At the recently-concluded Extraordinary APRM Forum Meeting of participating heads of state and government in Cotonou, Benin on 25-26 October 2008, Nigeria and Burkina Faso brought to nine the total number of countries "peer reviewed". All participating countries have had to struggle with the long, complex and sometimes confusing and intimidating Self-Assessment Questionnaire covering key issues in four thematic areas: democracy and political governance, economic governance, corporate governance, and socio-economic development. Answering these questionnaires risks becoming a highly technical exercise, accessible only to experts and elites. But the APRM also requires that the voices and views of ordinary citizens be heard and incorporated. One practice that is becoming standard is conducting nationally representative opinion surveys among the population.

Professor Anyang' Nyong'o is a renowned reformist and political scientist in Africa and is best known for his role in Kenya's "second liberation." He holds a doctorate degree in Political Science from the University of Chicago and has taught in universities in Kenya, Mexico and Ethiopia. Upon the re-introduction of multiparty politics in Kenya in 1991, he was involved in the founding of Forum for the Restoration of Democracy (FORD), which provided the premier opposition machinery in the run-up to the 1992 general elections. He was also involved in the formation of the National Rainbow Coalition (NARC), which defeated KANU, the party that had ruled Kenya for 24 years. In the subsequent NARC government, he became the Minister of Planning and National Development. Besides teaching, he is widely published in Africa and abroad. Prof. Nyong'o has also been at the frontline in championing the reform agenda in Africa, especially through the establishment of NEPAD. At the time of publication of this book, he was Secretary General of the Orange Democratic Movement (ODM).

Book excerpt: Estimates indicate that as many as 1 in 4 Americans will experience a mental health problem or will misuse alcohol or drugs in their lifetimes. These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. It offers a set of conclusions and recommendations about successful stigma change strategies and the research needed to inform and evaluate these efforts in the United States.

Parenting Matters identifies parenting knowledge, attitudes, and practices associated with positive developmental outcomes in children ages 0-8; universal/preventive and targeted strategies used in a variety of settings that have been effective with parents of young children and that support the identified knowledge, attitudes, and practices; and barriers to and facilitators for parents' use of practices that lead to healthy child outcomes as well as their participation in effective programs and services. This report makes recommendations directed at an array of stakeholders, for promoting the wide-scale adoption of effective programs and services for parents and on areas that warrant further research to inform policy and practice. Care Without Coverage examines the real consequences for adults who lack health insurance. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Systematic reviews can be helpful for clinicians who want to integrate research findings into their daily practices, for patients to make well-informed choices about their own care, for professional medical societies and other organizations that develop clinical practice guidelines. There are no universally accepted standards for developing systematic reviews leading to variability in how conflicts of interest and biases are handled, how evidence is appraised, and the overall scientific rigor of the process.
