5 à 10% plus élevée chez les patients atteints de DT2 par rapport aux sujets sains. Nous avons utilisé la tomographie quantitative périphérique à haute résolution (HR-pQCT) pour caractériser les paramètres micro-architecturaux corticaux et trabéculaires, chez les DT2 comparé à des sujets sains. Nous avons conduit une étude cas-témoin nichée au sein de 3 cohortes prospectives, réunissant ainsi 507 participants, hommes et femmes inclus. Chaque cas était apparié à 2 témoins, sur le sexe, l'âge et l'IMC. Les hommes DT2 ont une densité minérale osseuse augmentée de façon significative au niveau du col fémoral (0.83 ± 0.13 vs 0.79 ± 0.14 g/cm2). Après ajustement sur la médiane d'évolution du diabète, on observe une aire totale au niveau du radius et du tibia diminué de façon significative. Parmi les femmes, au niveau du radius, l'aire corticale est significativement plus grande chez les DT2 (44.35 ± 9.26 vs 41.17 ± 10.05 mm2), de même pour la densité corticale qui est plus haute (826.6 ± 70.21 vs 804.37 ± 72.72 mg HA/cm3). In this classic of French cultural studies, Georges Didi-Huberman traces the intimate and reciprocal relationship between the disciplines of psychiatry and photography in the late nineteenth century. Focusing on the immense photographic output of the Salpetriere hospital, the notorious Parisian asylum for insane and incurable women, Didi-Huberman shows the crucial role played by photography in the invention of the category of hysteria. Under the direction of the medical teacher and clinician Jean-Martin Charcot, the inmates of Salpetriere identified as hysterics were methodically photographed, providing skeptical colleagues with visual proof of hysteria's specific form. These images, many of which appear in this book, provided the materials for the multivolume album Iconographie photographique de la Salpetriere. As Didi-Huberman shows, these photographs were far from simply objective documentation. The subjects were required to portray their hysterical "type"—they performed their own hysteria. Bribed by the special status they enjoyed in the purgatory of experimentation and threatened with transfer back to the inferno of the incurables, the women patiently posed for the photographs and submitted to presentations of hysterical attacks before the crowds that gathered for Charcot's "Tuesday Lectures." Charcot did not stop at voyeuristic observation. Through techniques such as hypnosis, electroshock therapy, and genital manipulation, he instigated the hysterical symptoms in his patients, eventually giving rise to hatred and resistance on their part. Didi-Huberman follows this path from complicity to antipathy in one of Charcot's favorite "cases," that of Augustine, whose image crops up again and again in the Iconographie. Augustine's virtuosic performance of hysteria ultimately became one of self-sacrifice, seen in pictures of ecstasy, crucifixion, and silent cries.

Book excerpt: L'approche psychologique de la maladie est devenue un critère de qualité de l'éducation thérapeutique des malades chroniques, rendant indispensable l'identification des représentations et du vécu du patient vis-à-vis de sa maladie. C'est l'objet de cette étude préliminaire, réalisée auprès de 13 diabétiques de type 2, au travers d'entretiens semi-directifs. Leur analyse thématique rend compte de la difficulté d'acceptation de la maladie. Le patient ne se sent pas malade et ne perçoit pas la gravité de son diabète, le“ vrai diabète”étant associé à un traitement par insuline. La prise en charge est vécue comme une contrainte, qui de plus l'isole de son entourage. Les stratégies d'ajustement sont souvent centrées sur l'évitement, mais aussi sur la recherche de soutien social, grâce auquel il peut faire face à la maladie et questionner ses schémas de santé. Le soignant, dans son action éducative, doit tenir compte de ces facteurs psychologiques pour amener le patient vers une acceptation de la maladie et de sa responsabilité dans la prise en charge.

Elle comprenait l'exploration de la perspective du patient, une demarche explicative, des conseils hygieno-dietetiques adaptes, un objectif de comprehension commune avec le medecin et une evaluation a 3 mois des resultats. En confrontant les donnees du discours avec les modeles transtheorique et transactionnel par procedure de triangulation theorique, cette demarche a corrobore une dynamique d'adaptation coping, des processus experientiels et comportementaux favorisant ou pas des changements. L'etude a illustre l'interaction symbolique entre medecins et patients a travers les representations personnages que chacun se fait de l'autre, bloquant ou facilitant la comprehension commune. L'ACP est apparu comme un processus therapeutique systemique adapte a l'education therapeutique."

However, they also stress the importance of certain “can’t miss” diagnoses such as Wilson’s disease, dopa-responsive dystonia, and Whipple’s disease in which delayed diagnosis in less emergent situations can lead to slowly evolving and often irreversible neurological damage with tragic consequences.
