Il ne s'agit pas d'une simple réédition du précédent ouvrage Occlusion et fonction publié en 2002. Les auteurs proposent ici une réflexion plus aboutie sur le rôle du calage/guidage dentaire fonctionnel et les techniques d'équilibration occlusale. La physiologie de la mastication et de la déglutition est développée dans la première partie, afin de comprendre les carences du modèle de fonctionnement habituel, imaginé il y a près d'un siècle, à partir d'un concept géométrique simplifié des mouvements mandibulaires. Le rôle des faces occlusales des dents postérieures pendant la mastication est expliqué. Il en résulte que les techniques occlusales habituelles ne permettent pas de vérifier l'intégralité des guidages fonctionnels des dents postérieures, ce qui présente le risque d'oublier des malocclusions. La réalisation prothétique et l'équilibration des dents naturelles et des implants doivent chercher à préserver et/ou à rétablir cette enveloppe fonctionnelle de guidage occlusal propre à chaque patient. Cela implique : de privilégier la simulation de la mastication par le patient, ou à l'aide de moyens externes, comme certains articulateurs ; de respecter ou reconstruire les cuspides et leurs versants de guidage par des techniques d'addition et/ou de soustraction. L'application de ces modalités techniques représente une voie privilégiée de prévention et de traitement des dysfonctions dento-articulaires. La dernière partie, qui traite de la biomécanique des implants, est nouvelle. Les critères de choix des implants, les protocoles et les résultats de la prothèse immédiate unitaire sur implants y sont largement développés, avec un recul supérieur à 5 ans. Ces propositions représentent une voie thérapeutique et holistique nouvelle dans l'approche de l'occlusion.

In contrast to the detailed paragraphs dealing with, for example, enamel and dentine, the section (about 40 pages and 20 illustra tions, mostly drawings) devoted to periodontal tissues failed to provide a factual review and summary of the contemporary knowledge and latest developments in research on the various components of the periodontium. Instead, much of the text was an attempt to arrive at conclusions from often purely semantic speculations, playing the various schools of thought against each other, provid ing arguments in favor of the authors' views and arguments for the feasibility and probability of accepting or rejecting the often diverse opinions, while the reader was referred to the already existing literature for factual details. Since 1936, however, factual details ofthe structural biology of the periodon tal tissues, i. e. their development, structure, function, and physiology, have been greatly extended and have been internationally accepted. With much less opin ionated belief to cope with, this knowledge has formed the solid foundation upon which diagnosis, prevention, and treatment in the fields of clinical perio dontology, modem orthodontics, and re- and transplantation procedures of teeth have been built.
